Abstract

Objective: to conduct a comparative analysis of the application of the standard features of high field magnetic resonance imaging and SWI (Susceptibility-weighted imaging) protocol in patients with different types of traumatic brain injury. Materials and methods. In the study 48 patients with traumatic brain injury were examined. 21 of them had a concussion, 22 - mild, 15 - brain contusion of moderate severity, and 5 - diffuse axonal injury. Examination record consisted of clinical assessment of patients, MRI examination on magnetic resonance imaging Magnetom Trio A Tim (Siemens, Germany) with the induction of the magnetic field of 3.0 Tesla, with access to standard T1-WI and T2-WI in the coronary, axial and sagittal planes. In addition, all patients underwent a protocol SWI (Susceptibility Weighted Imaging). Results of the study. It has been shown high diagnostic value of magnetic resonance imaging technique SWI in the diagnosis of acute traumatic brain injury. On SWI images the extent of the damage and their number was more seen than on T1 and T2 images. In some cases of diffuse axonal injury and cerebral concussion SWI were identified areas of microhemorrhage that are not diagnosed on the standard MRI protocols. Conclusion. New features of neuroimaging tools provide more accurate diagnosis. Also the morphological explanation the phenomenon of mild cognitive impairment is received, affective disorders and asthenic syndrome in some cases of consequences of mild traumatic brain injury. In some cases new records of neuroimaging can change the perception of the severity of the brain injury and, consequently, change the tactics of treatment and rehabilitation (2 figs, bibliography: 14 refs).
