Роль фекальных биомаркеров в прогнозировании воспалительных заболеваний кишечника

Аннотация
Фекальные биомаркеры (ФБМ) отражают выраженность воспаления слизистой оболочки кишечника в большей степени по сравнению с аналогичными маркерами сыворотки крови. Сывороточные маркеры могут изменяться по многим причинам, однако местное воспаление в кишечнике не всегда соответствует системному воспалительному ответу, выявленному в крови. На сегодня идентифицировано много ФБМ, но лишь некоторые из них были тщательно изучены у детей. В данной статье более подробно рассмотрены следующие перспективные маркеры: кальпротектин, S100A12, M2-пируваткиназа, остеопротегерин, миелопероксидазы, HMGB 1, матриксные металлопротеиназы, а также рекомендуемый в настоящее время фекальный кальпротектин. В настоящий момент не существует единого универсального маркера, который обладал бы высокой чувствительностью, специфичностью и прогностической способностью как при болезни Крона (БК), так и при язвенном колите (ЯК).
