CREATION OF EXPERIMENTAL MODEL IN VIVO TO ADEQUATE SURGICAL TECHNIQUE IN DOUBLE-LAYER METHODOLOGY OF TREATMENT OF MULTIPLE GUM RECESSIONS WITH THE USE OF PLASTIC MATERIAL
Abstract
An experimental model in vivo adequate to the technique of coronal bias in a two-layer technique for multiple gum recessions treatment with the use of plastic material was developed. Surgical intervention on the mandible of rats was performed, interaction of operated tissues with plastic material was assessed. In this study, plastic material of allogeneic origin (dura mater) was used. Observations of the animals were carried out for 28 days, results from dental status and macroscopic drugs were evaluated. The experiment proved the effectiveness of allogeneic dura mater. The proposed surgical model is applicable for evaluating the interaction of tissues with any plastic material.

PECULIARITIES OF APPLICATION OF AUTOLOGOUS BONE GRAFTS AND INDIVIDUAL ALLOGENEIC BONE IMPLANTS IN THE TREATMENT OF PATIENTS WITH ATROPHY OF LOWER JAW ALVEOLI
Abstract
The use of dental implants in the treatment of patients with partial / full adentia is the main and most qualitative method in modern dentistry. Every year the number of dental implants installed by the doctors, dental surgeons and maxillofacial surgeons, is steadily growing. One of the contraindications showing the impossibility of installation a dental implant in patients is the resorption of the alveolar process / part of the jaws. First of all in such a situation it is necessary to restore the volume of bone tissue to the level sufficient for the subsequent dental implantation. In this article the techniques of restoration of bone tissue volume are reviewed. The traditional technique using autologous bone grafts is compared with the innovative one using individual allogeneic bone implants and then the advantages of the latter are described.

PROLIFERATIVE ACTIVITY OF CHONDROBLASTS ON 3D CARRIER IN DIFFERENT CULTIVATION CONDITIONS
Abstract
The article presents an analysis of the proliferative activity of chondroblast cells on a 3D bio-carrier - a demineralised lyophilised spongiosis. The analysis was carried out in various conditions of cultivation: in a ventilated test tube at 5% CO2, 37°C; in a sealed airproof test tube; in a test tube completely filled with culture medium at 37°C. According to the results of DNA mesuarement, on the 7th day of cultivation the growth of cell number in comparison with the 0 day was 29.5% in the CO2 incubator and 34% in a thermostate. The histology revealed a local growth of the cell numbers (from one to several layers) on the surface of the carrier.

IDENTIFICATION OF LUMINOUS BACTERIA ISOLATED FROM THE BLACK AND THE AZOV SEAS
Abstract
The article studies phenotype of ten isolates of luminous bacteria isolated from water and fauna of the Black and Azov Seas. The following findings were obtained: four strains were identified as representatives of species Allivibrio fischeri, three strains were the representatives of species Photobacterium leiognathi and another three has been identified as Vibrio harveyi.

REHABILITATION TREATMENT OF PATIENTS WITH HABITUAL DISLOCATION OF SHOULDER
Abstract
This article is dedicated to the management of patients with habitual dislocation of shoulder. This disorder is quite common and affects mainly young and middle-aged people. Confined conservative treatment is rarely beneficial. Most cases of chronic shoulder instability require surgical invasion. Limitation of movements and weakness of shoulder girdle muscles develop postopreratively due to prolonged immobilization of upper extremity. This condition requires exercise therapy, massage and physiotherapeutic procedures. Rehabilitation of patients with habitual dislocation of shoulder should be started early in postoperative period and be continued after immobilization period. It helps to restore movements of the operated joint and upper extremity.

MEDICOBIOLOGIC, INFORMATION AND TECHNICAL ASPECTS OF GRAVITATIONAL THERAPY APPLICATION IN THE TREATMENT OF LIMB ISCHEMIA
Abstract
The article describes the possibility of using gravitational therapy for the treatment of limb ischemia, as well as monitoring the state of peripheral hemodynamics. It is proved that gravitational therapy is an effective and promising method for stimulating regional and collateral circulation at the range of gravity load of 1.5 - 3 Gz and the duration of treatment session from 10 to 20 minutes per day and 10-20 sessions for treatment course. Microgravity is simulated by using special short-arm centrifuge (SAHC). In addition, the concept and hardware for monitoring of the peripheral hemodynamics in the “man- SAHC” system was developed. The system has a remote control by radio channel and does not require programming.

