approach to the use of scales for assessing the level of consciousness, functional and general somatic status for rehabilitation and intensive care units is statistically and clinically insufficiently substantiated.</p> <p><bold>Aims:</bold> development and substantiation of a typical scheme for the clinical use of scales of the level of consciousness, general somatic and functional status of patients with consequences of brain damage of various etiologies.</p> <p><bold>Methods.</bold> A single-center prospective, retrospective study that included 118 patients on 20–50 days after traumatic brain injury, anoxic damage; consequences of acute cerebral circulatory disorders, and neurosurgical operations. In groups of patients who are in a coma, vegetative status, a state of minimal consciousness and a state of clear consciousness, the analysis of the statistical quality of clinical scales was carried out: scales of consciousness: the Glasgow Coma Scale (GCS); FOUR (Full Outline of UnResponsiveness); Coma Recovery Scale-Revised (CRS-R); the Glasgow Outcome Scale (GOS); the Extended Scale of the Glasgow Coma (Glasgow Outcome Scale Extended, GOSE); Rancho Los Amigos scale — scale of consciousness and interaction with the environment (the Rancho Los Amigos scale, RLAS); Functional status scales: DRS (the Disability Rating Scale); modified Rankin Scale mRS; Barthel Activities of Daily Living (ADL) Index; Functional Independence Measure (FIM); Karnovsky Index; Rivermead Mobility Index self-assessment of everyday life opportunities in Merton and Sutton (checklist self-maintenance Merton and Sutton community NHS trust); Scales of General somatic severity: assessment of multi-organ dysfunction on the APACHE II scale (Acute Physiology and Chronic Health Evaluation); APACHE IV scale; SAPS II scale (Simplified Acute Physiology Score); SOFA scale (Sequential Organ Failure Assessment). The functional state of the ANS was recorded using HRV parameters by recording 5-minute RR intervals of the electrocardiogram.</p> <p><bold>Results.</bold> There are 4 groups of patients who are in a state of coma, vegetative status, a state of minimal consciousness and a state of clear consciousness. In 67 patients out of 118 examined (56.8%) after brain and brain damage as a result of TBI, anoxia, stroke, the parameters of ANS functional activity are in the zone of pathological (sympathetic/parasympathetic hyperactivity) values. Manifestations of sympathetic/parasympathetic hyperactivity is progressively reduced from the patients in a coma to patients in clear consciousness.</p> <p><bold>Conclusions.</bold> Various scales of the level of consciousness, functional and general somatic severity of patients with the consequences of brain damage, objectively differ in clinical significance in the gradation of coma–vegetative status–minimal consciousness–clear consciousness.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Обоснование.</bold> Применение шкал оценок уровня сознания, функционального и общесоматического статуса для отделений реабилитации и интенсивной терапии статистически и клинически недостаточно обосновано.</p> <p><bold>Цель исследования</bold> — обосновать типовую схему клинического применения шкал уровня сознания, общесоматического и функционального статуса пациентов с последствиями повреждения головного мозга различной этиологии.</p> <p><bold>Методы. </bold>Одноцентровое проспективное, ретроспективное исследование, в которое включены 118 пациентов с повреждением головного мозга различной этиологии на 20–50-е сут после нарушения сознания. В группах пациентов, находящихся в коме, вегетативном статусе, состоянии минимального сознания и состоянии ясного сознания, проведен анализ статистического качества следующих клинических шкал: шкал сознания (шкала ком Глазго GCS; шкала подробной оценки состояния ареактивных пациентов FOUR; пересмотренная шкала восстановления после комы CRS-R; шкала выхода из ком Глазго GOS; расширенная шкала выхода из ком Глазго GOSE; шкала сознания и взаимодействия с окружающей средой Ранчос Лос Амигос RLAS); шкал функционального состояния (шкала инвалидности DRS; модифицированная шкала Рэнкина mRS; индекс активности повседневной жизнедеятельности Бартела ADL; шкала функциональной независимости FIM; индекс Карновского; индекс мобильности Ривермид RMI; шкала самооценки бытовых возможностей повседневной жизни Мертон и Саттон); шкал общесоматической тяжести (оценка мультиорганной дисфункции по шкалам APACHE II и APACHE IV; шкала упрощенной оценки физиологических расстройств SAPS II; шкала динамической оценки органной недостаточности SOFA). Функциональное состояние автономной нервной системы регистрировали по параметрам вариабельности ритма сердца путем пятиминутной записи RR-интервалов электрокардиограммы.</p> <p><bold>Результаты.</bold> Выделены 4 группы пациентов, находящихся в состоянии комы, вегетативного статуса, минимального и ясного сознания. У 67/118 (56,8%) пациентов после повреждения головного мозга в результате черепно-мозговой травмы, аноксии, острого нарушения мозгового кровообращения параметры функциональной активности автономной нервной системы находились в зоне патологических (симпатическая/парасимпатическая гиперактивность) значений. Проявления симпатической/парасимпатической гиперактивности прогрессивно снижаются — от пациентов, находящихся в состоянии комы, до пациентов в ясном сознании.</p> <p><bold>Заключение.</bold> Различные шкалы уровня сознания, функциональной и общесоматической тяжести пациентов с последствиями повреждения головного мозга объективно отличаются между собой клинической значимостью в градации кома–вегетативный статус–минимальное сознание–ясное сознание.</p></trans-abstract><kwd-group xml:lang="en"><kwd>GCS</kwd><kwd>FOUR</kwd><kwd>CRS-R</kwd><kwd>GOS</kwd><kwd>GOSE</kwd><kwd>RLAS-R</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>шкала ком Глазго</kwd><kwd>FOUR</kwd><kwd>CRS-R</kwd><kwd>GOS</kwd><kwd>GOSE</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><mixed-citation>Zappa S, Fagoni N, Selleri C, et al.
