The course of pregnancy in women dealing with industrial lubricant oils

Abstract

In the article results of the research of a current gestation in 248 women working at the Samara bearing plant (SBP), constantly exposed to industrial lubricant oils (the basic group) in comparison with 220 working women who were not exposed to influence of ecotoxication (the comparison group) are shown. The obtained data reveal that professional activity of the women dealing with ILO lasted more than a year authentically raises risk of complications of a pregnancy and a premature birth.

Features of enterovirus meningitis caused by ЕСНО and Coxsackie viruses in children

Abstract

In the article features of serous meningitis enterovirus etiology course in children of the Samara region in 2008-2010 are shown, when the epidemic rise of a case rate by serous meningitis was marked. The spectrum of activators is submitted: a virus of type ЕСНО have been isolated at the majority of the children, viruses of Coxsackie type have been identified in smaller percent. In the article the features of the clinical course of serous meningitis enterovirus etiology depending on type virus are shown. According to the results of the examination of the patients with serous meningitis enterovirus etiology caused by ЕСНО and Coxsackie viruses, the authors come to the conclusion that the clinical picture of the disease depends on the causative agents.

Abstract

For the diagnosis specification of juvenile idiopathic arthritis and the determination of the importance of pathogenetic changes the research of parameters humor immunity, which are the most informative, is shown in the given pathology, namely: a spectrum cytokines: TNF α, IL6, IL1β, IL4, a level of spontaneous both stimulation lymphocytes and monocytes IL1β. Cytokines value is a main factor in an inductive phase of the immune answer when they cause the development of base reactions of makrophages and antigenspecificaition cells: interaction of these cells with lymphocytes, activation and proliferation the latter, in the certain degree the formation of cells effectors. At late stages of the immune answer, during the realization of effector immunity functions the role cytokines becomes less significant. The level both stimulations lymphocytes and monocytes IL1β reflects the activity of the pathological process and can be used in forecasting outcomes of juvenile idiopathic arthritis.

Features of microelement constituent of blood serum in children suffered from chronic gastroduodenitis in the industrial centre of the Southern Urals Chelyabinsk

Abstract

The aim of the research is to determine the microelement structure of blood serum in children with chronic gastroduodenitis (CGD) who live in industrial centre of the Southern Urals Mountains of Chelyabinsk. The 54 children at the age from 7 to 17 years with CGD confirmed clinically are surveyed. The diagnosis is formulated with the account of clinical classification of chronic gastroduodenitis in children (L.I.Aruin. 1998). Depending on presence or absence Нр the patients have been divided into two groups. Zinc value, iron in blood serum while increasing copper and lead is found to decrease in spite of etiology. And in children with CGD associated with Нр changes of indicators are authentically more marked than in the group of the children with CGD without Нр (р <0,05).

The connection of the genetic factor with clinical, metabolic and immunological characteristics of the patients with psoriasis

Abstract

The article presents the results of the examination of 104 patients ranging in age from 18 to 65, who were given in-patient and out-patient treatment for psoriasis within 2005/2008 in the Republican Dermatovenerologic Dispensary, Municipal Dermatovenerologic Dispensary and “Cosmetological clinic” Ltd. situated in Ufa. The patients were males 76 (73.1%) and females 28 (26,9 %) respectively. The average age of the patients equaled 41.2 ± 2.3. The dependence of clinical, metabolic and immunological characteristics on the genetic factor has been analyzed. It has been found out that statistically genetic predisposition to psoriasis significantly depends on patients’ younger age of disease onset, relative level of concentration of lymphocytes and CD8+ lymphocytes in patients’ peripheral blood, increase in index of psoriasis spreading and severity (PASI), relevant concentration of CD4+CD25+ bright and CD4+CD95+ lymphocytes. The genetic factor has no or little connection with the frequency of psoriasis relapse, development of such complicated forms of psoriasis as arthropathy and erythrodermatitis, biochemical markers of coagulative and anticoagulative systems, concentration of nitriteion (NO), the level of accumulation of lipid peroxidation products and antioxidant ferments activity, endogenous intoxication evidence and intensity.

LINICO-MICROBIOLOGICAL FEATURES OF MICROBIAL ECZEMA AND THE IMPROVEMENT OF THE METHOD OF TREATMENT

Abstract

The research results of 114 patients diseased with microbial eczema are represented. While analyzing the results given it was registered that in case of microbial eczema the significant growth of gram-positive coccal flora, generally represented by Staphylococcus, in lower number – by Streptococcus, is marked in lesion focuses when microorganisms have high semination. The density of gram-negative bacteria in the lesion area also significantly rose. The number of other gram-positive bacteria slightly reduced but without authentic differences. The number of opportunistic causative agents and gram-negative rods greatly rose. The patients were divided into 2 groups: the 1st group was treated by the basic therapy and apparatus for physical therapy of combined exposure Combis1, the 2nd group was treated by the basic therapy. The research showed high effectiveness of combined therapy rather than the basic treatment.

