This study was undertaken to provide a detailed description of the temporal dynamics of choriocapillaris perfusion using a high-frequency monitoring protocol based on optical coherence tomography angiography and an improved image-analysis method aimed at minimizing visualization artifacts.</p> <p><bold>AIM:</bold> To perform a quantitative topographical analysis of changes in choriocapillaris perfusion following antiangiogenic therapy with aflibercept in patients with neovascular age-related macular degeneration.</p> <p><bold>METHODS:</bold> A prospective, single-center cohort study involved patients with newly diagnosed active type 1 macular neovascularization, who were treatment-naïve, including antiangiogenic therapy–naïve ones. Exclusion criteria were type 2 or 3 macular neovascularization; moderate-to-high myopia or hyperopia; significant ocular media opacities; concomitant systemic or ocular conditions associated with shadowing artifacts; and retinal pigment epithelium detachment height &gt; 250 µm. All participants received three intravitreal injections of aflibercept (2.0 mg) at monthly intervals. Choriocapillaris perfusion changes were assessed using a high-frequency monitoring protocol with biweekly examinations over 12 weeks. We applied an enhanced choriocapillaris image-analysis method that reduced projection artifacts and compensated for signal attenuation. Statistical analysis included repeated-measures analysis of variance and paired t-tests.</p> <p><bold>RESULTS:</bold> The mean patient age was 70.8 ± 7.1 years; women accounted for 63.0%. All 29 participants completed follow-up. By week 12, a significant reduction in the FD% was observed in all regions (ANOVA: <italic>p</italic> &lt; 0.001; paired comparisons between baseline and week 12: <italic>p</italic> &lt; 0.001); the mean relative reduction was 9.8%, with the largest changes in peripheral zones R3–R5. No treatment-related adverse events were recorded.</p> <p><bold>CONCLUSION:</bold> High-frequency monitoring using an enhanced optical coherence tomography angiography protocol confirmed a significant improvement in choriocapillaris perfusion surrounding the macular neovascularization and the associated "dark halo" in patients with neovascular age-related macular degeneration treated with aflibercept. These findings improve the understanding of the microcirculatory effects of antiangiogenic therapy and support the potential value of this approach for treatment response assessment in routine clinical practice.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Обоснование. </bold>Несмотря на доказанную эффективность интравитреального введения препаратов, ингибирующих сосудистый эндотелиальный фактор роста, механизмы их воздействия на хориоидальную микроциркуляцию, в частности на перфузионные характеристики хориокапилляров в ранние сроки после начала терапии, требуют дальнейшего изучения. Настоящее исследование проведено для детального описания динамики изменения перфузии хориокапилляров с помощью протокола высокочастотного мониторинга оптической когерентной томографии–ангиографии и усовершенствованного метода анализа, минимизирующего артефакты визуализации.</p> <p><bold>Цель исследования. </bold>Провести количественный топографический анализ изменений перфузии хориокапилляров в результате проведения антиангиогенной терапии с использованием афлиберцепта при неоваскулярной возрастной макулярной дегенерации.</p> <p><bold>Методы. </bold>Проведено проспективное одноцентровое когортное исследование. Включали пациентов с впервые выявленной активной макулярной неоваскуляризацией 1-го типа при отсутствии предшествующего лечения, включая антиангиогенную терапию. В качестве критериев невключения рассматривали наличие макулярной неоваскуляризации 2–3-го типа, миопии/гиперметропии средней и высокой степени, выраженные помутнения оптических сред, а также сопутствующую соматическую и офтальмологическую патологию с эффектом затенения, высоту отслойки ретинального пигментного эпителия &gt; 250 мкм. Все участники получили три интравитреальные инъекции афлиберцепта по 2,0 мг с интервалом 1 мес. Для оценки динамики перфузии использовали протокол высокочастотного мониторинга с двухнедельными интервалами в течение 12 нед. Применён усовершенствованный метод анализа изображений хориокапилляров со снижением влияния проекционных артефактов и компенсацией затухания сигнала. Статистический анализ включал дисперсионный анализ с повторными измерениями (Repeated-Measures ANOVA) и парный <italic>t</italic>-тест.</p> <p><bold>Результаты. </bold>Средний возраст пациентов составил 70,8 ± 7,1 года; доля женщин — 63,0%. Все 29 участников завершили наблюдение. К 12-й неделе выявлено статистически значимое снижение процента дефицита кровотока в хориокапиллярах во всех зонах (ANOVA: <italic>p</italic> &lt; 0,001; парные сравнения исходных значений и 12-й нед.: <italic>p</italic> &lt; 0,001); среднее относительное снижение составило 9,8%, наиболее выраженное в периферических зонах R3–R5. Нежелательных явлений, связанных с терапией, не зарегистрировано.</p> <p><bold>Заключение. </bold>Применение усовершенствованного протокола оптической когерентной томографии–ангиографии с высокочастотным мониторингом объективно подтвердило значительное улучшение перфузии хориокапилляров, окружающих зону макулярной неоваскуляризации и «тёмного гало», у пациентов с неоваскулярной возрастной макулярной дегенерацией на фоне лечения афлиберцептом.
