ВКЛАД ФИКСИРОВАННОЙ КОМБИНАЦИИ ДОЗ АБАКАВИРА/ЛАМИВУДИНА В УЛУЧШЕНИЕ ПРИВЕРЖЕННОСТИ К ЛЕЧЕНИЮ БОЛЬНЫХ ВИЧ-ИНФЕКЦИЕЙ

Аннотация
В настоящее время при назначении схемы антиретровирусной терапии (АРВТ) использование фиксированных комбинаций доз (ФКД) НИОТ имеет определенные преимущества за счет уменьшения количества и кратности приема лекарственных препаратов. В обзоре представлены результаты нескольких крупных исследований (в том числе когортных) эффективности и безопасности схем АРВТ, включавших препараты абакавир и ламивудин в ФКД (препарат кивекса). Доля пациентов, имевших высокий (> 95%) уровень приверженности, обеспечивающий оптимальную вирусологическую супрессию вируса, была выше при назначении ФКД по сравнению с монопрепаратами (39 и 31%). Результаты исследований продемонстрировали, что при использовании ФКД возрастает приверженность не только к лечению в целом, но и к к дозе (с 91,9 до 97,1%), времени приема (с 86,3 до 95,5%), третьему препарату в схеме АРВТ (с 85 до 92,3%); удовлетворенность лечением (с 85 до 92%) и продолжительность пребывания на первой линии терапии. Кроме того, ФКД дают специалистам возможность более эффективного ведения сочетанных заболеваний, требующих одновременного лечения (оппортунистические инфекции, туберкулез, хронические вирусные гепатиты и т.д.). Обладая высокой вирусологической и иммунологической эффективностью и имея благоприятный профиль безопасности и переносимости, препарат кивекса включен в схемы АРВТ первой линии в большинстве современных рекомендаций, в том числе в рекомендациях российского Национального научного общества инфекционистов (2012).