The role of rapid virological response in effective outcome prediction of antivirus treatment in patients with chronic hepatitis C

Abstract

We have studied the role of rapid virological response in treatment outcome in patients with chronic hepatitis C: dependence between sustained virological response and rapid virological response was revealed. Prognostic meaning of other factors (early virological response, viral load and genotype, age of patients and type of therapy) were analyzed too. It was established, that young age, low viral load, genotype 3a and treating with pegylated interferons and ribavirin are associated with higher rate of sustained virological response. The most important predictors of sustained virological response were rapid and early virological responses.

The analysis of disability establishment in the patients with Parkinson disease

Abstract

Parkinson disease (PD) belongs to chronic neurodegeneration disorders and causes of disability of persons of middle and old age. The patients are referred to the medical and social commission to determine the group of disability. Postural instability, falls, restriction of every day activity are main factors of disability, which restrict professional activity. The aim of the research is to analysis of establishment of disability status among the patients with PD. Results: PD is the main cause of disability in the 141 patients (26,7%). The 1st disability group consists of the 32 patients (22,7%), the 2nd group consists of the 74 persons (52,5%), the 3d group includes the 35 patients (24,8%). Among the 276 patients the 137 persons (49,6%) need to correct the disability degree. There are 100 men (36,2%) which need to increase the degree of disability among them. There are also the 37 patients (13,4%) which need to decrease one. Conclusion. Clinic analysis of the determination of the disability stage and everyday activity turn out to be underestimated.

The aim of our research is to study the features of alexithymia of the patients with asthma in combination with essential hypertension. The 110 patients were examined and they were divided into 2 control groups according to basic nosologies and a group with combined pathology. There were more patients with alexithymia in the group with combined pathology then in the control groups. Alexithymia complicated the process of diagnostics and the treatment of the patients. It made worse the quality of life and prognosis. We need to pay attention to features of alexithymia to make treatment more effective.

Computer fotoplantografiya as a reliable method of congenital clubfoot diagnostics

Abstract

Congenital clubfoot is a common deformity of the foot inclined to relapse after surgery. In a dynamic growth monitoring of the child there are difficulties in tracing the eliminated components of clubfoot. The analysis of the treatment of 78 children (119 feet) with a typical form of congenital clubfoot ranging in age from 1 to 3 has been carried out. After surgery computer fotoplantografiya data was obtained in all patients to monitor anatomical and functional state of the foot. This method of diagnosis is reliable, noninvasive and absolutely harmless to the patient.

Early surgical treatment of craniofacial trauma

Abstract

One of the progressively developing branches of neurotraumatology is the surgery of craniofacial injuries. For the previous two decades in the world medicine it has become preferable to define the time for the surgical treatment of face bones fractures as early as possible. The analysis of the patients who underwent surgical treatment for craniofacial traumas in our clinic between 2007 and 2010 was carried out. The reconstructive surgeries caused no deterioration of the brain traumas course in any patient. During the postoperative period every patient showed good aesthetic and functional results. Reconstructive surgical treatment of bone fractures of the upper and midface areas can be performed in the maximally early periods of time.

The results of the application tumor markers in complex diagnostics investigations at a stage of surgical treatment of colorectal metastases of the liver

Abstract

The results of the application tumor markers (CEA, CA199, CA242) in complex tool researches at a stage of surgical treatment of colorectal livers’ metastases are considered in this article. The program consists of 60 patients diagnosed with colorectal cancer. Oncomarkers levels were defined by immunohemoluminescentic method before radiofrequency ablation of livers’ metastases and in 1, 3, 6 and 12 months after operation. Before surgical treatment individual oncomarkers levels were higher then discriminant concentration and depended of the sizes, volume and degree of a differentiation of a metastatic tumor. The researches have proved the efficiency of this oncomarkers in early diagnostics of relapses and a progression process at a stage dispensary observation after surgical treatment of livers metastases.

Abstract

Between 1995 and 2009 in the urological department of the Samara regional clinical hospital for veterans of wars planned simultaneous operations were performed in 256 patients of elderly and senile age. The analysis of the received data has enabled to establish that in the specialized geriatric urological departments more than 12 % of the patients need the performance planned simultaneous operations. Our new technologies developed and introduced in clinical practice enable to get positive clinical effect 2,3 times at the same time to lower economic expenses in comparison with stage by stage surgical treatment and considerably to improve the quality of life of this category of patients.

