Such approaches would facilitate the more effective implementation of medical rehabilitation measures. Studying of the dynamics of cardiovascular system dysfunction during ischemic stroke, determination of the potential for optimizing restorative treatment, and improvement of rehabilitation measures will enhance the clinical and social prognosis for patients, improve their quality of life, and reduce the incidence of complications and mortality.</p> <p><bold>AIM</bold><italic>:</italic> To evaluate the functional state of the cardiovascular system and its adaptive potential at the second stage of rehabilitation in patients after ischemic stroke.</p> <p><bold>MATERIALS AND METHODS</bold><italic>:</italic> Sixty-six patients aged 41 to 82 years underwent the second stage of rehabilitation after ischemic stroke, with an assessment of the functional state of the cardiovascular system. Holter electrocardiogram and blood pressure monitoring was performed on the first and tenth days of rehabilitation treatment. The analysis included blood pressure and heart rate. The changes in cardiovascular system function and adaptive potential were assessed using cardiovascular system function indices.</p> <p><bold>RESULTS</bold><italic>:</italic> An integral assessment of the functional state of the cardiovascular system demonstrated mobilization of the body’s adaptive capabilities during rehabilitation. A gradual decrease in systolic and diastolic blood pressure was noted, along with improvements in pulse pressure parameters, the functional change index, adaptive potential, Kerdo’s vegetative index, Robinson’s index, and endurance coefficient. A positive trend in cardiovascular system function was observed in 83.3% of cases (<italic>n</italic>=66). Women demonstrated better progress in circulatory system performance compared with men.</p> <p><bold>CONCLUSION</bold><italic>:</italic> During post-stroke rehabilitation, the use of integral indices to assess the functional state of the cardiovascular system is relevant and pathogenetically justified. A distinct positive trend in functional values and an increase in the adaptive potential of the cardiovascular system were observed, confirming the effectiveness of the rehabilitation measures taken. The proposed assessment methods may be useful for monitoring the effectiveness of rehabilitation in patients after ischemic stroke.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Обоснование.</bold> На сегодняшний день инсульты занимают лидирующую позицию среди показателей первичной инвалидизации, что обусловливает необходимость поиска новых подходов к оценке восстановительного лечения пациентов с острыми нарушениями мозгового кровообращения, которые позволят более эффективно проводить мероприятия по медицинской реабилитации. Изучение в динамике функциональных нарушений сердечно-сосудистой системы при ишемическом инсульте, определение возможности оптимизации восстановительного лечения и усовершенствование реабилитационных мероприятий позволят улучшить клинический и социальный прогноз пациентов, повысить качество их жизни и уменьшить количество осложнений и летальность.</p> <p><bold>Цель исследования</bold> ― оценить функциональное состояние сердечно-сосудистой системы и её адаптационный потенциал на втором этапе реабилитации у пациентов, перенёсших ишемический инсульт.</p> <p><bold>Материалы и методы.</bold> Для 66 пациентов в возрасте от 41 года до 82 лет после ишемического инсульта проведён второй этап реабилитации с исследованием функционального состояния сердечно-сосудистой системы. Осуществляли холтеровское мониторирование электрокардиограммы и артериального давления в первые и десятые сутки реабилитационного лечения. Проводили анализ параметров артериального давления и частоты сердечных сокращений. Динамику показателей сердечно-сосудистой системы, а также её адаптационный потенциал оценивали при помощи индексов функционирования сердечно-сосудистой системы.</p> <p><bold>Результаты.</bold> Интегральная оценка функционального состояния сердечно-сосудистой системы пациентов позволяет констатировать мобилизацию адаптационных возможностей организма при проведении реабилитационного лечения. Отмечено плавное снижение систолического и диастолического артериального давления, улучшение параметров пульсового давления, индекса функциональных изменений, адаптационного потенциала, вегетативного индекса Кердо, индекса Робинсона, коэффициента выносливости. Положительная тенденция функционирования сердечно-сосудистой системы отмечается в 83,33% наблюдений (<italic>n</italic>=66). У женщин наблюдается лучшая динамика по показателям работы системы кровообращения по сравнению с мужчинами.</p> <p><bold>Заключение.</bold> Использование индексов интегральной оценки функционального состояния сердечно-сосудистой системы актуально и патогенетически обосновано при реабилитационных мероприятиях после перенесённого ишемического инсульта. Отмечается отчётливая тенденция положительной динамики значений функционирования и повышения адаптационного потенциала сердечно-сосудистой системы, подтверждающая эффективность проводимых реабилитационных мероприятий.
