Dispensary observation – the primary instrument for the active outpatient management of such patients – consistently demonstrates substantial deficiencies in real-world practice: failure to meet registration deadlines, incomplete fulfilment of diagnostic standards, inadequate documentation, and low patient adherence to prescribed treatment. At the same time, the role of information provision as an important determinant of dispensary observation quality has not yet received comprehensive consideration in the Russian context.</title> <title>This paper presents a review of Russian and international publications addressing information provision in the medical care of patients under dispensary observation. The analysis enabled systematic synthesis of data on the organizational foundations and characteristic deficiencies of dispensary observation in Russia, and a description of the range of information technologies currently in use – from regional medical information systems to telemedicine platforms for remote monitoring and machine learning algorithms. The principal barriers to their implementation and the most promising directions for further development were identified. A substantial proportion of dispensary observation deficiencies are informational in nature and are potentially addressable through automation. Regional experience in Russia and international evidence alike indicate that the systematic application of information technologies can simultaneously increase dispensary observation coverage, reduce emergency hospitalization rates, and improve the alignment of prescribed therapy with current clinical guidelines.</title></abstract><trans-abstract xml:lang="ru"><p>Хронические неинфекционные заболевания остаются ведущей причиной преждевременной смертности в Российской Федерации, где на их долю приходится свыше 67% смертей взрослого населения. Диспансерное наблюдение, представляя собой основной инструмент активного ведения таких пациентов в амбулаторных условиях, в реальной практике систематически демонстрирует значительные дефекты: несоблюдение сроков постановки на учёт, неполноту выполнения диагностических стандартов, ненадлежащее документирование и низкую приверженность пациентов лечению. При этом роль информационного обеспечения как важного фактора качества диспансерного наблюдения до сих пор не получила целостного осмысления применительно к российским условиям.</p> <p>В настоящей работе представлен обзор отечественных и зарубежных публикаций, посвящённых информационному обеспечению при оказании медицинской помощи пациентам, находящимся под диспансерным наблюдением. Проведённый анализ позволил систематизировать данные об организационных основах и характерных дефектах диспансерного наблюдения в России, описать спектр применяемых информационных технологий от региональных медицинских информационных систем до телемедицинских платформ дистанционного мониторинга и алгоритмов машинного обучения. Были выявлены основные барьеры их внедрения и перспективные направления развития. Значительная часть дефектов диспансерного наблюдения имеет информационную природу и потенциально устранима средствами автоматизации.
