Abstract

Chronic non-communicable diseases remain the leading cause of premature mortality in the Russian federation, accounting for more than 67% of deaths among the adult population. Dispensary observation – the primary instrument for the active outpatient management of such patients – consistently demonstrates substantial deficiencies in real-world practice: failure to meet registration deadlines, incomplete fulfilment of diagnostic standards, inadequate documentation, and low patient adherence to prescribed treatment. At the same time, the role of information provision as an important determinant of dispensary observation quality has not yet received comprehensive consideration in the Russian context.

This paper presents a review of Russian and international publications addressing information provision in the medical care of patients under dispensary observation. The analysis enabled systematic synthesis of data on the organizational foundations and characteristic deficiencies of dispensary observation in Russia, and a description of the range of information technologies currently in use – from regional medical information systems to telemedicine platforms for remote monitoring and machine learning algorithms. The principal barriers to their implementation and the most promising directions for further development were identified. A substantial proportion of dispensary observation deficiencies are informational in nature and are potentially addressable through automation. Regional experience in Russia and international evidence alike indicate that the systematic application of information technologies can simultaneously increase dispensary observation coverage, reduce emergency hospitalization rates, and improve the alignment of prescribed therapy with current clinical guidelines.
