Additional chapters, forms, and appendices designed to include specific information about the rehabilitation process included the physical and rehabilitation medicine doctor examination list, multidisciplinary rehabilitation team members, examination lists, rehabilitation routing scale blank, list of MDRC discussions, individual patient management forms, standard rehabilitation discharge form, and list of rehabilitation quality criteria for adult patients with MSA/PNS dysfunction. Validated scales and test forms were also included.</p> <p>The unification of rehabilitation medical records for patients with MSA/PNS impairment makes it easier for the staff of the rehabilitation departments and centers to file medical records in accordance with the current legislation.</p></abstract><trans-abstract xml:lang="ru"><p>В настоящее время каждое реабилитационное учреждение решает проблему оформления реабилитационных медицинских карт эмпирически, исходя из существующих реалий конкретного учреждения.</p> <p>Цель нашего исследования ― разработка макетов формализованной медицинской карты пациента с нарушением функции периферической нервной системы и костно-мышечной системы, предназначенных для второго и третьего этапов медицинской реабилитации.</p> <p>Предложенный макет карт соответствует утверждённым в настоящее время формам 003/у (Медицинская карта стационарного больного) и 025/у (Медицинская карта пациента, получающего медицинскую помощь в амбулаторных условиях), но дополнен разделами, позволяющими вносить информацию в соответствии с требованиями Порядка организации медицинской реабилитации взрослых (N 788н от 31 июля 2020 г.). Дополнительные разделы, бланки и приложения, предназначенные для внесения в истории болезни специфической для реабилитационного процесса информации, включают разделы осмотров врача физической и реабилитационной медицины и членов мультидисциплинарной реабилитационной команды; бланки оценки пациента по шкале реабилитационной маршрутизации; протоколы заседаний мультидисциплинарной реабилитационной команды с формулировкой целей и задач, а также индивидуальной программы медицинской реабилитации; формализованный выписной реабилитационный эпикриз; лист критериев качества специализированной медицинской помощи при проведении медицинской реабилитации взрослых пациентов с нарушением функции костно-мышечной системы / периферической нервной системы; русифицированные версии бланков соответствующих шкал и тестов.</p> <p>Унификация реабилитационных медицинских карт по профилю нарушений функции костно-мышечной системы / периферической нервной системы облегчает сотрудникам отделений и центров реабилитации оформление историй болезни в соответствии с действующим законодательством.</p></trans-abstract><kwd-group xml:lang="en"><kwd>medical rehabilitation</kwd><kwd>medical records</kwd><kwd>musculoskeletal system</kwd><kwd>peripheral nervous system</kwd><kwd>standardization</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>медицинская реабилитация</kwd><kwd>медицинская карта</kwd><kwd>периферическая нервная система</kwd><kwd>костно-мышечная система</kwd></kwd-group><funding-group><funding-statement xml:lang="en">The publication was published in the framework of the program “Priority 2030”.</funding-statement><funding-statement xml:lang="ru">Публикация выполнена в рамках программы «Приоритет 2030».</funding-statement></funding-group></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><citation-alternatives><mixed-citation xml:lang="en">Zasypkin MY, Ponomareva IV.
