According to epidemiological data, the most common causes of cognitive dysfunction are Alzheimer’s disease and cerebrovascular diseases, accounting for about 90% of all cases.</p> <p><bold>AIM</bold>: to investigate the morphological features underlying the development of cognitive disorders in Alzheimer’s disease and cerebrovascular pathology.</p> <p><bold>MATERIALS AND METHODS</bold>: According to our developed and patented method of preparation of sectional material for electron microscopy а detailed study of the brain of elderly patients with Alzheimer’s disease and cerebrovascular pathology was carried out using electron microscopy on autopsy material.</p> <p><bold>RESULTS</bold>: Electron microscopy of the brains of patients with Alzheimer’s disease often reveals large extracellular reticulate-fiber structures closely adjacent to myelin fibers, which are part of a fibrous plaque. In vascular dementia, irregularly shaped hyperchromic masses with heterogeneous contents are found in the neuropil, which may represent future so-called “diffuse” plaques. In Alzheimer’s disease, fragments of future “diffuse” plaques are also observed in the cytoplasm of some cortical oligodendrocytes. In all deceased with Alzheimer’s disease, synaptic changes were found in the form of roughly granular disintegration of the entire synaptocomplex, i.e., synaptic vesicles, mitochondria, and the synaptic cleft proper.</p> <p><bold>CONCLUSION</bold>: This study provides an opportunity to take a new look at some questions of etiology and pathogenesis of such a disease as cognitive disorders, which is not easy to diagnose and treat.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Актуальность</bold>. В настоящее время в связи с увеличением продолжительности жизни чрезвычайно актуальной является проблема когнитивных нарушений, довольно часто обнаруживаемых у пожилых людей и являющихся грозным синдромом целого ряда нейродегенеративных заболеваний. Согласно эпидемиологическим данным, самыми частыми причинами развития когнитивной дисфункции являются болезнь Альцгеймера и цереброваскулярные заболевания, в сумме составляя около 90 % всех случаев.</p> <p><bold>Цель исследования</bold>: изучение морфологических особенностей, лежащих в основе развития когнитивных нарушений при болезни Альцгеймера и цереброваскулярной патологии.</p> <p><bold>Материалы и методы</bold>. По разработанному и запатентованному нами методу подготовки секционного материала для электронной микроскопии на аутопсийном материале выполнено детальное исследование головного мозга пожилых пациентов с болезнью Альцгеймера и цереброваскулярной патологией.</p> <p><bold>Результаты</bold>. При электронной микроскопии головного мозга пациентов с болезнью Альцгеймера часто обнаруживаются крупные внеклеточные сетчато-волокнистые структуры, вплотную прилежавшие к миелиновым волокнам, являющиеся частью волокнистой бляшки. При сосудистой деменции в нейропиле встречаются гиперхромные образования неправильной формы с гетерогенным содержимым, которые могут представлять собой будущие так называемые «диффузные» бляшки. При болезни Альцгеймера в цитоплазме некоторых олигодендроцитов коры головного мозга также наблюдаются фрагменты будущих «диффузных» бляшек. У всех умерших с болезнью Альцгеймера обнаруживались изменения синапсов в виде грубогранулярного распада всего синаптокомплекса, т. е. синаптических везикул, митохондрий и собственно синаптической щели.</p> <p><bold>Заключение</bold>. Проведенное исследование дает возможность по-новому взглянуть на некоторые вопросы этиологии и патогенеза такого непростого для диагностики и лечения состояния, как когнитивные нарушения.</p></trans-abstract><trans-abstract xml:lang="zh"><p><bold>背景</bold>。随着人类寿命的延长，认知障碍问题变得日益紧迫，这种问题在老年人中较为常见，且是多种神经退行性疾病的严重综合征。根据流行病学数据，阿尔茨海默病和脑血管疾病是导致认知功能障碍的最常见原因，二者合计约占所有病例的90%。</p> <p><bold>研究目的</bold>。探讨阿尔茨海默病和脑血管病理中导致认知障碍的形态学特征。</p> <p><bold>材料和方法</bold>。本研究采用我们开发并获得专利的电子显微镜切片材料制备方法，对阿尔茨海默病和脑血管病理的老年患者的尸检脑组织进行了详细研究。</p> <p><bold>结果</bold>。电子显微镜显示，阿尔茨海默病患者的脑部常见大块网状纤维结构，紧贴髓鞘纤维，构成纤维斑块的一部分。在血管性痴呆患者的神经胶质中观察到形状不规则、内容异质的高染色物，可能是未来“弥散性”斑块的前兆。在阿尔茨海默病患者的部分脑皮层少突胶质细胞的细胞质中也可见未来 “弥散性”斑块的碎片。所有阿尔茨海默病去世患者的突触均出现了粗颗粒解体现象，包括突触复合体（即突触小泡、线粒体和突触间隙）的解体。</p> <p><bold>结论</bold>。本研究为重新认识这种难以诊断和治疗的认知障碍状态的某些病因和发病机制提供了新 的视角。</p></trans-abstract><kwd-group xml:lang="en"><kwd>Alzheimer’s disease</kwd><kwd>brain</kwd><kwd>destruction of synapses</kwd><kwd>diffuse plaques</kwd><kwd>electron microscopy</kwd><kwd>fibrous plaques</kwd><kwd>vascular dementia</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>болезнь Альцгеймера</kwd><kwd>волокнистые бляшки</kwd><kwd>головной мозг</kwd><kwd>диффузные бляшки</kwd><kwd>деструкция синапсов</kwd><kwd>сосудистая деменция</kwd><kwd>электронная микроскопия</kwd></kwd-group><kwd-group xml:lang="zh"><kwd>阿尔茨海默病</kwd><kwd>纤维斑块</kwd><kwd>大脑</kwd><kwd>弥散性斑块</kwd><kwd>突触破坏</kwd><kwd>血管性痴呆</kwd><kwd>电子显微镜</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><citation-alternatives><mixed-citation xml:lang="en">Emelin AYu, Lobzin VYu, Vorobyov SV.
