Aim. Assessment of survival of children with steroid-resistant nephrotic syndrome in Ukraine, determination of risk factors for chronic renal failure development. Methods. Clinical evolution of 646 children with steroid-resistant nephrotic syndrome was performed according to the hospitalization periods (1980 - 2000 and 2001 - 2006). Results Clinical and morphological dynamics of steroid-resistant nephrotic syndrome in children was detected. In 1980 - 2000 the main risk factors for chronic renal failure development were renal function deterioration in the debut of the diasease and arterial hypertension. In 2001-2006 cumulative risk of chronic renal failure decreased (27,7% vs 39,7% in 1980-2000, p=0,008). In 2001-2006 risk of chronic heart failure became higher in boys aged 3 - 6 years. Conclusion. Dynamics of clinical and moprphological characteristics and chronic renal failure risk in children with chronic renal failure call for immunosuppressive therapy protocols optimization

Цель. Изучение выживаемости больных с стероид-резистентным вариантом гломерулонефрита с нефротическим синдромом (СР НС) в Украине, выявление факторов риска развития хронической почечной недостаточности (ХПН) у данной категории больных. Материал и методы. У 646 детей с СР НС проанализированы особенности течения заболевания и его исходов с учетом сроков госпитализации (1980-2000 и 2001-2006 годы). Результаты. Выявлены качественные изменения клинической и морфологической картины СР НС у детей в 2001-2006. Определены предикторы развития ХПН в 1980-2000 годы - наличие нарушения функции почек в начале заболевания и артериальная гипертензия (p=0,001). В 2001-2006 годах документировано снижение кумулятивной возможности развития ХПН в 2001-2006 годы до 27,7% (с 39,7% в 1980-2000 годы, p=0,008), но обнаружен высокий риск развития ХПН у мальчиков в возрасте 3-6 лет. Заключение. Динамика почечной выживаемости больных стероид-резистентным нефротическим синдромом требует адаптации к современным условиям протоколов иммуносупрессивной терапии
