THE USE OF MONOCLONAL ANTIBODY PREPARATIONS IN THE TREATMENT OF RECURRENT POLYPOUS RHINOSINUSITIS

Abstract

The main methods of therapy for patients with recurrent polypous rhinosinusitis are surgical and conservative, despite the successes achieved in the treatment of the polypous process, modern clinical studies show that there is no complete control over this disease. Taking into account the pros and cons of the above-mentioned treatment methods, it seems relevant for the scientific community to further search for new methods that combine high efficiency and low probability of developing undesirable systemic manifestations. Targeted biological therapy using monoclonal antibodies can be considered a modern method of treating patients suffering from recurrent polypous rhinosinusitis.

The aim of the study was to evaluate the effectiveness of monoclonal antibody preparations in the treatment of patients with recurrent polypous rhinosinusitis.

Materials and methods. The authors studied the outpatient records of 132 patients aged 20 to 87 years, in whose therapy drugs containing monoclonal antibodies (dupilumab, omalizumab, mepolizumab) were used. The main criterion for inclusion in this study was the presence of somatic pathology in patients – recurrent polypous rhinosinusitis and the underlying disease — bronchial asthma. The subjects were divided into 3 groups, in the treatment of which one of three monoclonal antibody preparations was used.

The results of the study. In each of the 3 groups, against the background of monoclonal antibody therapy, it was possible to observe a regression of the main symptoms of the polyposis process (loss of sense of smell and taste, sensations of pain / pressure in the face, a decrease in runny nose, mucus runoff along the back wall of the pharynx, nasal congestion). The effectiveness of the therapy was assessed based on the results of the endoscopic picture (Lund-Kennedy scale), CT picture (Lund-Mackay scale) and data from the subjective questionnaire SNOT-22.

Conclusion. All the studied molecules have shown their effectiveness in the treatment of CKD to varying degrees. However, the most preferred molecule, the use of which makes it possible to achieve a more pronounced regression of the main clinical manifestations, reduce the frequency of recurrence of the polyposis process with no need for repeated surgical interventions, and also allow for a significant improvement in the quality of life of patients, was dupilumab
