Studying ischemic stroke characteristics within the first five days after onset may help optimize therapy and predict outcomes.</p> <p><bold>Aim:</bold><italic> </italic>To evaluate the prognostic significance of water-electrolyte imbalance in the hyperacute period of cardioembolic and atherothrombotic ischemic stroke subtypes regarding disease progression and outcomes.</p> <p><bold>Methods</bold>:<italic> </italic>This observational, single-center, prospective, randomized, single-arm study included 96 patients aged 30–80 years during the hyperacute period of severe ischemic stroke of cardioembolic and atherothrombotic subtypes. ROC analysis was conducted to determine the prognostic value of plasma sodium levels for disease outcomes.</p> <p><bold>Results:</bold><italic> </italic>Ischemic stroke occurred in the carotid basin in 89 of 96 patients and in the vertebrobasilar basin in 7 of 96 patients, respectively. The atherothrombotic subtype was predominantly associated with vertebrobasilar basin lesions, whereas the cardioembolic subtype was primarily associated with carotid basin lesions. The highest mortality was observed in patients with hyperosmolar hypernatremic syndrome. ROC analysis demonstrated strong prognostic significance of plasma sodium levels in the hyperacute period for predicting mortality, valid across both cardioembolic and atherothrombotic ischemic stroke subtypes as well as for the entire cohort.</p> <p><bold>Conclusion:</bold><italic> </italic>Developed in the hyperacute phase of ischemic stroke, hyperosmolar hypernatremic syndrome is an independent predictor of fatal outcomes. High prognostic value of plasma sodium levels was confirmed during the hyperacute period across different pathogenetic subtypes of ischemic stroke.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Обоснование.</bold> Острое нарушение мозгового кровообращения — одна из главных проблем современной медицины. Наиболее часто встречающийся тип этого нарушения — ишемический инсульт (ИИ). Изучение особенностей течения ИИ в первые пять дней с момента начала заболевания поможет подобрать оптимальную терапию и предсказать исход болезни.</p> <p><bold>Цель.</bold> Оценка прогностической ценности водно-электролитных нарушений в острейшем периоде кардиоэмболического и атеротромботического подтипов ИИ для течения и исходов заболевания.</p> <p><bold>Методы.</bold> В обсервационное одноцентровое проспективное выборочное неконтролируемое исследование включены 96 пациентов в острейшем периоде тяжёлого ИИ кардиоэмболического и атеротромботического патогенетических подтипов в возрасте от 30 до 80 лет. В целях определения прогностической значимости уровня натрия плазмы крови на исход заболевания выполнен ROC-анализ.</p> <p><bold>Результаты.</bold> У 89 пациентов из 96 диагностирован ИИ в каротидных бассейнах, у 7 в вертебрально-базилярном бассейне. Атеротромботический подтип преимущественно определялся при локализации ИИ в вертебрально-базилярном бассейне, а кардиоэмболический — в каротидном бассейне. Результаты исследования показывают, что наибольшая летальность регистрируется при гиперосмолярном гипернатриемическом синдроме. ROC-анализ показал хорошую прогностическую значимость уровня натрия плазмы крови в острейшем периоде заболевания для определения летальности. Такой вывод был справедлив как для кардиоэмболического, так и для атеротромботического подтипов ИИ, а также для выборки в целом.</p> <p><bold>Заключение.</bold> Гипернатриемический гиперосмолярный синдром, развившийся в острейшем периоде ИИ, является самостоятельным предиктором летального исхода.
