They were examed using the motor section of the FIM scale (FIM<sub>m</sub>), the VLT scale. The rehabilitation program was standardized and based on the National Clinical Guidelines for physical rehabilitation in spinal cord injured patients.</p> <p><bold>RESULTS. </bold>At admission, the age of the patients was 31 (24.0; 43.0) years, the proportion of patients with DU C<sub>4</sub>–C<sub>6 </sub>was 117 (62 %), with complete damage types A and B was 134 (70 %) patients, men 151 (79 %). Functional improvement was 19 (12.0; 28.0) points on the VLT scale; 14 (5.0; 21.0) points on the FIMm scale over the entire observation period, and was predominantly noted in the first rehabilitation cycle. The proportion of patients completely independent increased on FIMm activities: eating by 25 %, dressing the upper body by 33 %, dressing the lower body by 20 %, self-care domain by +34 %, wheelchair transfer by 21 %, walking by 5 %, and the proportion of patients completely dependent decreased: eating by 9 %, upper dressing by 13 %, lower dressing by 12 %, self-care domain by 7 %, wheelchair transfer by 21 %, wheelchair mobility by 11 %, and walking by 4.5 %. Changes on the VLT scale domains were balance domain 14 %, 1 finger 18 %, «hand» 26 %, and manipulation 11 %. The gain in food intake activity (FIM) was higher in patients with DU C<sub>6</sub>–C<sub>8</sub> (40–50 %) and incomplete motor damage (43 %), on the VLT scale domains similar to C<sub>6</sub>–C<sub>8</sub> (12–18 %), and incomplete motor damage (20 %).</p> <p><bold>CONCLUSION. </bold>The most significant functional recovery occurred in self-care and transfer activities, as well as hand and pinch grasping.</p></abstract><trans-abstract xml:lang="ru"><p><bold>ВВЕДЕНИЕ. </bold>Распространенность тяжелых повреждений позвоночника и спинного мозга в мире растет, при этом структура и характер восстановления утраченных функций остаются малоизученными.</p> <p><bold>ЦЕЛЬ. </bold>Изучить профиль изменчивости функциональной двигательной активности пациентов со спинальной тетраплегией для уточнения релевантных реабилитационных задач.</p> <p><bold>МАТЕРИАЛЫ И МЕТОДЫ.</bold> Было обследовано 190 пациентов с тяжелой цервикальной миелопатией С<sub>4</sub>–D<sub>1</sub> в возрасте 18–60 лет, у которых на протяжении 5–7 лет после спинномозгового повреждения были прослежены изменения клинико-функционального статуса, с помощью двигательного раздела шкалы FIM (FIMm), шкалы VLT. Программа реабилитации была стандартизированной и базировалась на существующих национальных клинических рекомендациях (СРР) по физической реабилитации у спинальных пациентов.</p> <p><bold>РЕЗУЛЬТАТЫ.</bold> При поступлении возраст пациентов составил 31 (24,0; 43,0) год; доля пациентов с ДУ С<sub>4</sub>–С<sub>6</sub> 117 (62 %) человек, с полнотой повреждения, типы A и B, — 134 (70 %) пациента, мужчин 151 (79 %) человек. Функциональное улучшение по шкале VLT составило 19 (12,0; 28,0) баллов; по шкале FIMm — 14 (5,0; 21,0) баллов за весь период наблюдений и было отмечено преимущественно в первый реабилитационный цикл. Доля пациентов, полностью независимых, увеличилась по активностям FIMm: прием пищи — на 25 %, одевание верхней части туловища — на 33 %, одевание нижней части туловища — на 20 %, домен «уход за собой» — на 34 %, пересаживание в кресло-коляску — на 21 %, ходьба — на 5 %, а доля пациентов, полностью зависимых, уменьшилась: прием пищи — на 9 %, одевание верхней части — на 13 %, одевание нижней части — на 12 %, домен «уход за собой» — на 7 %, пересаживание в кресло-коляску — на 21 %, передвижение в коляске — на 11 %, ходьба — на 4,5 %. Изменения по доменам шкалы VLT составили: домен «баланс» — 14 %, 1 палец — 18 %, «кисть» — 26 %, манипуляции — 11 %. Прибавка активности «прием пищи» (FIM) была выше у пациентов с ДУ: С<sub>6</sub>–С<sub>8</sub> (40–50 %) и при неполном двигательном повреждении (43 %), по доменам шкалы VLT аналогично: С<sub>6</sub>–С<sub>8</sub> (12–18 %) и при неполном двигательном повреждении (20 %).</p> <p><bold>ЗАКЛЮЧЕНИЕ. </bold>К наиболее изменчивым функциональным активностям относятся навыки самообслуживания и трансфера, а также формирование кистевого и пальцевого захватов кисти.</p></trans-abstract><kwd-group xml:lang="en"><kwd>spinal cord injury</kwd><kwd>rehabilitation</kwd><kwd>tetraplegia</kwd><kwd>activities of daily living</kwd><kwd>arm hand skilled performance</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>повреждение спинного мозга</kwd><kwd>реабилитация</kwd><kwd>тетраплегия</kwd><kwd>ежедневная жизнедеятельность</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><mixed-citation>National Spinal Cord Injury Statistical Center.
