Abstract

The problems of vertebrae fractures diagnostics of patients with osteoporosis were considered in this article. The possibility of skeleton scintigraphy in the determination of osteosynthetical processes level in patients with vertebrae fractures of osteoporotic genesis has been studied. The diagnostic efficiency of anatomic methods (СT, MRI, roentgenography) is in comparison with vertebrae metabolic changes visualized by using of scintigraphy with osteotropic indicators.

Abstract

During the open, controlled, parallel-group comparative study we evaluated the feasibility of supplemental silimar drug to treat moderate and severe psoriasis. We surveyed 65 patients: 30 patients in the control group (conventional therapy) and 35 patients in the studied group, to which in addition to conventional treatment (metotreсsat 5 mg every 12 hours 2-3 times a week) we administered the drug silimar at a dose of 0.2 g three times a day 30 minutes before meals for 35 days. The feature of the study is to calculate the quantitative effect of the intervention.

The significance of non-drug therapies in correction of main ultrasound parameters of liquorodynamics in children with cerebral ischemia

Abstract

To increase the effectiveness of standard treatment of cerebral ischemia (CI) in children in the neonatal period, the total magnetic therapy and hyperbaric oxygenation were included. The authors' developed methods were confirmed by the RF patents. The dynamics of the main ultrasound parameters reflecting the state of the ventricular system, the structure of brain tissue and its microcirculation was analyzed. The use of non-drug therapies in treatment of cerebral ischemia contributed to the rapid regression of the size of the ventricles of the brain, restoring their shape, reducing the total index of hyperechogenicity.

Cellular component of a system inflammation in patients with moderate and mild stage of chronic obstructive pulmonary disease during of its exacerbation

Abstract

In this article we cite data which demonstrate cellular component of a system inflammation in patients with moderate and mild stage of chronic obstructive pulmonary disease (COPD) in dynamics of hospital therapy during the exacerbation of the given disease. 17 patients participated in research with mild and 21 patient with moderate stage of COPD. The control group consists of 20 conditionally healthy persons. The received results demonstrate that COPD exacerbations in patients with moderate and mild stage is characterized by strengthening phagocytic activity against decrease in number of such cells. Also in such patients activation Т and B cells, against the raised level of the T-NK-cells that depict presence of chronic inflammation in patients already with a mild stage is observed. In patients of the moderate stage the exhaustion of NK-cells is observed. It demonstrates the weakness of antiviral and antineoplastic immunity.

The clinical and laboratory characteristic of influenza caused by pandemic strain A H1N1 in pregnant women

Abstract

Influenza and other acute respiratory viral infections take first place among infectious diseases. The most severe disease was observed in pregnant women dew to changes in immune, respiratory and cardiovascular systems. Pregnant women were hospitalized in the infectious disease clinic where they were examined. Main flu symptoms caused by a pandemic strain A H1N1 virus are fever, intoxication, tracheobronchitis. The average duration of the hectic period consists of 6 ± 1.3 days. The changes in biochemical study were characterized by low levels of blood lactate dehydrogenase (LDH) up to 300 e/l, creatine phosphokinaze (CPK) to 525 e/l. Severe influenza complications as pneumonia were observed in 15 patients (16%). Peculiarities of the severe influenza course is the lack of correlation between the clinical and radiological picture of lung complications, progression of acute respiratory distress syndrome requiring constant respiratory support.

The same type picture of initial neurophysiological indicators of blink reflex in both studied groups of patients, as well as their unidirectional dynamics as a result of the offered combination therapy in patients with cervical myofascial pain syndrome and cervical and essential hypertension suggests the mechanisms of origin and the maintenance of blood pressure. Registration of the blink reflex dynamics in clinical practice can objectify the severity of comorbidity, and evaluate the effectiveness of treatment and rehabilitation activities. Estimation of parameters of the blink reflex in patients with dysfunction of the cervical spine allows physician to rationally combine manual therapy and acupuncture with drugs.

The spectral opticaltomography study of the layers of the retina in patients with multiple sclerosis

Abstract

In our research work the spectral optical tomography Spectralis OCT BluePeak study of the layers of the retina 39 (77 eyes) patients with multiple sclerosis and 12 (24 eyes) comparatively healthy patients with myopic refraction was carried out. For visualization of the fundus it was clearly identified 8 out of 10 layers of the retina. With the help of the developed method the apparent thickness of each layer with an accuracy of ± 5 microns was calculated. It was shown that the patients with multiple sclerosis in addition to general thinning of the first layer of nerve fibers in the retina peripapillary area had statistically significant thinning of the layer of the retina in ganglion layer paramakula area. The average ganglion layer of the retina in the patients with multiple sclerosis ranged 85,75±1,68 µm, whereas in the control group of patients with myopic refraction of the layer thickness varied 97,92±2,26 µm (p<0,01). Layered evaluation of the retina showed characteristic deficits ganglion layer of the retina in patients with multiple sclerosis, which may be a specific marker of multiple sclerosis.

