Применение ингибиторов протонной помпы у больных ВЗК при терапии глюкокортикостероидами: дань традиции в гастроэнтерологии или необходимость?

Аннотация
Необходимость применения системных глюкортикостероидов (ГКС) для индукции ремиссии у пациентов с воспалительными заболеваниями кишечника (ВЗК) не вызывает сомнений. Таким образом, эта группа больных находится в зоне риска развития неблагоприятных событий во время проводимой терапии, в т.ч. и со стороны верхних отделов желудочно-кишечного тракта (ЖКТ). ГКС обладают способностью снижать защитные и репаративные функции слизистой желудка и 12-перстной кишки, тем самым повышая возможность образования эрозивно-язвенных изменений гастродуоденальной зоны. В силу этого применение ингибиторов протонной помпы (ИПП) безусловно способно снизить риск ульцерогенеза и желудочно-кишечных кровотечений. Но, как и любая группа препаратов, ИПП не лишены побочных эффектов и риски некоторых из них выше у пациентов с ВЗК. В современных отчетах о результатах проведенных крупных исследований потенциальная польза от назначения кислотосупрессивной терапии доказана только в отношении лиц с повышенными рисками неблагоприятных событий, например при одновременном приеме ГКС и нестероидных противовоспалительных препаратов, пожилом возрасте, анамнезе эрозивно-язвенных заболеваний верхних отделов ЖКТ, при анамнезе ЖКТ кровотечений. Таким образом, требуется четкое понимание возможных преимуществ ИПП-терапии пациентов с ВЗК, находящихся на терапии ГКС.
