According to the national program "Chronic tonsillitis", the proportion of young patients aged from preschool to primary school increased from 41% to 65%. In addition, the incidence of paratonsillar abscesses has increased almost four times compared with the figures recorded before the pandemic.</p> <p><bold>The aim of the study.</bold> To determine the role of COVID-19 and other clinical and laboratory factors in the formation and deterioration of the course of chronic pathology of the lymphoepithelial pharyngeal ring.</p> <p><bold>Materials and methods. </bold>In this prospective cohort study, 135 patients (&lt;18 years) admitted with acute respiratory viral infection (ARVI) to St Petersburg Children’s City Hospital No. 5 (April 2021 – October 2022) were evaluated at baseline and again six months later. Assessments comprised symptom and history taking, pediatric and ENT examinations, video-endoscopy of the nasal cavity and nasopharynx, complete blood counts, and PCR testing of nasopharyngeal swabs for SARS-CoV-2 and a respiratory virus panel. Statistical analysis (R 4.4.2) used the Shapiro–Wilk test, Pearson’s χ² or Fisher’s exact test, and the Mann–Whitney U test; p&lt;0.05 was deemed significant.</p> <p><bold>Results and discussion. </bold>CAP was present at baseline in 68 children (50.4%). Over six months, CAP developed in 20 of the remaining 67 children (29.9%). Key qualitative predictors were prior COVID-19 (85% vs 34.8%; p&lt;0.001) and co-infection with two or more respiratory viruses (25% vs 2.2%; p=0.008). Quantitative predictors included higher platelet counts (median 389.5 × 10⁹/L vs 295.5 × 10⁹/L; p=0.01), elevated ESR (median 23 mm/h vs 13 mm/h; p&lt;0.001) and more frequent ARVI episodes in the preceding six months (median 2 vs 1; p=0.012). Surgery was required in 18 of the 68 CAP cases (26.5%); risk factors were a positive SARS-CoV-2 PCR (88.9% vs 38%; p&lt;0.001) and age &lt;6 years, whereas children who reported throat pain were less likely to require surgery (5.6% vs 42%; p=0.002).</p> <p><bold>Conclusions. </bold>COVID-19 markedly increases the risk of developing or worsening CAP within six months of an ARVI episode. Elevated platelet and ESR values, and frequent respiratory infections identify children at higher risk, while absence of throat pain in younger patients signals a greater probability of requiring surgery.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Введение.</bold> За последние пять лет в России наблюдается существенный подъем числа случаев хронического аденотонзиллита среди детей, что совпадает с периодом развития пандемии COVID-19.</p> <p><bold>Цель исследования</bold>. Определить роль влияния COVID-19 и других клинико-лабораторных факторов на формирование и ухудшение течения хронической патологии лимфоэпителиального глоточного кольца.</p> <p><bold>Материалы и методы.</bold> В проспективное когортное исследование включено 135 пациентов &lt;18 лет, госпитализированных с ОРВИ в СПб ГБУЗ «ДГКБ № 5 им. Н.Ф. Филатова» с апреля 2021 по октябрь 2022 г.; контрольное обследование через 6 мес. Были проведены сбор жалоб и анамнеза, педиатрический и оториноларингологический осмотр, видеоэндоскопия полости носа и носоглотки, общий анализ крови, ПЦР мазков на SARS-CoV-2 и панель респираторных вирусов.</p> <p><bold>Результаты и обсуждение.</bold> При первичном осмотре аденотонзиллярная патология выявлена у 68 детей, а через 6 мес. хроническая патология сформировалась еще у 20 пациентов без исходной патологии. Качественные предикторы развития хронической аденотонзиллярной патологии: перенесенная COVID-19-инфекция и сочетанная вирусная инфекция. Значимыми количественными факторами оказались, повышенная скорость оседания эритроцитов и частые ОРВИ. Оперативное лечение потребовалось 18 из 68 детей с ХАП; факторами риска были положительная ПЦР на SARS-CoV-2 и возраст &lt;6 лет, тогда как боль в горле имела защитное значение.</p> <p><bold>Заключение.</bold> Перенесенный COVID-19 значительно повышает риск возникновения и прогрессирования хронической аденотонзиллярной патологии в течение последующих 6 месяцев. </p></trans-abstract><kwd-group xml:lang="en"><kwd>adenotonsillar pathology</kwd><kwd>children</kwd><kwd>COVID-19</kwd><kwd>risk factors</kwd><kwd>SARS-CoV-2</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>хроническая аденотонзиллярная патология</kwd><kwd>COVID-19</kwd><kwd>дети</kwd><kwd>риск-факторы</kwd><kwd>SARS-CoV-2</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><citation-alternatives><mixed-citation xml:lang="en">1.
