The most common complaints with which patients seek medical care, as well as possible correlations between bilirubin levels and comorbidities, require clarification. The question of the presence of gender differences in the structure of concomitant clinical symptoms in GS is of particular interest.</p> <p><bold>The aim: </bold>to identify the clinical features, frequency, and spectrum of comorbidities in adult patients with GS.</p> <p><bold>Material and methods. </bold>A retrospective study included 53 patients (24 male and 29 female individuals, median age 40 years) with benign hyperbilirubinemia (GS).</p> <p><bold>Results.</bold> All subjects with GS had gastrointestinal complaints. Approximately half of the patients had GS combined with gastroesophageal reflux disease and irritable bowel syndrome (56.6 and 47.2%, respectively). Dyspeptic symptoms were present in 90.6% of the subjects. Association between biliary sludge and gallbladder dysfunction was statistically significant (p &lt; 0.05). Hepatic steatosis was diagnosed in 20.8% of patients, with a significant association with obesity (p &lt; 0.01). GS was combined with cholelithiasis in a third of the cases, and 7 subjects (13.7%) had a history of cholecystectomy. Chronic gastritis associated with Н. рylori was diagnosed in 12 patients (22.6%), and gastric ulcer and duodenal ulcer were diagnosed in 3 (5.7%) persons. Individuals with the homozygous GS genotype (7TA/7TA) had higher bilirubin levels (38.2 versus 27.4 μmol/L in heterozygous carriers; p &lt; 0.01).</p> <p><bold>Conclusion.</bold> GS is a common hereditary condition that requires dynamic monitoring. In adult patients with GS, episodes of jaundice are accompanied by a variety of clinical manifestations of dyspeptic nature, and among gastrointestinal diseases, gastroesophageal reflux disease, irritable bowel syndrome, gallbladder dysfunction, and cholelithiasis are most often diagnosed. No gender differences in the pattern of complaints were identified in patients with GS.</p></abstract><trans-abstract xml:lang="ru"><p>Высокая частота распространенности синдрома Жильбера (СЖ) среди населения РФ, его клинический полиморфизм диктует необходимость тщательного изучения ассоциированных с ним состояний и заболеваний. Уточнения требуют наиболее частые жалобы, с которыми пациенты обращаются за медицинской помощью, а также возможные корреляции между уровнем билирубина и сопутствующей патологией. Особый интерес вызывает вопрос, существуют ли половые различия в структуре сопутствующих клинических симптомов при СЖ.</p> <p><bold>Цель</bold> – выявить особенности клинической картины, частоту и спектр сопутствующих заболеваний у взрослых пациентов с СЖ.</p> <p><bold>Материал и методы.</bold> В ретроспективное исследование были включены 53 пациента (24 мужчины и 29 женщин, медиана возраста 40 лет) с доброкачественной гипербилирубинемией (СЖ).</p> <p><bold>Результаты.</bold> Все исследуемые с СЖ имели жалобы со стороны гастроинтестинального тракта. Примерно у половины больных СЖ сочетался с гастроэзофагеальной рефлюксной болезнью и синдромом раздраженного кишечника (56,6 и 47,2% соответственно). Симптомы диспепсического характера имели 90,6% участников. Связь между билиарным сладжем и дисфункцией желчного пузыря была статистически значимой (p &lt; 0,05). Стеатоз печени диагностирован у 20,8% больных, при этом установлена его достоверная связь с ожирением (p &lt; 0,01). В трети случаев СЖ сочетался с желчнокаменной болезнью, холецистэктомия в анамнезе была у 7 (13,7%) исследуемых. Хронический гастрит, ассоциированный с Н. рylori, был диагностирован у 12 (22.6%), язвенная болезнь желудка и двенадцатиперстной кишки – у 3 (5,7%) человек. У лиц с гомозиготным генотипом СЖ (7ТА/7ТА) отмечались более высокие показатели билирубина (38,2 против 27,4 мкмоль/л у гетерозиготных носителей; p &lt; 0,01).</p> <p><bold>Заключение. </bold>СЖ – распространенное наследственное состояние, требующее динамического наблюдения. У взрослых пациентов с СЖ эпизоды желтухи сопровождаются разнообразными клиническими проявлениями диспепсического плана, а среди заболеваний желудочно-кишечного тракта чаще диагностируются гастроэзофагеальная рефлюксная болезнь, синдром раздраженного кишечника, дисфункция желчного пузыря и желчнокаменная болезнь.
