It is necessary to search for available methods for screening low muscle mass.</p> <p><bold>Objective: </bold>Determination of the sensitivity and specificity of calf circumference measurement as a surrogate method for assessing muscle mass in comparison with the «gold standard» – dual-energy X-ray absorptiometry (DXA) in women with RA.</p> <p><bold>Materials and methods:</bold> 201 women with confirmed RA who signed informed consent were examined. Clinical and laboratory examination was performed, including calf circumference measurement and determination of body composition with calculation of appendicular muscle index (AMI) using DXA.</p> <p><bold>Results: </bold>The mean age of the examined women was 59.3±9.0 years. Low AMI was detected in 18.4% of patients. Calf circumference positively correlated with AMI (r=0.52, p&lt;0.001), regardless of the presence of obesity. ROC analysis revealed the value of calf circumference (33.8 cm) with the optimal ratio of sensitivity (73.0%) and specificity (76.2%) for detecting low muscle mass. The area under the curve was 0.789 (95% confidence interval 0.726–0.844, p&lt;0.001).</p> <p><bold>Conclusion: </bold>Calf circumference values are not sensitive enough for screening low muscle mass. It is necessary to search for additional factors to increase the sensitivity of the method without significantly reducing specificity.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Обоснование: </bold>Вторичная саркопения (СП) при ревматоидном артрите (РА) не диагностируется из-за сложности анализа композиционного состава тела и малой информативности определения мышечной силы из-за суставного синдрома. Необходим поиск доступных методов скрининга низкой мышечной массы.</p> <p><bold>Цель исследования: </bold>определить чувствительность и специфичность измерения окружности голени в качестве суррогатного метода оценки мышечной массы в сравнении с «золотым стандартом» – двухэнергетической рентгеновской абсорбциометрией (DXA) у женщин с РА.</p> <p><bold>Материалы и методы: </bold>Обследована 201 женщина с достоверным РА, подписавшая информированное согласие. Проведено клинико-лабораторное обследование, включавшее измерение окружности голени и определение композиционного состава тела с расчетом аппендикулярного мышечного индекса (АМИ) с помощью DXA.</p> <p><bold>Результаты: </bold>Средний возраст обследованных женщин составил 59,3±9,0 лет. Низкий АМИ выявлен у 18,4% пациенток. Окружность голени положительно коррелировала с АМИ (r=0,52, р&lt;0,001), независимо от наличия ожирения. В ходе ROC-анализа установлена величина окружности голени (33,8 см), обладающая оптимальным соотношением чувствительности (73,0%) и специфичности (76,2%) для выявления низкой мышечной массы. Площадь под кривой составила 0,789 (95% доверительный интервал [ДИ]: 0,726–0,844, р&lt;0,001).</p> <p><bold>Заключение: </bold>показатели окружности голени обладают недостаточно высокой чувствительностью для скрининга низкой мышечной массы.
