Феномен гиперпрогрессирования опухолей на фоне применения блокаторов PD1: мифы или реальность?

Аннотация
Новый класс лекарственных препаратов - блокаторы PD1 и CTLA4 - все шире и с успехом применяются в реальной клинической практике. По мере этого накапливаются случаи неожиданно бурного роста опухолевой массы после начала иммунотерапии, что приводит к ухудшению общего состояния пациента. Ускорение роста опухоли не менее чем в 2 раза по сравнению с ранее наблюдавшейся скоростью называют гиперпрогрессированием. Не все специалисты признают наличие этого феномена, многие считают, что ускоренный рост опухоли представляет собой естественное течение опухолевого процесса. В данной статье описано два клинических случая, соответствующих критериям гиперпрогрессирования, а также представлен обзор литературы, включая описание других клинических случаев и потенциальных иммунных и молекулярно-генетических механизмов, которые могут лежать в основе этого явления.
