However, in all patients, there was a recurrence of the exudative activity of the process. Aflibercept treatment method consisted of 3 monthly “loading” intravitreal injections with follow-up period of 4 months.</p> <p><bold>RESULTS.</bold> One month after 1st aflibercept injection among all patients, the average index of best-corrected visual acuity (BCVA) increased to 0.41 ± 0.02 and the central retinal thickness (CRT) index decreased to 307 ± 14.5 μm versus the initial CRT value 431 ± 64 μm. After the 3rd aflibercept injection, the CRT index was the lowest and amounted to 189.5 ± 13.0 μm, which was accompanied BCVA increase to 0.42 ± 0.03 versus 0.29 ± 0.05 as the initial value. According to the optical coherence tomography data, after “loading” phase, good anatomical effect was observed with significant edema reduction, complete resorption of fluid in the subretinal space, and decrease of the pigment epithelial detachment’s height.</p> <p><bold>CONCLUSION.</bold> Evaluating the results of our study, we found that the use of angiogenesis inhibitor aflibercept made it possible to suppress the signs of activity of choroidal neovascularization and to obtain additional improvement of visual functions in patients with neovascular age-related macular degeneration, when the therapeutic effect from the early ranibizumab therapy was insufficient or completely absent.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Цель</bold> — оценить клиническую эффективность применения ингибитора афлиберцепта у пациентов с резистентной к ранибизумабу неоваскулярной формой возрастной макулярной дегенерации.</p> <p><bold>Материалы и методы.</bold> Группа исследования состояла из 13 пациентов (13 глаз), которым в течение года проводили терапию ранибизумабом (от 6 до 8 инъекций с интервалом от 1,5 до 2 мес.). Однако у всех пациентов отмечался рецидив экссудативной активности процесса. Методика лечения афлиберцептом состояла из 3 ежемесячных загрузочных интравитреальных инъекций с периодом наблюдения 4 мес.</p> <p><bold>Результаты.</bold> Через 1 мес. после первого введения афлиберцепта средний показатель максимальной корригированной остроты зрения (МКОЗ) в целом по группе повысился до 0,41 ± 0,02, а показатель центральной толщины сетчатки (ЦТС) уменьшился до 307 ± 14,5 мкм, при исходной ЦТС 431 ± 64 мкм. После третьей инъекции афлиберцепта ЦТС была наименьшей — 189,5 ± 13,0 мкм, при этом МКОЗ повысилась до 0,42 ± 0,03 относительно 0,29 ± 0,05 исходно. При оценке данных оптической когерентной томографии, после загрузочной фазы наблюдался хороший анатомический эффект с существенным уменьшением отёка, полным рассасыванием жидкости в субретинальном пространстве и уменьшением высоты отслойки пигментного эпителия.</p> <p><bold>Заключение.</bold> При оценке результатов проведённого нами исследования выявлено, что применение ингибитора ангиогенеза афлиберцепта позволило подавить признаки активности хориоидальной неоваскуляризации и получить дополнительное улучшение зрительных функций у пациентов с неоваскулярной формой возрастной макулярной дегенерации при недостаточном эффекте или же полном его отсутствии от раннего применения ранибизумаба.</p></trans-abstract><trans-abstract xml:lang="zh"><p/></trans-abstract><kwd-group xml:lang="en"><kwd>neovascular age-related macular degeneration</kwd><kwd>aflibercept</kwd><kwd>ranibizumab</kwd><kwd>tachyphylaxis</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>неоваскулярная возрастная макулярная дегенерация</kwd><kwd>афлиберцепт</kwd><kwd>ранибизумаб</kwd><kwd>тахифилаксия</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><citation-alternatives><mixed-citation xml:lang="en">Libman ES, Kaleeva EV, Ryazanov DP.
