Spasticity and pain negatively affect rehabilitation outcomes and complicate patient care. Surgery methods initially yield good results but are associated with numerous complications in the long term. Lumbosacral spinal cord epidural stimulation significantly reduces spasticity and pain in 61.8% of adults and may be a preferred method for managing spasticity and pain syndrome in children with spinal cord injury.</p> <p><bold>AIM</bold><italic>:</italic> To assess the long-term effectiveness of epidural spinal cord stimulation in the management of spasticity and pain syndrome in children with spinal cord injury.</p> <p><bold>MATERIALS AND METHODS</bold><italic>:</italic> The study included 15 children aged 14 to 17 years with spinal cord injury sequelae who were admitted to the Research Institute of Emergency Pediatric Surgery and Trauma since 2020. The assessment of the current condition was conducted through clinical assessment using the ASIA scale (to assess the impairment of spinal cord conduction), Ashworth and Tardieu scales (to assess muscle tone), MRS (to evaluate muscle strength), VAS (to measure pain syndrome), and the pendulum test. Additionally, an electromyographic assessment (F-wave) was performed to evaluate the severity of spasticity.</p> <p><bold>RESULTS</bold><italic>:</italic> An epidural stimulator was implanted in 15 patients. Positive outcomes were observed in 12 (80%) children, with a reduction in spasticity by an average of 1–2 points on the Ashworth and Tardieu scales. The pendulum test demonstrated improvement in 12 (80%) cases, showing an increase in oscillatory movements. Pain severity decreased in 12 (80%) patients, muscle strength improved in 10 (66.7%) patients, and electromyographic parameters (reduction in the number of pathological F-waves) improved in 9 (60%) patients. In 7 (46.7%) cases, the dose of antispastic medication was reduced. No clinical improvement was observed in 3 (20%) patients.</p> <p><bold>CONCLUSION</bold><italic>:</italic> Epidural stimulation is the treatment of choice for children with severe spastic syndrome due to spinal cord injury that is refractory to conservative therapy. Proper patient selection and objective assessment of their current condition increase the effectiveness of this method. In addition to improving pathological muscle tone, epidural stimulation also alleviates pain.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Обоснование.</bold> Спастический и болевой синдромы выявляются у 60–78% пациентов с последствиями позвоночно-спинномозговой травмы выше поясничного утолщения. Спастика и боль ухудшают реабилитационный прогноз и препятствуют осуществлению ухода за больными. Применяемые хирургические методы лечения первоначально дают хороший результат, но в отдалённом периоде имеют множество осложнений. Эпидуральная стимуляция спинного мозга на уровне поясничного утолщения значимо уменьшает спастический и болевой синдромы у 61,8% взрослых пациентов и может быть методом выбора коррекции спастического и болевого синдромов у детей с последствиями позвоночно-спинномозговой травмы.</p> <p><bold>Цель исследования</bold> ― оценить применение эпидуральной стимуляции спинного мозга в коррекции спастического и болевого синдромов у детей с последствиями позвоночно-спинномозговой травмы в динамике.</p> <p><bold>Материалы и методы. </bold>В исследовании приняли участие 15 детей с последствиями позвоночно-спинномозговой травмы в возрасте от 14 до 17 лет включительно, поступившие в НИИ НДХиТ с 2020 года. Диагностика актуального состояния проведена при помощи клинического осмотра с использованием шкал ASIA (определение степени нарушения проводимости спинного мозга), Эшфорта и Тардье (определение мышечного тонуса), MRS (определение мышечной силы), ВАШ (определение болевого синдрома), а также маятникового теста. Кроме того, проводилась электронейромиографическая оценка (F-волна) выраженности спастического синдрома.</p> <p><bold>Результаты. </bold>Эпидуральный стимулятор установлен 15 пациентам. У 12 (80%) детей наблюдалась положительная динамика в виде уменьшения спастичности в среднем на 1–2 балла по шкалам Эшфорта и Тардье, маятниковый тест показал положительную динамику в виде увеличения колебательных движений в 12 (80%) случаях, у 12 (80%) пациентов снизился болевой синдром, у 10(66,7%) ― увеличилась мышечная сила, у 9(60%) ― улучшились электронейромиографические показатели (уменьшение количества патологических F-волн), в 7 (46,7%) случаях была снижена доза антиспастического препарата. Положительной динамики не выявлено у 3 (20%) пациентов.</p> <p><bold>Заключение. </bold>Эпидуральная стимуляция у детей с выраженным спастическим синдромом вследствие позвоночно-спинномозговой травмы, резистентным к консервативному лечению, является методом выбора при лечении спастического синдрома. Правильный отбор пациентов, объективная оценка их актуального состояния увеличивают эффективность данной методики.
