Increasing of disease activity leads to disease progression, and assessing this parameter requires searching for specific biomarkers.</p> <p><bold>The aim:</bold> to evaluate the value of 8-isoprostane and melatonin as biomarkers of COPD activity.</p> <p><bold>Material and methods. </bold>One hundred patients in the acute phase of COPD (69 males and 31 female, clinical group E, mean age 60.5 [52.5; 68.5] years) were examined. Changes in salivary melatonin and serum 8-isoprostane levels were assessed. A correlation analysis was made between these biomarkers and the clinical and functional parameters of patients, including the frequency of hospitalizations after one year.</p> <p><bold>Results.</bold> A direct correlation was established between salivary melatonin levels and forced vital capacity (FVC), forced expiratory volume in 1 second (FEV<sub>1</sub>), and peak expiratory flow rate (PEF), and an inverse correlation was found with the modified mMRC score. Serum 8-isoprostane levels were inversely correlated with the six-minute walk test results and spirometry parameters – FVC, FEV<sub>1</sub>, and peak expiratory flow rate 25, 50, and 75% (MOC<sub>25</sub>, MOC<sub>50</sub>, and MOC<sub>75</sub>). A direct correlation was found between 8-isoprostane concentrations and CAT and mMRC scores. By day 10, a decrease in 8-isoprostane levels and an increase in melatonin levels were observed. These results indicate the oxidant properties of 8-isoprostane and the antioxidant activity of melatonin. After one year of follow-up, statistically significant associations were found between the levels of these biomarkers and the frequency of hospitalizations.</p> <p><bold>Conclusion.</bold> The obtained data confirm that 8-isoprostane and melatonin may serve as promising biomarkers for monitoring COPD activity.</p></abstract><trans-abstract xml:lang="ru"><p>Ключевым нововведением Глобальной инициативы по хронической обструктивной болезни легких (ХОБЛ) (GOLD) в 2026 г. стала концепция «активность ХОБЛ». Повышение активности заболевания приводит к его прогрессированию, и для оценки этого параметра требуется поиск специфических биомаркеров.</p> <p><bold>Цель</bold> – оценить значимость 8-изопростана и мелатонина в качестве биомаркеров активности ХОБЛ.</p> <p><bold>Материал и методы.</bold> Обследованы 100 пациентов в фазе обострения ХОБЛ (69 мужчин и 31 женщина, клиническая группа Е, средний возраст 60,5 [52,5; 68,5] года). Оценивалась динамика изменения уровня мелатонина в слюне и 8-изопростана в сыворотке крови. Проводился корреляционный анализ между указанными биомаркерами и клинико-функциональными параметрами больных, частотой госпитализаций через год.</p> <p><bold>Результаты.</bold> Установлена прямая корреляционная связь уровня мелатонина в слюне с форсированной жизненной емкостью легких (ФЖЕЛ), объемом форсированного выдоха за 1-ю секунду (ОФВ<sub>1</sub>), пиковой объемной скоростью выдоха (ПОС) и обратная – с количеством баллов по модифицированному опроснику mMRC. Уровень 8-изопростана в сыворотке крови имел обратную связь с результатами теста шестиминутной ходьбы и показателями спирометрии – ФЖЕЛ, ОФВ<sub>1</sub>, максимальной объемной скоростью при выдохе 25, 50, 75% (МОС<sub>25</sub>, MOC<sub>50</sub>, MOC<sub>75</sub>). Прямая связь концентрации 8-изопростана была выявлена с количеством баллов по данным опросников CAT и mMRC. К 10-му дню отмечалось снижение уровня 8-изопростана и повышение содержания мелатонина. Полученные результаты указывают на оксидантные свойства 8-изопростана и антиоксидантную активность мелатонина.
