Abstract

The article presents in comparative perspective the number of platelets and their volume, lipid metabolism, vascular-platelet hemostasis in elderly people without clinical and instrumental manifestations of cardiovascular disease, in patients suffering from coronary heart disease and in the group of patients with ischemic heart disease complicated by persistent forms of atrial fibrillation. Correlation analysis between the studied indicators was conducted. It shows the relationship between the risk of atrial fibrillation and laboratory parameters in geriatric patients with ischemic heart disease.

The efficacy and safety of thrombolytic therapy of acute coronary syndrome with ST-segment elevation in pre-hospital stage

Abstract

The article provides an analysis of the results of thrombolytic therapy in 85 patients with acute coronary syndrome with ST-segment elevation. The patients were divided into groups according to the time from the onset of pain to the conduction of thrombolysis: group 1 CLT up to 1 hour, group 2 with CLT from 1 to 2 hours, group 3 with CLT more than 2 hours. It is shown that the pre-hospital thrombolysis with the use of fibrin-specific plasminogen activator conducted within 1 hour from the onset of the symptoms of the disease is the most effective and safe. When you increase the time before treatment, the effectiveness is reduced.

Characteristics of microcirculation in veterans of war with arterial hypertension

Abstract

The article presents the results of a survey of the functional condition of the microvasculature in combat veterans with arterial hypertension by laser doppler flowmetry. We investigated the condition of basal blood flow, as well as the change of microcirculation affected by occlusion test. The obtained results show significant microcirculatory disturbances in combat veterans suffering from hypertension. Represented data indicate the predominance of passive mechanisms of microcirculation regulation, low adaptive capacity and reserve capabilities of microvasculature in the studied group.

Comparative analysis of methods of diagnosis of chronic fetal hypoxia using clinical epidemiology tests

Abstract

The purpose of the study was to estimate the diagnostic value of different methods of diagnosis of chronic fetal hypoxia, to select the most informative ones and to optimize obstetric tactics. In a randomized prospective study chronic fetal hypoxia was revealed in 216 pregnant women in the group of high risk of developing severe placental insufficiency using diagnostic tests and taking into account the fetal daily rhythmostasis. Retrospectively, we used diagnostic tests of clinical epidemiology to calculate sensitivity, specificity, positive and negative predictive values of the results, the diagnostic accuracy of methods for diagnosing chronic fetal hypoxia, taking into consideration the histological verification of the diagnosis of placental insufficiency and the blood pH determination in the umbilical artery (blood was obtained before the first breath). The study showed that a complex approach to the diagnosis of chronic fetal hypoxia with placental insufficiency, taking into account the fetal daily rhythmostasis, has the greatest diagnostic value in comparison with the other methods.

In the course of a randomized, open, comparative, controlled, parallel-group study evaluated the clinical efficacy of matrix systems to restore contact areas of the teeth of the side group. The study involved 33 patients with defects of hard dental tissues (2 class on Black). Randomly selecting created two groups: 15 patients in the control group, treatment which includes recovery by the retaining ring sectional matrix and the wedge, as well as the study group, 18 patients, instead of fixing ring which set the locking system of the nickel-titanium alloy adaptive worktop. Qualitative evaluation of interventions made in 5 days after the restoration of the proximal regions of the teeth of the side group.

Abstract

We have estimated the efficiency of dental haemostatic kit «Haemoblock DENT» application in planned tooth extraction on patients with idiopathic thrombocytopenic purpura. Comparative analysis of results of treatment in immediate postoperative period of two clinical groups with idiopathic thrombocytopenic purpura has been performed according to the evidentiary medicine principles. Reasonably improved clinical outcomes, such as considerable socket bleeding time decreasing, postoperative alveolitis risk reduction and early masticatory apparatus function recovery have been achieved in all patients of the clinical group where dental haemostatic kit «Haemoblock DENT» has been applied. This result permits to recommend it for clinical use.

Reconstructive surgery in treatment of patients with malignant tumors of the pelvic ring

Abstract

22 patients with malignant tumors of the pelvic ring were observed. 9 (41%) patients underwent reconstructive surgery with endoprosthesis replacement, 5 (23%) - with iliosacral or ischiosacral fixation, 4 (18%) - with combination of steel nails and PMMK, and 4 (18%) - with PMMK only. Results: wide resection was achieved in 82% cases. 4 patients had disease progression within 8-16 months. In all patients functional results after surgery were perfect or good. Complications were observed in 8 (36%) cases.

Results of cytoreductive treatment of patients with synchronous bilobar liver metastases of colorectal cancer

Abstract

The program of study includes 115 patients diagnosed with colorectal cancer, stage 4, with synchronous single bilobar liver metastases. In 61 patients (control group) cytoreductive surgery was carried out with atypical liver resection. In 54 patients (study group) cytoreductive tumor removal was followed by RFA of liver metastasis. RFA bilobar liver metastases allowed to increase significantly the number of cytoreductive operation R0, reduce the amount of intraoperative blood loss, risk of postoperative complications and, consequently, reduce the time of patient’s staying in hospital. RFA improved long-term outcomes, increasing survival rates and quality of life of patients on a FACT-C scale.

Abstract

The leading principles of noninvasive monitoring of hemodynamics are accuracy, reliability, possibility of dynamic supervision over the patient, complexity, existence of the minimum quantity of complications, practicality, low cost, availability of received information. Parameters of noninvasive systemic hemodynamics, which can be determined by the device NICO, play an essential role in the assessment of the condition of a patient when carrying out an anesthetic support during extensive oncologic operations. Intraoperatively we observed variability of cardiac index, cardiac output, and stroke volume depending on used anesthetic. Noninvasive monitoring of hemodynamics is informative and traces indicators of systemic hemodynamics at any surgical manipulations, and serves as a reference point during anesthesia. With application of Desflurane, fast awakening of the patient with smaller influence on hemodynamics in comparison with Sevoflurane is observed.

The intraocular pressure at carrying out accommodative load according to the presence of optical aberrations

Abstract

The relationship of hydrodynamic processes with accommodation receives more and more evidence in the works of various authors. For the first time the influence of accommodative load on the level of IOP in eyes with different values of astigmatism was revealed in this research work. The mean value of intraocular pressure reduction was the greatest in patients with astigmatism up to 0.75 D. Patients with astigmatism up to 0.75 D showed a statistically significant increase in the level of intraocular pressure after accommodative load compared to the initial data.

The content of total amount of flavonoids in air-dry material should be at least 1.5%.
