This article presents current aspects of MN and personal experience with the treatment of MN with positive PLA2R antibodies, confirmed by kidney biopsy, in HIV-infected patients. Treatment of idiopathic membranous nephropathy with glucocorticosteroids in combination with anti-CD20+ monoclonal antibodies (rituximab) against the background of normalization of the immune status due to antiretroviral therapy has resulted in remission of nephrotic syndrome without complications associated with the use of immunosuppressive drugs. Our experience confirms the effectiveness of this therapeutic approach.</p></abstract><trans-abstract xml:lang="ru"><p>Мембранозная нефропатия (МН), ассоциированная с антителами к рецептору фосфолипазы A2 (PLA2R), встречается крайне редко у пациентов с ВИЧ без известных сопутствующих инфекций и вызывает значительные трудности с точки зрения ведения и выбора терапевтической тактики. В статье представлены современные аспекты МН и личный опыт терапии МН с положительными антителами к PLA2R, подтвержденной биопсией почки, у пациентов с ВИЧ-инфекцией. Лечение идиопатической МН глюкокортикостероидами в сочетании с моноклональными антителами к CD20+ (ритуксимаб) на фоне нормализации иммунного статуса благодаря антиретровирусной терапии позволило достичь ремиссии нефротического синдрома без осложнений, связанных с применением иммуносупрессивных препаратов.
