Hypoxic conditioning (HC), in particular, intermittent hypoxic-hyperoxic exposures (IHHE), which can improve adaptation mechanisms and reduce inflammatory processes, is a promising method of rehabilitation of patients with COLD, along with basic medication and physiotherapy.</p> <p><bold>AIM.</bold> To evaluate the efficacy and safety of IHHE application in the programs of complex rehabilitation of patients with COLD.</p> <p><bold>MATERIALS AND METHODS.</bold> A single-center blind randomized placebo-controlled study with participation of 60 patients (32 men, age 58,2 ± 14,1 years, desiase’s duration 15,9 ± 2,8 years) with COLD (occupational bronchial asthma — 59.3 %), occupational chronic bronchitis — 18.5 %, occupational chronic obstructive pulmonary disease — 11.1 %) and occupational exogenous allergic alveolitis — 11.1 %), divided into two groups was conducted: the main group received 12 sessions of IHHE in 3 weeks, control group — placebo procedures. Both groups underwent standard medical and physiotherapeutic rehabilitation. Clinical, functional and laboratory parameters before and after the rehabilitation course were evaluated.</p> <p><bold>RESULTS AND DISCUSSION.</bold> In the IHHE group there was a significant improvement of hypoxic resistance (according to Stange’s and Gench’s tests, hypoxic test), reduction of broncho-obstructive manifestations, improvement of external respiratory function indices and reduction of blood pressure. There was also an increase in exercise tolerance according to the six-minute walk test (6MWT) (an increase of 11.9 ± 8.6 % from baseline), a decrease in the severity of dyspnea and subjective assessments of exercise severity according to the Borg scale. In the control group, the positive dynamics in the indicators of hypoxic tolerance, broncho-obstruction, and 6MWT tolerance were significantly less pronounced. The results of the study confirm that IHHE procedures can serve as a significant addition to the standard rehabilitation of patients with COLD, contributing to the improvement of clinical and functional parameters, correction of comorbid disorders.</p> <p><bold>CONCLUSION.</bold> IHHE is a safe and effective method to be added to standard rehabilitation of patients with COLD, leading to more pronounced improvements of hypoxic resistance, exercise tolerance, reduction of broncho-obstruction and normalization of blood pressure.</p></abstract><trans-abstract xml:lang="ru"><p><bold>ВВЕДЕНИЕ.</bold> Хронические профессиональные заболевания легких (ХПЗЛ) представляют собой серьезную медицинскую и социальную проблему, связанную с высокой степенью коморбидности и системными нарушениями, включая кардиометаболические изменения, выраженный оксидативный стресс, хроническое воспаление, автономную и иммунную дизрегуляцию, и ведут к инвалидизации и снижению качества жизни. Перспективным методом реабилитации пациентов с ХПЗЛ, наряду с базисной медикаментозной и аппаратной физиотерапией, является гипоксическое кондиционирование (ГК), в частности, интервальные гипоксически-гипероксические воздействия (ИГГВ), которые могут улучшать адаптационные механизмы и снижать воспалительные процессы.</p> <p><bold>ЦЕЛЬ.</bold> Оценить эффективность и безопасность применения ИГГВ в программах комплексной реабилитации пациентов с ХПЗЛ.</p> <p><bold>МАТЕРИАЛЫ И МЕТОДЫ.</bold> Проведено одноцентровое слепое рандомизированное плацебо-контролируемое исследование с участием 60 пациентов (32 мужчин, 28 женщин, средний возраст составил 58,2 ± 14,1 лет, средняя длительность заболевания — 15,9 ± 2,8 лет) с ХПЗЛ (профессиональная бронхиальная астма — 59,3 %, профессиональный хронический бронхит — 18,5 %, профессиональная хроническая обструктивная болезнь легких — 11,1 % и профессиональный экзогенный аллергический альвеолит — 11,1 %), разделенных на две группы: основная группа (<italic>n</italic> = 30) получала 12 процедур ИГГВ за 3 недели, контрольная группа (<italic>n</italic> = 30) — плацебо-процедуры. Обе группы проходили стандартную медикаментозную и физиотерапевтическую реабилитацию. Оценивались клинические, функциональные и лабораторные показатели до и после курса реабилитации, а также их относительная динамика.</p> <p><bold>РЕЗУЛЬТАТЫ И ОБСУЖДЕНИЕ.</bold> В группе ИГГВ отмечено значительное улучшение гипоксической устойчивости (по пробам Штанге и Генча при проведении гипоксического теста), снижение бронхообструктивных проявлений, улучшение показателей функции внешнего дыхания и снижение артериального давления. Также наблюдалось увеличение толерантности к физической нагрузке по данным теста шестиминутной ходьбы (ТШХ) (прирост — 11,9 ± 8,6 % от исходных значений), уменьшение выраженности одышки и субъективных оценок тяжести нагрузки по шкале Борга. В контрольной группе положительная динамика в индикаторах гипоксической толерантности, бронхообструкции, переносимости ТШХ была значимо менее выражена. Результаты исследования подтверждают, что процедуры ИГГВ могут служить значимым дополнением к стандартной реабилитации пациентов с ХПЗЛ, способствуя улучшению клинических и функциональных показателей, коррекции коморбидных расстройств.</p> <p><bold>ЗАКЛЮЧЕНИЕ.</bold> Процедуры ИГГВ являются безопасным и эффективным методом дополнения стандартной реабилитации пациентов с ХПЗЛ, приводящим к более значимому улучшению гипоксической устойчивости, переносимости физических нагрузок, снижению бронхообструкции и нормализации артериального давления в сравнении с данными плацебо-контроля.</p></trans-abstract><kwd-group xml:lang="en"><kwd>chronic occupational lung disease</kwd><kwd>rehabilitation</kwd><kwd>intermittent hypoxic-hyperoxic exposures</kwd><kwd>exercise tolerance</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>хронические профессиональные заболевания легких</kwd><kwd>реабилитация</kwd><kwd>интервальные гипоксически-гипероксические воздействия</kwd><kwd>нагрузочная выносливость</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><mixed-citation>Косарев В.В., Бабанов С.А.
