Bleeding from the upper gastrointestinal tract is one of the most dangerous complications in patients with acute CVA (ischemic and/or hemorrhagic type). The combination of these conditions requires special attention in the diagnosis and choice of treatment methods. Central Nervous System pathology makes diagnosis, choosing methods of treatment and the use of drug more complicated to surgeons in the postoperative period. With onset of hemorrhage, antithrombotic treatment should be canceled, that can negatively affect the results of treatment of patients with ischemic type of cerebral vascular accident. The use of anti-ulcer drugs, namely proton pump inhibitors, may increase the risk of hospital-acquired pneumonia, Clostridium difficile infections, and cardiovascular diseases. These factors force us to consider issues related to diagnosis and treatment plans in this group of patients. The most promising direction in the treatment of patients with ischemic or hemorrhagic stroke in combination with gastroduodenal bleeding is the identification of risk factors for upper gastrointestinal bleeding, early endoscopic diagnosis and treatment, performing preventive treatment. The article presents a brief review of domestic and foreign literature, some issues of etiology and pathogenesis of gastroduodenal bleeding in patients with cancer, the results of our own research, capabilities of modern endoscopy in the diagnosis and treatment of gastrointestinal bleeding in this group of patients. The algorithm of management of patients with a combination of bleeding from the upper gastrointestinal tract and CNS pathologies.

Кровотечения из верхних отделов желудочно-кишечного тракта являются одним из грозных осложнений на фоне острого нарушения мозгового кровообращения (ОНМК) по ишемическому и/или геморрагическому типу. Сочетание данных состояний требует особого отношения в диагностике и выборе методов лечения. Патология центральной нервной системы усложняет хирургу процесс диагностики и выбора методов лечения, применения лекарственных средств в послеоперационном периоде. При развитии геморрагии отменяется антитромботическое лечение, что отрицательно влияет на результаты лечения больных ОНМК по ишемическому типу. Применение противоязвенных препаратов, а именно ингибиторов протонной помпы, может увеличить риск внутрибольничной пневмонии, инфекций Clostridium difficile и сердечно-сосудистых заболеваний. Данные факты заставляют тщательно разработать план диагностики и лечения у данной группы пациентов. Наиболее перспективным направлением в лечении больных ОНМК по ишемическому или геморрагическому типу и гастродуоденальными кровотечениями является выявление факторов риска кровотечения из верхних отделов желудочно-кишечного тракта, ранняя диагностика и лечение с применением эндоскопии, и проведение профилактического лечения. В статье представлен краткий обзор отечественной и зарубежной литературы, некоторые вопросы этиологии и патогенеза гастродуоденальных кровотечений у больных ОНМК, результаты собственных исследований, возможности современной эндоскопии в диагностике и лечении желудочно-кишечных кровотечений у данной группы больных, а также алгоритм ведения больных при сочетании кровотечений из верхних отделов желудочно-кишечного тракта с патологией центральной нервной системы.