THE STATUS OF THE GENE K-RAS IS AN INDEPENDENT PROGNOSTIC CRITERION IN PATIENTS WITH COLORECTAL CANCER
Abstract
Actuality. One of the major somatic mutations found in colon tumors is the mutation in K-RAS gene which is revealed with in 30% - 50% of cases. The mutation in the K-RAS gene can become a new prognostic factor in one and the same stage of a disease. Objective. The aim of the research is to investigate frequency of mutation in K-RAS gene in patients with colorectal cancer in the Republic of Tatarstan, and to assess its influence on prognosis and the course of the disease. Materials and methods. The research included 120 patients with a colorectal cancer who received the combined treatment in Republican Clinical Oncology Dispensary of the Republic of Tatarstan. Genetic analysis of postoperative specimens for the presence of mutation in K-RAS gene was perfoemed. Results of own researches. Of 120 patients 35 (26.7%) were revealed K-RAS gene mutation , 85 patients having a wild-type of of K-RAS gene. In addition 25 mutations were found in 12-th and 13-th codones, 7 mutations were in 61-st codon and 3 were in the 146-th codon. Results of a research showed that the mutant type of a K-RAS gene is revealed more often in Russian population (38.9%) than in Tatar population (in 21.2%) (р =0.045). The mutation in K-RAS gene is more prevalent in women than in men (37.7% and 20.3% correspondingly). The existing relationship is of significance (р =0.045). Colon tumors with a mutant K-RAS gene are more prone to lymphogenous metastasizing (in 54.2%), than tumors with wild-type (45.8%), that indirectly shows more aggressive course of tumoral process in case of mutant type of a gene (р = 0.004). Patients with mutant type of gene have remote synchronous metastasis more often (63.6%), than patients with wild-type (36.4%). This dependence is statistically significant (р =0.0001). Annual survival rate of patients with wild-type of K-RAS gene was 96.5%, with mutant type of a gene was 88.6% (р =0.0012). Conclusion. Thus, the tumor with mutant type of K-RAS gene is characterized by aggressive course and high potential to metastasizing. The gene mutation in a tumor can be an independent significant prognostic sign for an assessment of the course and outcome of the disease.

PROGNOSIS OF POSTOPERATIVE COMPLICATIONS IN COLORECTAL CANCER PATIENTS
Abstract
Nowadays, prediction in colorectal surgery is a very useful instrument in managing the health care system in hospitals. Search for reliable risk factors of postoperative complications allows to reveal complication risk group of patients and to decrease hospital stay for patients with a good prognosis. C-reactive protein is well known as a sensitive and dynamic marker of systemic inflammatory response syndrome. This study evaluates the feasibility of using of C-reactive protein as predictor of septic postoperative complications. Analysis of serum CRP in early postoperative period is shown to predict septic postoperative complications in colorectal cancer patients.

LONG-TERM OUTCOMES TREATMENT IN PATIENTS WITH DUODENAL ULCER PERFORATION
Abstract
The article is devoted to the treatment of patients with chronic duodenal ulcer perforation. An analysis of the long-term results of treatment of 160 working-age patients, who underwent operation on chronic duodenal ulcer perforation. 79 patients of control group underwent suture plicaton. The main group consisted of 81 patients. These patients were operated on peptic ulcer disease. The results of the study showed suture plication to increase the number of ulcer disease recurrence by 35.2%.

LIFT OPERATION IN THE TREATMENT OF PATIENTS WITH ADRECTAL FISTULAS
Abstract
Ligation of intersphincteric fistula tract space (LIFT procedure) is one of the most promising sphincter-preserving surgical interventions. The efficiency of LIFT technique in the treatment of 103 patients with extrasphincteric and transsphincteric anal fistulas was estimated. LIFT techniques proves to be more beneficial than segmental proctoplasty with the muco-submucosal flap displacement. The maximum efficiency was obtained in patients with high extrasphincteric and transsphincteric anal fistulas of the I - II degrees and short linear fistulous tract.