Abstract

The investigation of the quality of life of patients with rheumatoid arthritis associated with secondary osteoarthritis according to HAQ In this trial it was demonstrated the comparison analysis of the quality of life of patients with rheumatoid arthritis in combination with secondary osteoarthritis according to HAQ. It was revealed the relationship between the HAQ and DAS28 (index of activity). It was carried out the evaluation of efficacy of therapy with the use of low level laser irradiation. It the end of the study we concluded that secondary osteoarthritis in patients with rheumatoid arthritis deteriorate the quality of life. Inclusion in complex treatment the laser therapy improves the quality of life and activity of the disease.

Peculiarities of fatal outcome of patients with community-acquired pneumonia in the ICU

Abstract

A comparative analysis of in-hospital mortality of patients with community-acquired pneumonia (CAP) in the ICU unit in 2010 compared with 2000. It was revealed that among the dead in 2010 the average age dropped to 9 years (in 2000 50,1 (± 5,3) years, in 2011 - 41.2 years (± 4,1) - (p <0.05), the prevalence of men increased from 60% to 66%, a later date treatment (13.5% in 2000, 17.3% in 2010) was noted. Among the dead patients in 2010, there were patients with HIV infection (30%), a complication of the pathogenic H1N1 influenza (10%). The number of deaths from tuberculosis consisted of 8.4%.

To study the structural and functional changes of the right and left heart in primary and secondary arterial hypertension (AH). Materials and Methods. The study included 104 patients with various degrees of hypertension (WHO, 1999), including 58 women and 46 men (mean age 52 ± 7,3 years). Surveyed patients were divided into 2 groups: group 1- 70 patients with essential hypertension (EH) I stage (WHO, 1993), group 2 - 34 patients with secondary hypertension syndrome in primary aldosteronism (PA). Common indicators of systolic and diastolic function (DF) of the right ventricle (RV) and left ventricular (LV) were identified during echocardiography (echocardiography) with color Doppler. Results. The process of cardiac remodeling in primary and secondary hypertension has several stages and is characterized by sequential development of structural and functional changes in right and left heart.

Lack of information about the nature of changes in leptin and its relationship with anti-inflammatory factors in the development of acute coronary events has determined the feasibility of the study. The study included patients with myocardial infarction with and without type 2 diabetes. Identified giperleptinemiya and elevated concentrations of antiinflammatory factors such as CRP and IL-6, as well as their relationship with markers of myocardial necrosis (CK-MB) and leptin can be considered as one of the most important parts of the inflammatory response in myocardial infarction, not only in patients with diabetic history, but also in patients without this comorbidity. The study may form the basis for new diagnostic and therapeutic tactics correction of metabolic abnormalities in patients with acute coronary vascular disease.

Abstract

The steady growth of mortality from MI gives proof of searching of new markers of cardiovascular pathology development. It is known that the key role in the development of atherosclerosis dysfunction is played by oxidatively modified LDL and antibodies to them, generated on the background of oxidative stress. The lack of information about the participation of these markers in the development of myocardial infarction depending on the gravity of the impairment of coronary bed explains the rationale of this study. The patients with myocardial infarction with ST segment elevation with hemodynamically significant stenosis ≥ 75% of one, two, three or more of the coronary arteries were examined. The increase of concentration of oxLDL and antibodies to them on the background of increasing markers of lipid spectrum and indicators pro-/antioxidant systems, continuing and in the subacute period MI, may be a reflection of the gravity of the defeat of coronary bed, in connection with which significantly expand the possibilities of diagnostics of acute as well as recurrent coronary events in patients with myocardial infarction with ST segment elevation in conjunction with multivascular defeat.

