Аннотация

В большинстве стран мира отмечается рост заболеваемости и смертности от первичных злокачественных опухолей печени, из них гепатоцеллюлярная карцинома (ГЦК) диагностируется более чем у 90 % пациентов. При соблюдении Миланских критериев резекция печени остается основным методом лечения ГЦР, позволяющим надеяться на удовлетворительные отдаленные результаты. Существенное значение имеет и сохранность функции печени, т. к. частота сопутствующего цирроза печени у больных ГЦК достигает 80 %. В последние годы в связи с отработкой хирургической тактики и послеоперационного ведения больных летальность при резекции печени по поводу ГЦК не превышает 9 %, а 5-летняя выживаемость достигает 37 %. При этом остро стоит вопрос об объеме резекции печени в зависимости от распространенности опухолевого процесса и стадии сопутствующего цирроза печени.
