Chemotherapy for metastatic and unresectable skin melanoma is still a challenge. Efficiency of one-component regimens (monochemotherapy) does not exceed 10-15 %. Unfortunately, some improvement in progression-free survival observed during polychemotherapy has no effect on overall survival of patients. A promising direction of increasing the effectiveness of therapy and overall survival of patients with metastatic melanoma includes the individualization of drug treatment, with the determination of the molecular-genetic changes in the tumor and the treatment based on these results. This article reviews current treatment options with new promising targeted drugs.</p></abstract><trans-abstract xml:lang="ru"><p>Метастатическая меланома кожи характеризуется неблагоприятным прогнозом заболевания. Химиотерапия при метастатической и неоперабельной меланоме кожи до сих пор остается непростой задачей. Эффективность однокомпонентных режимов (монохимиотерапия) не превышает 10-15%. Некоторое улучшение выживаемости без прогрессирования, наблюдаемое при проведении полихимиотерапии, к сожалению, не оказывает влияния на общую выживаемость пациентов. Перспективным направлением увеличения эффективности терапии и общей продолжительности жизни больных метастатической меланомой кожи является индивидуализация лекарственного лечения, включающая определение молекулярно-генетических изменений в опухоли и назначение терапии с учетом полученных результатов.
