Структура распространенности сердечнососудистых заболеваний и хронической болезни почек у пациентов с сахарным диабетом 2-го типа стационарного звена

Аннотация
Сахарный диабет 2-го типа (СД 2) считается весомым фактором риска развития кардиоренальной патологии. Цель изучить распространенность атеросклеротических ССЗ (АССЗ), ХБП, ХСН у пациентов с разной длительностью СД 2, госпитализированных для стационарного лечения, и выявить предикторы их раннего развития и прогрессии. Материал и методы. В исследование были включены 1150 пациентов с СД 2 (483 мужчины и 667 женщин), госпитализированных в ГКБ им. В.П. Демихова за период 2020-2021 гг. Всем выполнялось стандартное клинико-лабораторное и инструментальное обследование, а также коррекция терапии. Для оценки роли СД в развитии кардиоренальной патологии все пациенты были стратифицированы на две группы по длительности болезни: в 1-ю группу вошли пациенты с СД 2 <5 лет, во 2-ю с СД 2 >10 лет. Результаты. Средний возраст исследованных пациентов составлял 65 [57;74] лет, медиана ИМТ 31,2 [26,3; 35,8] кг/м2. Средний показатель HbA1c был равен 9,6 [7,9; 10,9] %, 88,6% пациентов имели HbA1c >7%. Медиана глюкозы крови натощак составила 7,6 [6,4; 9,2] ммоль/л, постпрандиальной глюкозы 11,4 [9,7; 14] ммоль/л. У 69,8% участников регистрировалась дислипидемия. Из общего количества пациентов с СД 2 87,6% страдали АГ, 56% ХСН (43,91% ХСН с сохраненной ФВ, 12,09% ХСН с умеренно или сниженной ФВ), у 49,9% имелись АССЗ (ИМ 24,7%, инсульт 18,9%, ЗАНК 17%, стенокардия 6%). Самыми значимыми предикторами развития АССЗ, ХБП и ХСН были возраст >60 лет, длительность СД, АГ и ХБП. Мужской пол ассоциировался с увеличением шансов развития АССЗ, женский ХСН и ХБП. Заключение. Среди исследованных пациентов отмечалась высокая частота встречаемости АССЗ, ХСН и ХБП вне зависимости от длительности СД. Наблюдалась высокая распространенность основных факторов риска ССЗ: высокого уровня HbA1c, АГ, ожирения и дислипидемии. На основе проведенного анализа установлены факторы риска, ассоциированные с осложнениями со стороны сердечно-сосудистой и почечной систем у данной когорты больных.