ORGANIZATIONAL AND TACTICAL ISSUES OF ASSISTANCE TO POSTSTERNOTOMY MEDIASTINITIS PATIENTS
Abstract
The article is devoted to the study of unfavorable results of treatment of patients with infectious complications after sternotomy. Organizational and tactical approaches to the treatment of poststernotomy mediastinitis are demonstrated by means of analysis of treatment outcome of 132 patients. Depending on the chosen treatment modality patients were divided intoseveral study groups. The causes of unsatisfactory treatment outcomes consist in ineffective attempts to treat poststernotomy mediastinitis before admission to the surgical thoracic department, resternotomy in cardiosurgical department, delay in admission to specialized surgical thoracic department, increase in the initial patients’ multimorbidity, and multiresistance of microflora, lack of wound preparation for plastic closure.

CHALLENGES IN BRONCHOSCOPY OF TRACHEOSTOMIZED PATIENTS AND HOW TO OVERCOME THEM
Abstract
A traditional variant of bronchoscopy through a tracheostomy cannula may not only lead to the increased risk of bronchoscope damage but also deteriorate the quality of tracheal inspection and sanation and can make it impossible to use the conventional options for non-invasive protection of bronchoscopy. The aim of this study was to optimize the bronchoscopic procedure in tracheostomized patients. Materials and methods. The study was conducted in two separate lines. Firstly the reduction of the iatrogenic risk of bronchoscope damage after modification of the outer tracheostomy cannula was evaluated. Secondly an attempt to develop a bronchoscopy option was made, it was to exclude the shortcomings of the traditional procedure performed in the tracheostomized patients. Results. The proposed modification of the outer tracheostomy cannula provides a statistically significant reduction of specific repair situations. A technology chart of bronchoscopy in tracheostomized patients has been proposed and used in 36 bronchoscopic procedures (29 patients). As a result of this work, the shortcomings of traditional bronchoscopy in tracheostomized patients have been mainly overcome.

CLINICAL APPLICATION OF A MODIFIED APPROACH TO THE RESTORATION OF TEETH BY THE COMBINED USE OF GLASS-IONOMER CEMENT AND COMPOSITES
Abstract
Nowadays with the advent of new materials and adhesive systems and techniques, protocols for the joint use of composite and GIC require revision. In this study, we have evaluated the efficacy of a modified approach to the restoration of teeth by the combined use of glass ionomer and composites in the clinic. The study included 150 patients with localization of caries in the I-II class according to Black. The best results were observed in the group in which there was combined use of resin modified glass-ionomer cement, self-etch adhesive systems, bulk-fill composite and traditional composite.

HYDRODYNAMICS OF DRAIN FROM POROUS NONWOVEN MATERIAL FOR VACUUM-WASHING DRAINAGE
Abstract
Existing methods of purulent wounds draining in the maxillofacial area fail to provide the complete outflow of fluid. The author of the article suggested to drain purulent wounds with non-woven titanium material with continuous porosity. The aim of the study was to study hydrodynamic processes occurring in the drainage made of non-woven titanium material with through porosity and surrounding tissue of the wound. A 3D model of the drainage was created and installed in the wound cavity. In the process of drainage a negative pressure over the entire surface is revealed, it allows you to effectively aspirate inflammatory exudates from the wound cavity with odontogenic phlegmon. Due to high porosity drainage extrusion of saline does not occur, and aspiration of exudates takes place over its entire surface. The study reached the following conclusions. The design of the proposed drainage allows to perform vacuum therapy at a negative pressure of 125 mm Hg. The proposed device turns an open wound into a closed drained cavity, it also provides controlled outflow of fluid and targeted drug delivery.

LABORATORY STUDY OF FLUORESCENCE OF DEPULPED TEETH AFTER BLEACHING
Abstract
The aesthetic characteristics of the tooth imply harmony not only in the size and shape but also in the optical properties of the tooth tissues, including transparency, fluorescence, opalescence, gloss and others. The aim of study was to evaluate the intensity and spectral composition of the fluorescence emission of devitalized teeth before and after whitening. The object of research became 36 devitalized extracted human teeth which had been removed for medical reasons in patients of different age. Measuring of the spectra of fluorescent properties of the objects was carried out in the Institute of Physics, National Academy of Sciences. It was performed by a comparative evaluation of the fluorescence spectra of hard tissue of devitalized teeth in patients of different age groups before and after whitening.

