Most treatment methods for neurogenic dysphagia remain at the preclinical trial stage, and currently, there is no definitive evidence for an effective therapy. This article presents data from a clinical study of the impact of acupuncture therapy in treating neurogenic dysphagia during the acute phase of stroke and evaluates the role of acupuncture at the first stage of rehabilitation in the prevention of secondary complications.</p> <p><bold>AIM. </bold>To determine the role of acupuncture therapy in the treatment of neurogenic dysphagia at the initial stage of rehabilitation in patients during the acute phase of stroke.</p> <p><bold>MATERIALS AND METHODS. </bold>An open-label comparative clinical study was conducted at two clinical facilities: Republican Clinical Hospital named after N.A. Semashko, the Ministry of Health of the Republic of Buryatia; Irkutsk Regional Clinical Hospital, winner of the Order of the Badge of Honor. The study was approved by the Ethics Committee of the Irkutsk State Medical Academy of Postgraduate Education (Protocol No. 2 of 28.03.2024). The study involved a total of 72 patients suffering from impaired swallowing function during the acute phase of ischemic stroke. Dysphagia was diagnosed by a speech therapist using the Brain Institute Clinic scale (BIC scale), Fiberoptic Endoscopic Dysphagia Severity Scale (FEDSS), and the Penetration-Aspiration Scale (PAS). Randomization was performed using the method of sealed envelopes, with all the patients divided into two groups. The control group (35 patients) received standard therapy combined with physiotherapy using the vocaSTIM device. The study group (37 patients) received standard therapy in combination with acupuncture (one course consisting of 10 sessions).</p> <p><bold>RESULTS AND DISCUSSION. </bold>The comparative clinical study showed that most patients experienced regression of post-stroke dysphagia. According to the results of this study, reflexotherapy is effective in restoring swallowing function compared to physiotherapy, as evaluated by the BIC scale on day 28 (<italic>p</italic> = 0.00001), FEDSS on day 14 (<italic>p</italic> = 0.00784), PAS on day 14 (<italic>p</italic> = 0.00956), reduces the duration of tube feeding, accelerates the resolution of aspiration infiltrative changes in the lungs, and reduces the need for tracheostomy.</p> <p><bold>CONCLUSION. </bold>This treatment method plays an important role in the management of post-stroke dysphagia and in the prevention of secondary complications when administered in a timely fashion.</p></abstract><trans-abstract xml:lang="ru"><p><bold>ВВЕДЕНИЕ. </bold>Нейрогенная дисфагия является значимой проблемой в современной медицине ввиду частых вторичных осложнений, таких как аспирационная пневмония, системно-воспалительный процесс и нутритивный дефицит. Большинство методов лечения нейрогенной дисфагии находятся на стадии доклинических испытаний, убедительных доказательств по лечению дисфагии в настоящее время нет. В статье приведены данные клинического исследования влияния иглорефлексотерапии (ИРТ) на лечение нейрогенной дисфагии в острый период инсульта, а также определена роль акупунктуры на первом этапе реабилитации в профилактике вторичных осложнений.</p> <p><bold>ЦЕЛЬ. </bold>Определить роль ИРТ в лечении нейрогенной дисфагии на первом этапе реабилитации у больных в острый период инсульта.</p> <p><bold>МАТЕРИАЛЫ И МЕТОДЫ. </bold>Открытое клиническое сравнительное исследование проводилось на двух клинических базах: ГАУЗ «Республиканская клиническая больница имени Н.А. Семашко» Минздрава Республики Бурятия; Иркутская областная клиническая больница ордена «Знак почета». Исследование одобрено этическим комитетом Иркутской государственной медицинской академии последипломного образования (протокол № 2 от 28.03.2024). В данном клиническом исследовании апробированы 72 пациента с нарушением функции глотания в острый период ишемического инсульта. Диагностика дисфагии проводилась логопедом по шкале Клиники Института Мозга (КИМ), видеоэндоскопическими исследованиями, шкалой оценки степени тяжести дисфагии (Fiberoptic Endoscopic Dysphagia Severity Scale — FEDSS) и шкалой пенетрации-аспирации (Penetration-Aspiration Scale — PAS). Рандомизация проводилась по методу конвертов, все пациенты распределены на 2 группы. В контрольную группу, получающую стандартную терапию в сочетании с физиотерапией (vocaSTIM), вошло 35 больных; 37 пациентам, составившим исследуемую группу, проводилась базисная терапия в сочетании с ИРТ (1 курс — 10 сеансов).</p> <p><bold>РЕЗУЛЬТАТЫ И ОБСУЖДЕНИЕ. </bold>В<bold> </bold>ходе сравнительного клинического исследования у большинства пациентов отмечается регресс постинсультной дисфагии. По результатам данного исследования метод рефлексотерапии показывает эффективное восстановление функции глотания в сравнении с физиотерапией, оцениваемой по шкале КИМ на 28-й день (<italic>p</italic> = 0,00001), FEDSS — на 14-й день (<italic>p</italic> = 0,00784), PAS — на 14-й день (<italic>p</italic> = 0,00956), уменьшается длительность пребывания на зондовом питании, наблюдается более быстрый темп разрешения аспирационных инфильтративных изменений в легких и снижается необходимость проведения трахеостомии.</p> <p><bold>ЗАКЛЮЧЕНИЕ. </bold>Данный метод играет важную роль в лечении постинсультной дисфагии, а также в профилактике вторичных осложнений при своевременной терапии.</p></trans-abstract><kwd-group xml:lang="en"><kwd>acupuncture</kwd><kwd>stroke</kwd><kwd>rehabilitation</kwd><kwd>dysphagia</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>иглорефлексотерапия / акупунктура</kwd><kwd>инсульт</kwd><kwd>реабилитация</kwd><kwd>дисфагия</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><mixed-citation>Скворцова В.И., Губский Л.В., Стаховская Л.В. и др.
