Through the example of Abobotulinumtoxin A, for which extended indications for use in ICP patients have been obtained in 2017, an optimal approach to the choice of targeted muscles, the calculation of the total dose per procedure and dose for each muscle and injection site, are discussed, as well as the need for repeated injections, the optimal intervals between injections and other aspects, taking into account the child body weight, the degree of spasticity, and the level of development of motor skills. Also, the article presents two clinical observations as examples of determining indications for injecting Abobotulinumtoxin A and choosing the optimal scheme.</p></abstract><trans-abstract xml:lang="ru"><p>В статье рассматривается проблема многоуровневого подхода к проведению инъекций препаратами ботулинического токсина типа А (БТА) для коррекции спастичности при детском церебральном параличе (ДЦП). На примере Abobotulinumtoxin A, для которого в 2017 г. получены расширенные показания к применению пациенами с ДЦП, обсуждаются оптимальный подход к выбору мышц-мишеней, расчет общей дозы на процедуру и дозы для каждой мышцы и точки инъекции, необходимость повторных введений, оптимальные интервалы между ними и другие аспекты с учетом массы тела ребенка, степени спастичности, уровня развития двигательных навыков.