THE STATE OF ORAL CAVITY MICROFLORA IN PATIENTS WITH CHRONIC INFLAMMATORY PERIODONTAL DISEASE
Abstract
The article presents results of complex treatment of 98 patients with mild chronic generalized periodontitis who were treated on the clinical base of the Therapeutic Dentistry Department with a course of Additional Professional Education at Bashkir State Medical University. Clinical features of these patients were given. Microbiological assessment of the contents of periodontal pocket was made. The issues of diagnosis and treatment of patients with chronic generalized periodontitis by means of laser phoresis with alginic acid gel were studied. The results of our studies have shown that such treatment of chronic generalized periodontitis by means of laser phoresis with alginic acid gel is an effective method for restoring periodontal tissues.

THE EFFICIENCY OF REMOVABLE PLATE APPARATUS AND MYOGYMNASTIC IN THE TREATMENT OF DISTAL OCCLUSION IN CHILDREN WITH NASAL BREATHING DISORDER
Abstract
This article shows the results of application of removable plate apparatus in the children with a distal bite and narrowing of the lumen of the upper respiratory tract. Orthodontic treatment was performed for the patients with saggital opening of 0 - 8 mm. The application of removable plate apparatuses along with myogymnastic excercises is noticed to be the most effective for the patients with distal occlusion, narrowing of the upper respiratory tract lumen and saggital opening more than 3 mm.

COMPARATIVE EVALUATION OF IVF PROTOCOLS IN WOMEN WITH VARIOUS CLINICAL VARIANTS OF THE CURRENT OF POLYCYSTOPIC OVARY SYNDROME
Abstract
IVF is one of the methods to ensure fertility in women suffering from PCOS. The choice of protocol appears to be difficult for the mixed response of ovaries to stimulation. The aim of the study is to compare the efficiency of IVF protocols in women with various forms of PCOS. The investigation includes 145 women with PCOS. 78 patients fall under the first group (the main group), these women have metabolic disorders, hyperandroginism and type 2 diabetis. The second group (comparison group) includes 67 patients who have hyperandrogenism but without metabolic disorders. The examination of women in both groups and following the protocols were performed according to the order of Health Ministry № 107н (August 30th ,2012). Results. In women suffering from metabolic syndrome and accompanying PCOS, the effectiveness of IVF protocols is lower than in women having no metabolic disorder. The findings of IVF protocols in both groups shows that the number of aspirated follicles, immature oocyte and the number of received embryos is higher in a long protocol than it is in a short one. The results of IVF procedures in women of both groups. Impregnation frequency and child bearing is higher in the group free from metabolic disturbances Conclusion. Long protocol of IVF is better for women with combined PCOS and metabolic syndrome. To avoid severe ovarian hyperstimulation syndrome in women with PCOS but without metabolic syndrome the use of sequeneed cycle is recommended.

MONITORING OF PREGNANCY IN WOMEN WITH HERPES INFECTION, DEPENDING ON THE APPROACHES TO THE RELAPSE PREVENTION
Abstract
The article presents a comprehensive assessment of pregnancy in women with relapsing herpetic infection taking into consideration approaches to relapse prevention. Clinical and laboratory-instrumental examination of the state of fetoplacental complex was performed. An association between the relapse rate and the formation of pathological conditions in pregnant women was revealed and proved. The results of the study confirm the benefit of pregestation and antenatal relapse prevention by means of the developed method. It is also favourable for the condition of the fetoplacental complex, gestation course, mechanisms of phetoprotection in case of intrauterine infection with herpes simplex virus.

