The development and clinical presentation of HCM do not always conform to the traditional view of its monogenic inheritance pattern. One key to addressing this issue may lie in identifying epigenetic mechanisms regulating gene expression, particularly DNA methylation, involved in the pathogenesis of the disease and modifying its course. Utilizing previously obtained whole-genome data, we identified four extended genomic regions with reduced methylation levels in the myocardium of patients with HCM. These include region chr2:113993204-113994075 located within the transcribed area of the <italic>PAX8</italic> gene, as well as three other regions – chr6:31148369-31148577, chr8:11565217-11567212, and chr8:22132791-22133357 – that are associated with promoters of genes <italic>PSORS1C3</italic>, <italic>GATA4</italic>, and <italic>PIWIL2</italic>, respectively. We were able to demonstrate altered expression of <italic>PAX8</italic> and <italic>GATA4</italic> genes containing one each of these four mentioned regions. Our findings will expand the currently very limited understanding of the unique features of epigenetic regulation in this condition.</p></abstract><trans-abstract xml:lang="ru"><p>Гипертрофическая кардиомиопатия (ГКМП) – самое распространенное наследственное заболевание сердца с частотой встречаемости 1:500 – 1:200 человек. Развитие и клиническая картина ГКМП далеко не всегда укладываются в традиционное представление о моногенном характере ее наследования. Одним из ключей к решению этой проблемы может стать идентификация эпигенетических механизмов регуляции экспрессии генов, в первую очередь метилирования ДНК, вовлеченных в патогенез заболевания и модифицирующих характер его течения. Используя полученные нами ранее полнотеномные данные, мы выявили четыре протяженных региона генома, уровни метилирования которых снижены в миокарде пациентов с ГКМП. Это регион chr2:113993204-113994075, располагающийся в транслируемой области гена <italic>PAX8</italic>, и три региона (chr6:31148369-31148577, chr8:11565217-11567212 и chr8:22132791-22133357), ассоциированные с промоторами генов <italic>PSORS1C3</italic>, <italic>GATA4</italic> и <italic>PIWIL2</italic> соответственно. Мы наблюдали изменение экспрессии генов <italic>PAX8</italic> и <italic>GATA4</italic>, содержащих по одному из четырех упомянутых регионов.
