The pathogenetic mechanisms of DM2 are discussed. The important role of insulin resistance as a key element in the development of DM2 is emphasized. The molecular mechanisms of action of metformin are considered in detail; its glycemic and extraglycemic effects, and the place of this drug in modern therapy of type 2 diabetes are analyzed.</p></abstract><trans-abstract xml:lang="ru"><p>Заболеваемость сахарным диабетом 2 типа (СД2) неуклонно растет во всем мире, а эффективность контроля гликемии при этом заболевании все еще недостаточна. Обсуждаются патогенетические механизмы СД2. Подчеркивается важная роль инсулинорезистентности как ключевого звена развития СД2.