PEELING OF THE INTERNAL LIMITING MEMBRANE IN SURGERY ON RHEGMATOGENOUS RETINAL DETACHMENT (PRELIMINARY RESULTS)
Abstract
The aim of our study was to compare the effectiveness of the results of surgical treatment of rhegmatogenous retinal detachment after endovitreal intervention in groups of patients who had undergone of ILM removal and those who had not been administered such a procedure. Of 20 patients, 10 patients underwent endovitreal intervention with removal of the ILM (the first group) and in 10 patients ILM was preserved (second group). In all cases the operation was completed with tamponade of vitreal cavity with the sude of silicone oil. All patients were examined preoperatively and were followed up at the interval of 1, 3, and 6 months after the operation. In the early postoperative period complete adherence of the retina was observe in all cases, but in the second group recurrence of retinal detachment was revealed in 1 case (10%) 4 months after removal of silicone oil. Optical coherence tomography (OCT) failed to reveal epiretinal fibrosis, and cystic macular edema in patients of the first group. However, in patients of the second group OCT demonstrated epiretinal fibrosis (30%) and cystoid macular edema (70%) 6 months later. Taking into consideration all the data we can conclude that ILM removal in the surgical treatment of patients with retinal detachment is an effective method and can be recommended.

ADHERENCE TO THE FIRST-LINE THERAPY WITH DISEASE MODIFYING DRUGS IN PATIENTS WITH MULTIPLE SCLEROSIS
Abstract
The article is devoted to the question of multiple sclerosis patients’ adherence to therapy. Interpretation of research data on the compliance of patients, the level of their physical and emotional state, and adherence to treatment depending on disease duration, severity of neurological symptoms and used medication are presented in the article. The results showed the level of adherence to treatment to be dependent on a number of factors. Patients with high levels of anxiety and depression demonstrated lower levels of adherence. The article stresses that it is necessary to diminish anxiety and depression level in patients with clinical and subclinical levels, because they influence significantly physical condition of patients. It is concluded that an attending physician should take into consideration patients’ adherence to therapy.

FINDINGS OF GLYCEMIA MONITORING AFTER THE INSULIN THERAPY INITIATION IN PATIENTS WITH TYPE 2 DIABETES
Abstract
Introduction. Initiation of insulin treatment is an important step in type 2 diabetes patients. Evaluation of carbohydrate level together with CGMS provides opportunity to estimate treatment efficacy. Aim of study is to evaluate CGMS results in type 2 diabetes patients after initiation of insulin treatment and 3 months later. Methods: CGMS was performed in 16 patients with T2DM within one week after hospital discharge. In 11 patients CGMS was repeated in 3 months. Patients requiring initiation of insulin treatment due to ineffective oral antidiabetic treatment were included. Those suffering from acute illnesses or conditions in previous 3 months were excluded from the study. All data are presented in Me[Q25;Q75] format. Results: The level of HbA1c decreased from 9,1[7,9;9,6]% to 7,1[6,8;7,6]% (p=0,006) months later. Decrease in moderate glycemia (9,2[8,0;9,5] mmol/l vs.7,9[7,3;9,1] mmol/l, p=0,03) and duration of glycemia more than 10 mmol/l (29[19;40]% vs. 16[8;29]%, р=0,04) by CGMS were also observed. CGMS results after hospital discharge did not correlate significantly with HbA1c 3 months later while there was strong and statistically significant correlation of CGMS results carried out 3 months later (R=0.89 p=0.0005 for mean glucose). No significant association was observed between HbA1c and glucose variability. Also there was no significant correlation between corresponding results of first and second CGMS.

ETIOTROPIC TREATMENT OPTIONS IN PATIENTS WITH HCV-INFECTION AND CYTOPENIA
Abstract
Aim. The article aims to observe the efficiency of alternative (etiopathogenic) treatment schemes in HCV-patients with limited access to the usual doses of interferon-α caused by cytopenia and without possibility to use modern antiviral drugs of direct action. Materials and methods. Reduced doses of interferon-α with dosed by weight ribavirin and interleukin-2 were administered daily within 48 weeks. Results. Clinical manifestations improved, asthenovegetative and dyspepsial syndromes disappeared or highly reduced, liver size decreased and its function improved (biochemical response). Conclusion. The suggested treatment regimen provides a sustained viral response in 16.6% cases, improves clinical manifestations, biochemical liver function, provides a substitution immunocorrective effect by reducing immune cell-factor imbalance.

The results of comparing the occurrence of risk factors among healthy children and those with hearing disorder are given in the article. The most important of them are family history of hearing syndromal pathology, prematurity and intrauterine infection.
