Due to emergence of new rehabilitation protocols, it is extremely important to know the time of occurrence of consequences of spinal cord injury and adequate diagnosis of complications, which allows timely identification of life-threatening conditions of one of the body systems.</p> <p>Clinical case description. We present a clinical case of autonomic dysreflexia in neurogenic lower urinary tract dysfunction after traumatic cervical spinal cord injury in a 17-year-old girl. In traumatic spinal cord injury above the level of the sixth thoracic vertebra, autonomic dysreflexia as a variant of autonomic dysfunction is a common complication. This clinical case will be of interest to clinicians of all specialties who encounter patients with spinal cord injury. It is shown that misinterpreted symptoms of increased blood pressure during bladder filling were the cause of observation and treatment by a cardiologist for arterial hypertension, as well as hospitalization in the cardiac surgery department for planned surgical treatment of renal artery stenosis, which was thought to be the cause of the episodic increases in blood pressure. Lack of knowledge about the presence of autonomic dysfunction in spinal cord injury patients may have led to unnecessary surgical treatment. During the joint management of the patient with a pediatric cardiac surgeon, signs of autonomic dysreflexia were detected against the background of traumatic spinal cord disease at the level of the cervical spine, hypotensive treatment and planned surgical intervention were canceled.</p> <p>Conclusion. Subsequently, an adequate regimen of periodic bladder catheterization and antimuscarinic drug therapy was prescribed, which successfully reduced the dysreflexia.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Обоснование. </bold>В России среди всех случаев общего травматизма на травму спинного мозга приходится 0,7–6,8%. На фоне развития новых современных реабилитационных технологий крайне важными становятся как знания о времени возникновения последствий позвоночно-спинномозговой травмы, так и адекватная диагностика осложнений, позволяющая вовремя выявить угрожающие жизни состояния со стороны одной из систем организма.</p> <p>Описание клинического случая. Представлен клинический случай течения автономной дисрефлексии при нейрогенной дисфункции нижних мочевых путей на фоне позвоночно-спинномозговой травмы на уровне шейного отдела спинного мозга у девочки 17 лет. Автономная дисрефлексия как вариант вегетативной дисфункции является распространённым осложнением для позвоночно-спинномозговой травмы выше уровня VI грудного позвонка. Данный клинический случай будет интересен врачам всех специальностей, сталкивающихся с пациентами с последствиями повреждения спинного мозга. Показано, что неверно оценённые симптомы повышения артериального давления при переполнении мочевого пузыря стали причиной наблюдения и лечения у кардиолога от артериальной гипертензии, а также госпитализации в кардиохирургическое отделение для планового оперативного лечения стеноза почечной артерии, который, по мнению специалистов, являлся причиной эпизодических подъёмов артериального давления. Отсутствие знаний о наличии вегетативной дисфункции у пациентов с позвоночно-спинномозговой травмой могло стать причиной неоправданного оперативного лечения. При совместном с детским кардиохирургом ведении пациента были выявлены признаки автономной дисрефлексии на фоне позвоночно-спинномозговой травмы на уровне шейного отдела спинного мозга, в связи с чем отменены гипотензивное лечение и плановое оперативное вмешательство.</p> <p><bold>Заключение. </bold>Назначение адекватного режима периодической катетеризации мочевого пузыря и медикаментозная терапия холинолитическими препаратами позволили купировать проявления автономной дисрефлексии и снизить риски сосудистой катастрофы в долгосрочной перспективе.</p></trans-abstract><kwd-group xml:lang="en"><kwd>spinal cord injury</kwd><kwd>autonomic dysreflexia</kwd><kwd>neurogenic lower urinary tract dysfunction</kwd><kwd>intermittent catheterization</kwd><kwd>case report</kwd><kwd>arterial hypertension</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>позвоночно-спинномозговая травма</kwd><kwd>автономная дисрефлексия</kwd><kwd>нейрогенная дисфункция нижних мочевых путей</kwd><kwd>периодическая катетеризация</kwd><kwd>клинический случай</kwd><kwd>артериальная гипертензия</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><citation-alternatives><mixed-citation xml:lang="en">Morozov IN, Mlyavykh SG.