Interleukin-1receptor antagonist in hypertensive patients with varying degrees of cardiovascular complications

Abstract

Purpose. The aim of this study was to evaluate the concentration of interleukin-1 receptor antagonist in hypertensive patients with varying degrees of cardiovascular complications. Materials and methods. In 161 patients with I, II grade arterial hypertension aged 35 to 56 years, divided into 3 groups depending on the risk of development of cardiovascular complications were gone examination including echocardiography, ultrasound dopplerography of brachiocephalic vessels , daily blood pressure monitoring, lipid spectrum, glucose level, creatinine level, glomerular filtration rate and IL-1(ra) concentrations in blood serum. Results. In hypertensive patients with moderate cardiovascular risk the severity of inflammatory process depends on the presence of abdominal obesity. In hypertensive patients with high risk of cardiovascular complications direct correlation between IL-1(ra) and intima-media thickening and inverse correlation between IL-1(ra) concentration and GFR was found, it can indicate the association of inflammatory process, renal damage and structural changes of vascular wall which can play a crucial role in pathogenesis of arterial hypertension and development of organ damage.

The surgical reconstruction of left ventricle in patients with coronary heart disease complicated by heart failure

Abstract

From 1996 till 2010 in our hospital 136 patients with coronary heart disease complicated by heart failure underwent surgical treatment. The patients were divided into 2 groups: I - the patients with a left ventricular aneurysm (n = 70), II - the patients with ischemic cardiomyopathy (ICMP) (n = 66 patients). All the patients underwent coronary artery bypass surgery, reconstruction of the left ventricle (Dor procedure), the correction of mitral regurgitation. Hospital mortality in group II was 5.4%, all the patients in group I had surgery safely. All the surviving patients after surgery have decreased the size of the left ventricle decreased spherical cavity of the ventricle, increased its fraction. 10 - year survival rate in group I was 78% in the II - 56,5%. Freedom from heart failure of high functional classes (NYHA III, IV), 10 years after surgery the patients in group I was 80.2%, in group II - 68%. The surgical procedure on all components of cardiac pathology in the patients with coronary artery disease and heart failure can achieve satisfactory long-term results.

The variability of heart beat in hypertensive patients with the acute ischemic stroke period

Abstract

211 patients aged from 40 to 84 were surveyed. The basic survey sample was 103 hypertensive patients in the acute ischemic stroke period, the control group consisted of 78 hypertensive patients without stroke, the healthy control group included 30 apparently healthy patients. The time indices of Heart Rate Variability were estimated on the evidence of ECG monitoring. The depletion of Heart Rate Variability was diagnosed in the patients of the basic group, maximally - in case of the acute stroke. The degree of Heart Rate Variability depletion correlated with age, severity of stroke, waist circumference, body-weight index, hyperglycemia level.

Internal bronchial ozone and laser therapy in children with chronic bronchitis

Undergone ozone and laser therapy 42 children with chronic bronchitis were examined. The children were divided into 4 groups: the first group was a control one, the second group consisted of the patients undergone laser therapy, the third group included the children undergone ozone therapy, the fourth group consisted of the patients undergone combined ozone and laser therapy. The best effect was noted in the group of combined use of ozone and laser therapy, in which after the treatment the indexes of bronchoalveolar lavage fluid were improved, the level of catalase was increased, succinate dehydrogenase, the amount of dien conjugate and malone dialdehyde was decreased.

Peculiarities of serological indicators (specific Ag A and Ig G to gliadine and antibodies to tissue transglutaminase) in children with celiac

Abstract

The article presents the research results of serological indicators in 108 children ranging in age from 5 months to 18 years with celiac disease who are examined and treated in the gastroenterology department of N.N. Ivanova ММBI city clinical hospital №1. Antibodies to gliadine (AG) class Ig A, Ig G and antibodies to tissue transglutaminase (ATTT) are taken into account as serological markers. The increase of the AGA than АТТT is more often revealed in the examined patients. In most cases, the increased AG class IgG and most of these changes are recorded in the second group of the children. The increase of antibodies to gliadine is more frequently registered in typical celiac. In the patients with atypical form the increase as AG, so АТТT is revealed. In the children with a latent form, we identify only the increase of the AG class IgG. In the patients ranging in age from 6 to 12 month with the manifestation of celiac the increase of AG and АТТT are more often registered than in other age periods. In the early manifestations of celiac (up to 6 months) the increase of AG is only noted, АТТT remains within the age limit, which may be due to violation of the production in the children of this age.

