Despite its relative frequency, timely detection in young patients is difficult. This is due to the lack of a unified approach to diagnosis. This implies the need to generalize clinical and statistical data and introduce advanced, minimally invasive and accurate diagnostic methods into clinical practice.</p> <p><bold>The aim of the study.</bold> Development of an optimized strategy for the detection of paratonsillar abscesses in children, based on the analysis of retrospective clinical data and principles of evidence-based medicine.</p> <p><bold>Materials and methods.</bold> A retrospective analysis of the medical records of 55 children with a diagnosis of paratonsillar abscess was performed. The average age of the patients was 11 years. The study included patients with confirmed paratonsillar abscess, for whom there was complete information about the treatment and its results. Descriptive statistics methods were used to analyze the collected clinical data. At the same time, a systematic review of relevant scientific literature was carried out, covering publications by Russian and foreign authors for the period from 2018 to 2024.</p> <p><bold>Results and discussion.</bold> The polymorphic nature of the microflora with a predominance of Streptococcus pyogenes and anaerobic microorganisms was revealed. An integrated approach, including ultrasound diagnostics, timely drainage and empirical antibiotic therapy, ensures recovery in 92.7% of cases. The importance of a multidisciplinary approach in the management of pediatric patients with PTA is emphasized.</p> <p><bold>Conclusion.</bold> Patients with peritonsillar abscess have a typical clinical and anamnestic profile: school age, association with episodes of acute respiratory viral infections and chronic tonsillitis, winter seasonality. Ultrasound examination has been confirmed as an indispensable method for objective verification of the stage of abscess formation, differential diagnosis of paratonsillitis and abscess, as well as for safe and effective drainage under visual control. Based on the results of the study, a diagnostic algorithm has been developed that provides for stratification of patients according to the severity and volume of the abscess with the determination of indications for conservative therapy, ultrasound-controlled drainage or abscess tonsillectomy<bold>. </bold></p></abstract><trans-abstract xml:lang="ru"><p><bold>Введение. </bold>Перитонзиллярный абсцесс – распространенное гнойное заболевание в области головы и шеи у детей. Несмотря на его относительную частоту, своевременное выявление у маленьких пациентов затруднено. Это связано с отсутствием унифицированного подхода к диагностике. Из этого следует необходимость в обобщении клинико-статистических данных и внедрении в клиническую практику передовых, малоинвазивных и точных методов диагностики.</p> <p><bold>Цель исследования. </bold>Разработка оптимизированной стратегии выявления паратонзиллярных абсцессов у детей, опираясь на анализ ретроспективных клинических данных и принципов доказательной медицины.</p> <p><bold>Материалы и методы.</bold> Проведен ретроспективный анализ медицинских карт 55 детей с установленным диагнозом «паратонзиллярный абсцесс». Средний возраст пациентов составил 11 лет. В исследование были включены пациенты с подтвержденным паратонзиллярным абсцессом, по которым имелась полная информация о проведенном лечении и его результатах. Для анализа собранных клинических данных применялись методы описательной статистики. Параллельно был осуществлен систематический обзор актуальной научной литературы, охватывающий публикации российских и зарубежных авторов за период с 2018 по 2024 год.</p> <p><bold>Результаты и обсуждение. </bold>Выявлен полиморфный характер микрофлоры с преобладанием Streptococcus pyogenes и анаэробных микроорганизмов. Комплексный подход, включающий УЗИ-диагностику, своевременное дренирование и эмпирическую антибиотикотерапию, обеспечивает выздоровление в 92.7% случаев. Подчеркивается важность мультидисциплинарного подхода в ведении педиатрических пациентов с ПТА</p> <p><bold>Заключение. </bold>У пациентов с перитонзиллярным абсцессом установлен типичный клинико-анамнестический профиль: школьный возраст, связь с эпизодами ОРВИ и хроническим тонзиллитом, зимняя сезонность. Ультразвуковое исследование подтверждено как незаменимый метод для объективной верификации стадии абсцедирования, дифференциальной диагностики паратонзиллита и абсцесса, а также для безопасного и эффективного дренирования под визуальным контролем.
