One of the most promising approaches is the impact on the molecular mechanisms of aging, including the activation of adaptive systems and suppression of pathological processes in the body. Methods of extracorporeal hemocorrection have proved to be a good idea in this respect.</p> <p><bold>AIM. </bold>To evaluate the effectiveness and safety of hardware plasmapheresis as a technology for correcting aging biomarkers.</p> <p><bold>MATERIAL AND METHODS.</bold> A technique of therapeutic plasmapheresis use was introduced for the correction of aging biomarkers. Twenty-four participants (male and female) aged 40–60 years with an elevated level of one or more aging biomarkers underwent a course of therapeutic plasmapheresis in the daytime hospital. All participants underwent four procedures of therapeutic hardware plasmapheresis once every 3 days with 30 % volume of circulating plasma followed by replacement with colloid (5 % albumin solution) and crystalloid solutions (saline solution) in a 1:3 ratio or only crystalloid solutions.</p> <p><bold>RESULTS AND DISCUSSION.</bold> A comparative evaluation of aging biomarkers before the procedure and 17 and 30 days after hardware plasmapheresis showed that therapeutic plasmapheresis affects the levels of human aging biomarkers in blood. A significant decrease in the levels of such biomarkers as homocysteine, urea, gamma-glutamyl transpeptidase, alkaline phosphatase, creatine phosphokinase, cholinesterase, and uric acid was shown. No significant differences were detected when we performed a comparative assessment of biochemical blood parameters following plasmapheresis with or without albumin replacement on biochemical blood parameters. Stable hemodynamic parameters during plasmapheresis and the absence of adverse reactions in patients confirm the safety and tolerability of the therapeutic plasmapheresis procedure.</p> <p><bold>CONCLUSION.</bold> Implementing this technique into clinical practice will allow the development of approaches to etiotropic therapy of many chronic age-related pathologies. These treatments have the potential to increase life expectancy and improve its quality.</p></abstract><trans-abstract xml:lang="ru"><p><bold>ВВЕДЕНИЕ.</bold> В связи с демографическим старением населения проблема лечения возрастных заболеваний и профилактики преждевременного старения в современном здравоохранении приобрела особую актуальность. Одним из наиболее перспективных подходов является воздействие на молекулярные механизмы старения, включая активацию адаптивных систем и подавление патологических процессов в организме. В этом плане хорошо зарекомендовали себя методы экстракорпоральной гемокоррекции.</p> <p><bold>ЦЕЛЬ.</bold> Оценка безопасности и эффективности применения метода аппаратного плазмафереза в качестве технологии коррекции биомаркеров старения.</p> <p><bold>МАТЕРИАЛЫ И МЕТОДЫ.</bold> Разработана методика применения лечебного плазмафереза с целью коррекции биомаркеров старения. Пациенты обоего пола в возрасте 40–60 лет, у которых был выявлен повышенный уровень одного или нескольких биомаркеров старения, проходили курс лечебного плазмафереза в условиях дневного стационара. Всем пациентам были выполнены 4 процедуры лечебного плазмафереза один раз в 3 дня объемом 30 % объема циркулирующей плазмы с последующим замещением коллоидными (5%-й раствор альбумина) и кристаллоидными растворами (физраствор) в соотношении 1:3 или только кристаллоидными растворами.</p> <p><bold>РЕЗУЛЬТАТЫ И ОБСУЖДЕНИЕ.</bold> Определение содержания биомаркеров старения в крови до и через 17 и 30 дней после начала лечения показало, что аппаратный плазмаферез оказывает влияние на уровень маркеров старения человека. Было отмечено достоверное снижение уровня таких биомаркеров, как гомоцистеин, мочевина, гамма-глютамилтранспептидаза, щелочная фосфатаза, креатинкиназа, холинэстераза и мочевая кислота. При сравнительной оценке влияния плазмафереза с возмещением и без возмещения альбумином на биохимические показатели крови статистически значимых различий выявлено не было. Стабильные показатели гемодинамики во время процедуры и после проведения плазмафереза, а также отсутствие у пациентов нежелательных реакций подтверждают безопасность и хорошую переносимость процедуры лечебного плазмафереза.</p> <p><bold>ЗАКЛЮЧЕНИЕ.</bold> Внедрение данной методики в клиническую практику позволит разработать подходы к этиотропному лечению целого ряда хронических возрастзависимых патологий, что может способствовать увеличению продолжительности жизни и повышению ее качества.</p></trans-abstract><kwd-group xml:lang="en"><kwd>aging biomarkers</kwd><kwd>anti-aging therapy</kwd><kwd>hemocorrection</kwd><kwd>plasmapheresis</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>биомаркеры старения</kwd><kwd>антивозрастная терапия</kwd><kwd>гемокоррекция</kwd><kwd>плазмаферез</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><mixed-citation>United Nations Department of Economic and Social Affairs, Population Division (2022).