The problems of treatment of the patients with recurrent inguinal hernia and the ways of their solution

Abstract

The analysis of the treatment of 1057 patients with inguinal hernias was carried out. Recurrent inguinal hernia was revealed in the 103 patients. There were 34 patients undergone previous operations using the methods of plastic stretch among them. The existing classification of recurrent inguinal hernias does not reflect the essence of the problem that requires further additional development. The results of the treatment of recurrent inguinal hernia applying plastics tension ways are considered to be unsatisfactory. The complications were caused after it in 45,6% of the patients and the relapses were in 12,7% of the patients. The solution of the problem of recurrent inguinal hernia requires further study.

Features of the spread of skin melanoma in the Samara region

Abstract

One of the important problems in oncology is early diagnostics of a skin melanoma. Making structurally no more than 10 % from all forms of malignant neoplasms of a skin, it is responsible for 80 % of the lethal outcomes concerning this group of tumors. In spite of the fact that it has external localization very often a melanoma is already diagnosed at late stages of the development. The estimation of quality of medical aid rendering to the patient with a skin melanoma in the Samara region has been carried out. Dynamics of indicators of a case rate, active and early diagnostics, an one year lethality were analyzed. The interrelation of separate ecological factors of environment, the basic social and economic indexes such as the condition of household and industrial wastes, the change of ozone layer concentration, the peculiarity of solar activity, food peculiarities, a standard of living of the population with growth of a case rate of a skin melanoma in the population of the Samara region have been studied.

Computed tomography (CT) images of the temporal bone: the dimensions of anatomical structures of the inner ear in normal health and with Meniere’s disease

Abstract

The aim of this study was to determine the dimensions of anatomical structures of the inner ear on computed tomography (CT) images in normal subjects and to define the type of its changes in patients with Meniere’s disease (MD). CT images of the temporal bone of 29 patients without ear pathology and 30 patients with MD have been evaluated. The dimensions of the different structures of the inner ear were measured, using multiplanar reformations and three-dimensional reconstructions. In patients with MD, the length and the width of the vestibular aqueduct and the width of its external aperture were smaller, the size of the bony island of the lateral semicircular canal was smaller, and the diameter of lateral semicircular canal was bigger, compared with the values from the control group. We also found out that in patients with MD the distances between the bony labyrinth and the posterior petrous surface and between the posterior semicircular canal and the jugular fossa were smaller, than in normal subjects. In patients with MD, we found no significant differences in the majority of the values between affected and nonaffected ears. Our study confirms the hypothesis that congenital and developmental factors, such as hypoplastic vestibular aqueduct, dysplastic lateral semicircular canal and hypoplastic posterior part of the petrous bone, might play a role in pathogenesis of MD.

To the question of thyroidectomy expediency in patients with toxic goiter

Abstract

The comparative analysis of the treatment of 167 patients with various forms of toxic goiter was carried out. Calculations of sizes of retained tissue of thyroid gland 3 grams weight were carried out. Separate results of extremely subtotal resection of thyroid gland and thyroidectomy were studied. The retrospective analysis of 45 preparations of the tissue removed during operation of thyroid gland was carried out. It is established that it is morphologically justified to consider expansion of operative treatment on thyroid with toxic goiter in favor of thyroidectomy in relation with great percentage of lymphoid infiltration that shows high risk of the relapse development of this pathology. After thyroidectomy it is much easier to find hormone replacement therapy for a patient that facilitates its postoperative rehabilitation.

Abstract

The aim of the work was to improve the results of pulmonary resection by developing and implementing ways of sealing the wounds of biological adhesives, and drugs. The results of applying additional methods sealing line of manual and mechanical suture after atypical resection of the lung were analyzed. The methods of aerial and hemostasis are particularly necessary in pulmonary tissue. The application of differential approach to the choice of sealing the seam line enables to reduce the number of complications in the post-operative period.

The choice of approaches to the treatment of patients with open arterial canal

Abstract

The aim of the study is to assess immediate results of surgical and transcatheter treatment of patients with patent ductus arteriosus and to detect risk factors of development residual shunt after PDA occlusion. 253 consecutive patients were included. Surgery group contained 108 patients and transcatheter group contained 145 patients. There were no death cases in both groups. Residual shunts developed in 19 patients (15.83%) in surgery group and in 2 patients (1.37%) in transcatheter group (р<0,0001). Patients with residual shunts in comparison with those with no residual shunts have significantly larger diameter of pulmonary end of the PDA (р<0,0001). Transcatheter occlusion of patent ductus arteriosus can be safely and effectively achieved in patients with ductus diameters up to 3.7 mm.

