Data from the medical records of patients with ACS treated at the St. Petersburg Research Institute of Emergency Care named after I.I. Dzhanelidze from 2020 to 2024 were analyzed. Two groups of patients were identified: Group 1 – 94 patients with ACS and a concomitant diagnosis of gout; Group 2 (control) – 95 patients with ACS but no gout. The groups were matched by gender, age, and type of ACS. All patients had their serum creatinine measured and their glomerular filtration rate (GFR) calculated using the CKD-EPI formula upon hospital admission, on days 2–3, and after 7 days of hospitalization. Acute kidney injury (AKI) and its stages were determined according to KDIGO guidelines. The results were statistically processed.</p> <p><bold>Results. </bold>Patients were divided into subgroups by chronic kidney disease (CKD) stage based on their baseline eGFR values. Stage III–IV CKD was observed in 35 (37.2%) patients in the gout+ACS group, compared to 13 (13.7%) in the control group, p&lt;0.05. The incidence of AKI among patients with ACS and gout and in the control group was 23 (24.5%) and 8 (8.4%), respectively (p&lt;0.05). In Group 1, the incidence of AKI increased with decreasing eGFR levels on admission. The mean age of patients who developed AKI was statistically significantly higher, and the initial eGFR level was lower than in patients without AKI (p&lt;0.05). In patients who developed AKI in Group 1, non-ST-elevation ACS was the most common – 17 (73.9%); in patients with AKI without gout, ST-elevation ACS was the most common – 5 (62.5%), p&lt;0.05. Fatal outcomes occurred in 9 (9.6%) patients in Group 1 and 4 (4.2%) patients in Group 2 (p&gt;0.05). Among patients with AKI, there were 7 (30.4%) fatal outcomes in Group 1 and 2 (25.0%) in Group 2 (p&gt;0.05).</p> <p><bold>Conclusion. </bold>Patients with gout are more susceptible to developing AKI in ACS due to their underlying renal dysfunction (gouty nephropathy). This determines a more unfavorable prognosis in these patients.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Цель исследования. </bold>Оценить функцию почек в динамике у пациентов с подагрой при остром коронарном синдроме (ОКС).</p> <p><bold>Материал и методы. </bold>Проанализированы данные историй болезни пациентов с ОКС, находившихся на лечении в СПб НИИСП им. И.И. Джанелидзе в 2020–2024 гг. Выделены две группы пациентов: группа 1 – 94 больных ОКС с сопутствующим диагнозом подагра, группа 2 (контрольная) – 95 пациентов с ОКС без подагры. Группы были сопоставимы по полу, возрасту и типу ОКС. У всех пациентов определяли уровень креатинина сыворотки крови и рассчитывали скорость клубочковой фильтрации (СКФ) по формуле CKD-EPI при поступлении в стационар, в динамике на 2–3-и сутки, а также после 7-х суток госпитализации. Острое повреждение почек (ОПП) и его стадии определяли в соответствии с рекомендациями KDIGO. Выполнена статистическая обработка результатов.</p> <p><bold>Результаты. </bold>В зависимости от исходных значений рСКФ пациенты были разделены на подгруппы по стадиям хронической болезни почек (ХБП). ХБП III–IV ст. наблюдалась у 35 (37,2%) больных в группе с подагрой+ОКС, в контрольной выборке таких пациентов было 13 (13,7%), p&lt;0,05. Число случаев ОПП среди больных ОКС и подагрой и в контрольной группе составило 23 (24,5%) и 8 (8,4%) соответственно (p&lt;0,05). В группе 1 частота ОПП увеличивалась со снижением уровня рСКФ при поступлении в стационар. Средний возраст пациентов с развившимся ОПП был статистически значимо выше, а исходный уровень рСКФ ниже, чем у пациентов без ОПП (p&lt;0,05). У пациентов с развившимся ОПП в группе 1 преобладал ОКС без подъема ST – 17 (73,9%), у пациентов с ОПП без подагры преобладал ОКС с подъемом ST – 5 (62,5%), p&lt;0,05. Летальный исход произошел у 9 (9,6%) пациентов группы 1 и у 4 (4,2%) больных группы 2 (p&gt;0,05). Среди пациентов с ОПП летальных исходов было 7 (30,4%) в группе 1 и 2 (25,0%) – в группе 2 (p&gt;0,05).</p> <p><bold>Выводы. </bold>Пациенты с подагрой более подвержены развитию ОПП при ОКС, что обусловлено исходно имеющейся у таких больных дисфункцией почек – подагрической нефропатией.
