Moreover, hospitalization itself increases the risk of nosocomial infections. Patients with organic lesions of the central nervous system, schizophrenia, and alcohol dependence are particularly vulnerable, but research work in this area is limited.</p> <p>The aim: to study peculiarities and risk factors for infectious complications (IC) in patients with mental disorders hospitalized in a multidisciplinary hospital.</p> <p>Material and methods. A retrospective study was performed using “Clinical, laboratory, and pharmacotherapeutic peculiarities of the onset and clinical course of infectious complications in patients with psychosomatic profiles (CLIOPS)” database. The study included 727 patients (385 males and 342 females). Frequency and types of IC, their association with underlying diagnosis, and risk factors were analyzed. All the participants were divided into two groups: 360 (49.5%) without IC and 367 (50.5%) with IC. Nosocomial infections accounted for 61.3% of the infectious complications.</p> <p>Results. The main infectious complications in the studied patients were pneumonia (59.7%, of which 67.6% were nosocomial), urinary tract infections (13.9%), and intra-abdominal infections (6.5%). In patients with organic mental disorders, including symptomatic ones, the incidence of IC was statistically higher than in other groups: χ²(4) = 51.51 (p &lt;0.001). Risk factors for the development of IC included residence in boarding facilities (odds ratio (OR) 1.58), surgical interventions (OR 4.64), use of antipsychotic drugs (OR 2.6), delirium (OR 2.05), and cardiovascular diseases (OR 1.52).</p> <p>Conclusion. IC are a key cause of hospitalization and mortality in psychiatric patient population. The highest risk of infectious complications is associated with organic CNS lesions. Delirium and the use of psychotropic medications require special monitoring in this category of patients. The development of specialized infection monitoring programs in psychiatric hospitals is necessary.</p></abstract><trans-abstract xml:lang="ru"><p>Инфекционные заболевания остаются одной из ведущих причин госпитализации и летальности среди пациентов с психическими расстройствами. При этом госпитализация сама по себе повышает риск внутрибольничных инфекций. Особую уязвимость демонстрируют пациенты с органическими поражениями центральной нервной системы, шизофренией и алкогольной зависимостью, однако исследования в этой области ограничены.</p> <p>Цель – изучить особенности и факторы риска инфекционных осложнений (ИО) у пациентов с психическими расстройствами, госпитализированных в многопрофильный стационар.</p> <p>Материал и методы. Проведено ретроспективное исследование на основе базы данных «Клинико-Лабораторные и фармакотерапевтические особенности дебюта и течения Инфекционных Осложнений у пациентов Психосоматического профиля (КЛИОПС)» 727 пациентов – 385 мужчин и 342 женщин. Анализировались частота и типы ИО, связь с основным диагнозом, факторы риска. Участники были разделены на две группы: 360 (49,5%) человек без ИО и 367 (50,5%) с ИО. Среди ИО 61,3% составили нозокомиальные инфекции.</p> <p>Результаты. Основными ИО у исследованных пациентов были пневмонии (59,7%, из них 67,6% – нозокомиальные), инфекции мочевыводящих путей (13,9%), интраабдоминальные инфекции (6,5%). У пациентов с органическими психическими расстройствами, включая симптоматические, частота ИО была статистически выше, чем в других группах: χ²(4) = 51,51 (p &lt;0,001).Факторами риска развития ИО выступали проживание в интернатах (отношение шансов (ОШ) 1,58), оперативные вмешательства (ОШ 4,64), прием антипсихотиков (ОШ 2,6), делирий (ОШ 2,05) и сердечно-сосудистые заболевания (ОШ 1,52).</p> <p>Заключение. ИО – ключевая причина госпитализации и летальности в популяции психиатрических пациентов. Наибольший риск ИО ассоциирован с органическими поражениями ЦНС. Особого контроля в этой категории больных требуют делирий и прием психотропных препаратов.