Up-to-date approaches to anesthesia during the mini cholecystectomy access

Abstract

The advantages of the type of anesthesia during the mini cholecystectomy access with the help of the comparative analysis of the impact of different types of anesthesia in the patients with underlying medical conditions were presented. The criteria of anesthesia choice, depending on the source physical status of the patient were offered. 120 patients with different concomitant diseases, aged 25 to 75 years (mean age 41,84 ± 1,01 years) who underwent cholecystectomy from mini to routinely access: under combined anesthesia by inhalation anesthetics (60 patients) and total intravenous anesthesia with mechanical ventilation (60 patients) were examined. All the patients during surgery underwent noninvasive monitoring of blood pressure and heart rate, oxygen saturation, blood gases. It was shown that the minimal hemodynamic changes occured during the induction and maintenance of anesthesia based on sevoflurane or isoflurane.

The obtained treatment results of the carotids stenosis impairments testify to safety and high efficiency carotid angioplasty with stenting.

The treatment results of chest injury, complicated by hemothorax with suppressed haemorrhage managed by active sanitation of pleural cavity

Abstract

After the application of pleural cavity active sanitation on 44 patients ranging in age from 15 to 86 with chest injuries, complicated by hemothorax with suppressed haemorrhage, the analysis has been done. Since 2010 the method of pleural cavity active sanitation through drainage tube, invented by the author (Patent RF method invention application № 2456935, 27.07.2012, Izmaylov E.P. et al.) has been used in conservative treatment. The essence of the method is fractional lavage of pleural cavity with physiological and/or novocaine solution carried out through Bülau drain, followed by broad-spectrum antibiotic done interpleurally. 4-5 days after the injury magnet therapy and/or calcium chloride electrophoresis should be done on the chest on the side of the injury 5 to 7 times. Depending on the type of conservative treatment all patients were divided into 2 groups. The test group consisted of 60 (57,7%) patients who were treated traditionally. 36 (60%) patients with concomitant chest injuries were included in this group. The main treatment group consisted of 44 (42,3%) patients, who had active sanitation of pleural cavity. 15 (34%) patients with concomitant chest injuries were among them. The patient groups were matched by condition, age, concomitant diseases and nature of injuries. 104 patients were with chest injuries, complicated by hemothorax with suppressed haemorrhage, 9 people died (8,6%). Active sanitation of pleural cavity for patients with chest injury, complicated by hemothorax with suppressed haemorrhage, has reduced the number of thoracotomies by 28,6% and the number of deaths by 3,2%.

This task was realized by us due to the improvement of the method of preparation of deep burns for autodermoplasty with the offered wound cover "AKTIVTEX BTEM." The combination of these drugs - butola and eucalyptus oil has strong antibacterial, antinecrotic, dehydrating properties, high penetration into the tissue.

Abstract

Togliatti is a region with high spreading of HIV infection among the population. It heads the list in the region and it is second in Russia. the situation of HIV spreading and tuberculosis among HIV-infected citizens of Togliatti is studied. The analysis of mortality in comorbidity of HIV / TB is presented. It is revealed that tuberculosis is a major disease and the cause of death among HIV-infected patients. The greatest risk for mortality in the infected HIV patients in 2000/20001, with the decreasing of CD4 - cells less than 200 cells in mсl. (73%) is revealed. The death from TB patients occurs at an average in 3.6 months from the date of tuberculosis manifestation. HAART prescription in these cases is not affected on the unfavorable outcome. HAART taking at an earlier date has reduced the incidence of tuberculosis among HIV-infected patients at 16%.

The treatment pathomorphism assessment of prostate cancer afterthe hormonal therapy

Abstract

For the purpose of studying treatment effects of neoadjuvant androgen deprivation in prostate cancer, histological material from 130 patients are examined. 30 patients had received hormonal treatment before radical prostatectomy. All patients were undergone TRUS-guided 12-cores biopsy. Histological, computer morphometry, immunohistochemical methods were used. Rate of (+) surgical margin in group with the hormonal treatment was 13%, in the control group - 17%. The response to androgen deprivation was variable, with areas of the cancer appearing unaffected. The important task is to identify the grade of treatment effects, because the therapy depends on it.

Foetal communications morphology at Sudden Infant Death Syndrome (SIDS)

Abstract

For the purpose of foetal communications morphology studying in children died from Sudden Infant Death Syndrome (SIDS) the investigation of 35 newborn and infant hearts with SIDS was performed. 38 infant hearts with sudden death caused by gastric aspiration and infection diseases made the control group. Examination of hearts was done in gross with separate weighting and with use of histological methods. Much more late time of foetal communications obliteration (under 8 month of life) in infants with SIDS and also increase in both ventricles weight were revealed. Connected tissue is mainly located in the oval foramen area of interatrial septum. These factors lead to hemodynamic abnormalities and may play an important role in SIDS.
