Cardiovascular toxicity remains a significant concern in women with breast cancer undergoing anticancer therapy. Conventional methods for assessing cardiotoxicity, primarily based on left ventricular ejection fraction, have limited prognostic value and may not fully reflect post-treatment functional decline and reduced quality of life.</p> <p><bold>AIM:</bold> This study aimed to evaluate the contribution of various physiological factors to reduced physical performance in patients with breast cancer receiving anthracycline-based chemotherapy.</p> <p><bold>METHODS:</bold> This cohort study included 30 patients with breast cancer undergoing neoadjuvant anthracycline-based chemotherapy. Comprehensive assessment comprised echocardiography, laboratory tests, psychometric questionnaires, and the six-minute walk test. Statistical analysis included correlation and regression analyses to identify predictors of reduced physical performance.</p> <p><bold>RESULTS:</bold> Most patients were overweight or obese and had concomitant anemia and hypertension. Patients with low daily physical activity more frequently exhibited a 10% decrease in left ventricular ejection fraction over the past 3 months and poorer performance on the six-minute walk test. Multiple regression analysis identified anemia severity, hypertension level, reduced left ventricular ejection fraction, and the number of chemotherapy and radiotherapy cycles as significant predictors of decreased physical performance. Psychoemotional disorders and treatment adherence did not have a statistically significant impact.</p> <p><bold>CONCLUSION:</bold> Reduced physical performance in patients receiving anthracycline-based chemotherapy for breast cancer is driven by a combination of cardiovascular, pulmonary, and hematologic factors. The study was limited by a small sample size. Future research should focus on the development of personalized rehabilitation programs aimed at minimizing cardiotoxic effects and restoring functional capacity in patients with cancer.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Обоснование.</bold> Современные успехи в онкологии способствуют увеличению продолжительности жизни пациентов. Актуальной проблемой у женщин, страдающих раком молочной железы, остаётся кардиотоксичность противоопухолевой терапии. Традиционные методы оценки кардиотоксичности, основанные на показателях фракции выброса левого желудочка, обладают ограниченной прогностической ценностью и не всегда отражают снижение функционирования и жизнедеятельности после лечения.</p> <p><bold>Цель исследования</bold> ― оценить вклад различных физиологических факторов в снижение физической активности при раке молочной железы у пациенток, получающих антрациклиновую химиотерапию.</p> <p><bold>Методы.</bold> В когортное исследование включены 30 пациенток, страдающих раком молочной железы, проходивших неоадъювантную антрациклиновую химиотерапию. Комплексная оценка проводилась с использованием эхокардиографии, лабораторных исследований, психометрических опросников и теста шестиминутной ходьбы. Статистический анализ включал корреляционный и регрессионный анализ для выявления предикторов снижения физической работоспособности.</p> <p><bold>Результаты.</bold> Большинство пациенток имели избыточную массу тела или ожирение, а также сопутствующую анемию и артериальную гипертензию. Пациентки с низкой повседневной активностью чаще отмечали снижение фракции выброса левого желудочка на 10% за последние три месяца, а также худшие показатели по результатам теста шестиминутной ходьбы. Множественный регрессионный анализ выявил, что степень тяжести анемии, уровень артериальной гипертензии, низкая фракция выброса левого желудочка и количество курсов лучевой и химиотерапии являются значимыми предикторами снижения физической работоспособности. Психоэмоциональные расстройства и приверженность к лечению статистически значимого влияния не оказали.</p> <p><bold>Заключение.</bold> Снижение физической работоспособности у пациенток с раком молочной железы, получающих антрациклины, обусловлено комплексным воздействием сердечно-сосудистых, лёгочных и гематологических факторов. Ограничением исследования было небольшое количество выборки.
