Among them, 103 patients had MI with non-obstructive CA (MINOCA) as indicated by coronaroangiography, and 103 patients had MI with obstructive CA (MIOCA). Through the random number method, 59 patients were selected (34 patients in the first group and 25 patients in the second group), and the EF and elastic properties of their arterial walls were evaluated. The patients in both groups were initially comparable in terms of age, gender, clinical, and medical history and the frequency of application of the main groups of medical drugs that influenced prognosis. The 1-year prognosis of the two groups of patients was studied on the basis of the presence/absence of the functional and morphological alterations of the vessel wall.</p> <p><bold><italic>Results.</italic></bold> The evaluation of the EF of patients with MINOCA showed that the occlusion index by amplitude (OIA) was below the threshold values in 22 of 34 (64.7%) cases of MINOCA and 22 of 25 (88.0%, р&lt;0.05) cases of MIOCA. Their average values of OIA were 1.7 (1.5; 2.3) and 1.4 (1.2; 1.8), respectively (р&lt;0.05). The values of the phase shifts between the channels below the norm were equally frequent in the two groups (88.2 and 88.0%, р&gt;0.05). The comparison of the average values of this parameter did not show any statistically significant difference. The calculated augmentation indices normalized to the pulse rate of 75 beats per min (AIp75) in the study groups were 12.5 (9.9; 17.9) and 18.8 (12.9; 20.8), respectively (р&gt;0.05). The elasticity of the vessel wall decreased in 82.4% of patients with MINOCA and 100% of patients with MIOCA (р&lt;0.05). The frequency of cardiovascular events did not significantly differ between the groups for 1 year (р&gt;0.05).</p> <p><bold><italic>Conclusion.</italic></bold> Functional changes in the vessel walls (endothelial dysfunction and reduction of vessel wall elasticity) of patients with MINOCA were recorded in almost 2/3 of the cases; however, the incidence in patients with MIOCA was still higher (88.0%) than that in patients with MINOCA. The 1-year prognosis in the study groups had no differences.</p></abstract><trans-abstract xml:lang="ru"><p><bold><italic>Цель. </italic></bold>Провести сравнительный анализ состояния функции эндотелия (ФЭ), эластичности сосудистой стенки и их влияния на годовой прогноз у больных с инфарктом миокарда (ИМ) при обструктивном и необструктивном поражении коронарных артерий (КА).</p> <p><bold><italic>Материалы и методы.</italic></bold> На первом этапе было отобрано 206 пациентов с диагнозом ИМ, из них 103 пациента без обструктивного поражения КА (ИМБОКА) по результатам коронароангиографии и 103 пациента – с ИМ и обструктивным поражением КА (ИМОКА). Методом случайных чисел было отобрано 59 пациентов (34 пациента – из первой группы, 25 – из второй), у которых проведена оценка ФЭ и эластических свойств артериальной стенки. Исходно пациенты обеих групп были сопоставимы по возрастно-половой и клинико-анамнестическим характеристикам, а также по частоте применения основных групп лекарственных препаратов, влияющих на прогноз. Изучался годовой прогноз пациентов двух групп в зависимости от наличия/отсутствия функциональных и морфологических изменений сосудистой стенки.