Grounds of ways and the treatment period of the complications in post-operative patients with traumas and arctation of hepaticocholedochus

Abstract

The results of hepaticoenterostomy operations on a replaceable transhepatic Getts drainage set in 51 patients with traumas and arctation of hepaticocholedochus have been studied. The results of treatment of the patients with traumas and arctations of hepaticocholedochus have shown that the lethality just after the operation is possible in 3,9 %, in the long term 4,1 %. 13,3 % of the patients have complications which are the indication for operations referred to their correction. The patients after Getts operation should be operated for elimination of a hernia and closing functioning of enteral fistulas, and these operations should be carried out after the period of hepaticoenterostomos drainage (2 years) and expositions (not less than 6 months) after drainage removal, excluded the relapse of arctation.

In the urology department of the Samara clinical hospital for war veterans (1995 - 2009) the planned simultaneous operations were performed in 1986 elderly patients with BPH. The investigation revealed the necessity of performing of planned simultaneous operations in more than 10% of the patients with BPH in the specialized geriatric urological departments. In addition to the positive clinical effect it gives the possibility to decrease economic costs 2,3 times less comparatively with the staged surgery and significantly improve the quality of life of those patients.

Abstract

The new scheme for surgical treatment of exotropia: histological basis and clinical effect Tissue fragments of medial rectus muscle’s tendons excised during strabismus surgery were processed for histological investigation with van Gieson’s staining. Maximal resection length was 7mm. The muscle fibers appeared at the distance 7mm from sclera. As there are functionally important palisade nerve endings around the tips of the muscle fibers it is mandatory not to exceed the above mentioned resection value. According to histological data the new scheme for surgical treatment of exotropia was suggested. This scheme allowed to decrease the quantity of postoperative undercorrection and don’t due to overcorrection.

Abstract

Biomechanics bechaviour of human enucleated 35 eyes was studied the anterior-posterior axis of eyes was at the rang 23,7+0,17 mm. The eyes was studied in perfusion apparatus. Pressure-volume curves and calculated rigidity coefficient KR=dP/dV were analyzed. Results: The ocular rigidity varied at the range of 0,0072-0,0354 mm Hg/mm3, the mean was 0,017+0,001 mm Hg/mm3. The rigidity coefficient depended on age (r=0,5, p=0,002), anterior-posterior axis (r=-0,39, p=0,02) and hydrostatic pressure. Ocular rigidity coefficient is common parameter biomechanics bechaviour eye ball.

The blood neutrophilic granulocyte condition and lipid peroxidation – antioxidant system of students on different stages of circannian cycle were evaluated. It was stated that the condition of neutrophils, the malonic dialdehyde level (in plasma and peripheral blood erythrocytes) and catalasa activity (in plasma and peripheral blood erythrocytes) of students – swimmers of the second and the first categories, masters of sport, candidates in master of sport are considerably different from those of students of the same age and sex who do not go in for sports.

Polymorphism of DNA reparation genes in patients with alcoholic liver disease

Abstract

The results of research of polymorphic loci of DNA repair system genes - XRCC1 (rs25487), XPD (rs13181), XRCC3 (rs861539) of patients with alcoholic liver disease are presented. The object of study - DNA samples (N = 82), isolated from blood leukocytes from populations of Bashkortostan Republic suffering from chronic alcoholism with manifestations of alcoholic liver disease. Polymorphic variants G28152A (Arg399Gln) of the XRCC1 gene, A35931C (Lys751Gln) of the XPD gene, C18067T (Thr241Met) of the XRCC3 gene were studied by analysis of restriction fragment length polymorphism products of polymerase chain reaction. We observed the significant increase frequencies both alcoholic liver disease genotype AC (Lys/Gln) gene XPD (OR=1,71; р=0,03) polymorphic locus, and genotype TT (Met/Met) XRCC3 gene (OR=2,46; p=0,03) polymorphic locus in hepatitis as compare to the healthy. We supposed that these genotypes may be used as genetic markers of predisposition to alcoholic hepatitis. Less susceptible to the liver disease persons with genotype AA (Lys/Lys) gene XPD (OR=0,60; р=0,04) and CT (Thr/Met) gene XRCC3 (OR=0,50; p=0,02).

The influence of associative vaccination against pneumococcus and hemophilic type B infections and flu on the data of functional tests in the patients with chronic obstructive pulmonary disease

The article presents analysis of respiratory function and test with a six-minute walking in patients with a chronic obstructive pulmonary disease (COPD) in 12 months after complete vaccination against pneumococcus, H. influenzae type b and influenza compared with unvaccinated patients with COPD. 1 group - 48 patients with COPD, vaccinated simultaneously «Pneumo-23», «Hiberiх», «Grippol plus» (mean age - 61,46±1,17 years). Group 2 - 80 non-vaccinated patients with COPD (mean age 54,65±0,6 years). In 12 months after complete vaccination in patients with COPD noted positive dynamics in the monitored parameters compared with a non-vaccinated patients with COPD.
