The novelty of this review lies in the comprehensive examination of the pathogenesis of DED and the substantiation of a promising treatment direction using corneal extracellular matrix biomimetics. The analysis showed that modern therapy of DED is not limited to simple lubrication and should be aimed at restoring ocular surface homeostasis and breaking the vicious cycle of chronic inflammation. Furthermore, it was found that multi-component preparations based on extracellular matrix biomimetics, in particular SFERO® Oko (BIOMIR Service JSC, Russia), demonstrate high efficacy in stimulating corneal regeneration and are promising for inclusion in therapeutic regimens for dry eye disease. Special attention is paid to drugs approved by the U.S. Food and Drug Administration, including cyclosporine; Xiidra® (lifitegrast 5%, Bausch &amp; Lomb Inc., USA)<sup><ext-link ext-link-type="uri" xlink:href="#_ftn1">[</ext-link><ext-link ext-link-type="uri" xlink:href="#_ftnref1">℘</ext-link><ext-link ext-link-type="uri" xlink:href="#_ftn1">]</ext-link></sup>; Eysuvis® (loteprednol etabonate 0.25%, Kala Pharmaceuticals, USA)<sup><ext-link ext-link-type="uri" xlink:href="#_ftn1">[</ext-link><ext-link ext-link-type="uri" xlink:href="#_ftnref1">℘</ext-link><ext-link ext-link-type="uri" xlink:href="#_ftn1">]</ext-link></sup>; Miebo® (perfluorohexyloctane, Novaliq, Germany)<sup><ext-link ext-link-type="uri" xlink:href="#_ftn1">[</ext-link><ext-link ext-link-type="uri" xlink:href="#_ftnref1">℘</ext-link><ext-link ext-link-type="uri" xlink:href="#_ftn1">]</ext-link></sup>; and Tyrvaya® (varenicline nasal spray, Oyster Point Pharma Inc., USA)<sup><ext-link ext-link-type="uri" xlink:href="#_ftn1">[</ext-link><ext-link ext-link-type="uri" xlink:href="#_ftnref1">℘</ext-link><ext-link ext-link-type="uri" xlink:href="#_ftn1">]</ext-link></sup>. This scientific review is based on an analysis of Russian and international sources from 2015 to 2025 and is intended for practicing ophthalmologists seeking a personalized approach to dry eye disease treatment.</p> <p><ext-link ext-link-type="uri" xlink:href="#_ftnref1">[℘]</ext-link> A drug is not registered in the Russian Federation.</p></abstract><trans-abstract xml:lang="ru"><p>Синдром «сухого глаза» — широко распространённое многофакторное заболевание, что обусловливает высокую актуальность поиска новых эффективных методов терапии. Новизна данной поисково-аналитической работы заключается в комплексном рассмотрении патогенеза синдрома «сухого глаза» и обосновании перспективного направления лечения с использованием биомиметиков внеклеточного матрикса роговицы. В результате проведённого анализа литературных данных установлено, что современная терапия синдрома «сухого глаза» не ограничена только простым увлажнением и должна быть направлена на восстановление гомеостаза глазной поверхности и прерывание порочного круга хронического воспаления. Кроме того, установлено, что многокомпонентные препараты на основе биомиметиков внеклеточного матрикса, в частности СФЕРО® око (АО «БИОМИР сервис», Россия), демонстрируют высокую эффективность в отношении стимуляции регенерации роговицы и являются перспективными для включения в терапевтические схемы при лечении синдрома «сухого глаза». Особое внимание уделено препаратам, одобренным Управлением по контролю за продуктами питания и лекарственными средствами США, включая циклоспорин, Xiidra® (лифитеграст 5%, Bausch &amp; Lomb Inc., США)<sup><ext-link ext-link-type="uri" xlink:href="#_ftn1">[℘]</ext-link></sup>, Eysuvis® (лотепреднол этабонат 0,25%, Kala Pharmaceuticals, США)<sup><ext-link ext-link-type="uri" xlink:href="#_ftn1">[℘]</ext-link></sup>, Miebo® (перфторгексилоктан, Novaliq, Германия)<sup><ext-link ext-link-type="uri" xlink:href="#_ftn1">[℘]</ext-link></sup>, а также назальный спрей с варениклином Tyrvaya® (Oyster Point Pharma Inc., США)<sup><ext-link ext-link-type="uri" xlink:href="#_ftn1">[℘]</ext-link></sup>.