</p> <p><bold><italic>Результаты.</italic></bold> У больных ИМБОКА при оценке ФЭ индекс окклюзии по амплитуде (ИОА) ниже пороговых значений зарегистрирован в 22 из 34 (64,7%) случаев ИМБОКА и в 22 из 25 – при ИМОКА (88,0%, р&lt;0,05). При этом, средние значения ИОА составили 1,7 (1,5; 2,3) и 1,4 (1,2; 1,8) соответственно (р&lt;0,05). Значения сдвига фаз между каналами в двух группах ниже нормальных значений встречались одинаково часто (88,2 и 88,0%, р&gt;0,05) и сравнение средних величин данного показателя статистического значимого различия также не выявило. Расчетный индекс аугментации, приведенный к пульсу 75 ударов в минуту (AIp75), в изучаемых группах составил 12,5 (9,9; 17,9) и 18,8 (12,9; 20,8) соответственно (р&gt;0,05). Снижение эластичности сосудистой стенки в группе ИМБОКА встречалось у 82,4% пациентов, при ИМОКА – в 100% случаев, (р&lt;0,05). Статистически значимых различий в частоте сердечно-сосудистых событий в течение года в группах зарегистрировано не было (р&gt;0,05).</p> <p><bold><italic>Заключение. </italic></bold>Функциональные изменения сосудистой стенки (эндотелиальная дисфункция и снижение эластичности сосудистой стенки) у пациентов с ИМБОКА были зарегистрированы почти в 2/3 случаев, однако при ИМОКА их частота еще выше (88,0%). Годовой прогноз в изучаемых группах с ИМБОКА и ИМОКА не различался.</p></trans-abstract><trans-abstract xml:lang="zh"><p><bold><italic>目的：</italic></bold>比较分析阻塞性和非阻塞性冠状动脉疾病（CA）合并心肌梗死（MI）患者的内皮功能（FE）、血管壁弹性及其对年预后的影响。</p> <p><bold><italic>材料与方法。</italic></bold>第一阶段，选择206例诊断为心肌梗死的患者，其中103例非阻塞性冠状动脉疾病（IMBOCA）患者和103例心肌梗死合并阻塞性冠状动脉疾病（IMOCA）患者。通过随机方法选择59例患者（第一组34例，第二组25例），评估了动脉壁的FE和弹性。基线检查时，两组患者在年龄、性别、临床记忆特征以及影响预后的主要药物组的使用频率方面具有可比性。对这两类患者的年度预测进行了研究，这取决于血管壁无功能和形态变化。</p> <p><bold><italic>结果。</italic></bold>MBOCA患者中，在评估FE时，34例IMBOCA患者中有22例（64.7%）振幅闭塞指数（IRA）低于阈值，25例IMBOCA患者中有22例（88.0%，p&lt;0.05）。平均IRA值分别为1.7（1.5；2.3）和1.4（1.2；1.8）（p&lt;0.05）。两组经络间的相移值低于正常值的频率相同（88.2%和88.0%，p&gt;0.05），该指标的平均值比较也未发现有统计学意义的差异。研究组中，计算出的增大指数（AIp75）分别为12.5（9.9；17.9）和18.8（12.9；20.8）（p&gt;0.05）。IMBOCA组82.4%的患者血管壁弹性降低，IMBOCA组100%的患者血管壁弹性降低（p&lt;0.05）。两组患者年内心血管事件发生率差异无统计学意义（p&gt;0.05）。</p> <p><bold><italic>结果。</italic></bold>IMBOCA患者的血管壁功能变化（内皮功能障碍和血管壁弹性降低）在近2/3的病例中得到记录，但是使用IMBOCA的患者其发生频率更高（88.0％）。IMBOCA和IMOKA研究组的年预后没有差异。</p></trans-abstract><kwd-group xml:lang="en"><kwd>myocardial infarction</kwd><kwd>MIOCA</kwd><kwd>MINOCA</kwd><kwd>endothelial function</kwd><kwd>elasticity of vessel wall</kwd><kwd>prognosis</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>инфаркт миокарда</kwd><kwd>ИМБОКА</kwd><kwd>функция эндотелия</kwd><kwd>эластичность сосудистой стенки</kwd><kwd>прогноз</kwd></kwd-group><kwd-group xml:lang="zh"><kwd>心肌梗塞</kwd><kwd>心肌梗塞 IMBOCA</kwd><kwd>内皮功能 血管壁的弹性</kwd><kwd>预报。</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><citation-alternatives><mixed-citation xml:lang="en">Matsuzawa Y, Lerman A.
