The technique of IOL posterior capture was originally developed by Gimbel and DeBrooff for congenital cataract surgery and involved capture of an intraocular lens optic through a posterior capsulorhexis opening with the haptics remaining in the capsular bag. The technique of IOL posterior capture is universal and successfully eliminates the risk of a wide range of intra- and postoperative complications. Clinical examples fully describe how the technique was used for failed anterior capsulorhexis, intraoperative break of the IOL haptics, rotational instability of a toric IOL, and secondary implantation in traumatic cataract and the empty vitreous cavity with silicone oil tamponade. In each case, posterior capture of the optic provided stable IOL centration and a capsular barrier preventing migration of lens epithelial cells into the optic zone, which is critical for preventing secondary cataract. The authors highlight that the technique preserves the anterior hyaloid membrane, minimizing the risks of vitreoretinal complications. Published analyses of clinical results of primary posterior capsulorhexis and posterior capture of the IOL optic have demonstrated their effectiveness and safety. The data presented in the article show that the technique of posterior capture of the IOL optic can effectively eliminate the risks of both haptic break and rotational instability of toric IOLs and provide predictable results compared with traditional approaches — lens exchange or ring implantation. The authors demonstrated that the technique may be used with various IOL models, including toric and three-piece lenses, provided that the haptics are open-loop. Therefore, posterior capture of the IOL optic is a universal cataract surgery technique, which successfully prevents several intraoperative complications and provides correct IOL position and transparency of the optical axis, thus ensuring stable functional results in the long-term postoperative period.</p></abstract><trans-abstract xml:lang="ru"><p>Разработанная H. Gimbel и B. DeBrooff для хирургии врождённой катаракты техника заднего «захвата» оптики интраокулярной линзы (ИОЛ) изначально включала выполнение первичного заднего капсулорексиса, в котором фиксируется оптическая часть линзы, а гаптические элементы остаются в сводах капсульного мешка. Технология «захвата» оптики ИОЛ демонстрирует универсальность для успешного разрешения широкого спектра интра- и послеоперационных осложнений. В работе оценивается клиническая эффективность и отдалённые послеоперационные результаты технологии заднего «захвата» оптики ИОЛ при нестандартных ситуациях хирургии катаракты в клинических случаях. На примерах подробно описано применение методики при несостоятельности переднего капсулорексиса и ротационной нестабильности торической ИОЛ, интраоперационном отрыве гаптики ИОЛ, вторичной имплантации при травматической катаракте и в условиях авитрии с силиконовой тампонадой. В каждом случае задний «захват» оптики позволял достичь стабильной центрации ИОЛ и формировал капсульный барьер, предотвращающий миграцию клеток хрусталикового эпителия в оптическую зону, что является ключевым механизмом профилактики вторичной катаракты. Особо подчеркивается, что методика обеспечивает сохранность передней гиалоидной мембраны, минимизируя риски витреоретинальных осложнений. Существующие в литературе работы, посвящённые анализу клинических результатов выполнения первичного заднего капсулорексиса и заднего «захвата» оптики ИОЛ, показали эффективность и безопасность. Приведённые в статье данные показывают, что методика заднего «захвата» оптической части ИОЛ позволяет эффективно решить проблемы как отрыва гаптики, так и ротационной нестабильности торической ИОЛ, обеспечивая прогнозируемый результат в сравнении с традиционными подходами — заменой линзы или имплантацией колец. Показана применимость техники с различными моделями ИОЛ, включая торические и трёхчастные линзы, при обязательном условии наличия незамкнутой гаптики.
