This virus replicates in B-lymphocytes but may be as well found in the epithelial cells of pharynx and parotid ducts. Its typical symptoms include fever, lymphadenopathy and herpetic angina, the endocrine system may also be involved in the pathologic process.</p> <p><bold>AIM: </bold>The aim of the study is the change in cortisol levels in children of different sex and age during the peak and convalescence of infectious mononucleosis caused by the Epstein–Barr virus.</p> <p><bold>MATERIALS AND METHODS:</bold> Under observation were 94 children aged 1 to 17 years, among them from 1 to 5 years — 22%, from 5 to 10 years — 39%, from 10 to 15 years — 27%, from 15 to 17 years — 12%, with a diagnosis of Epstein–Barr virus – mononucleosis moderate severity. The diagnosis was made on the basis of clinical, anamnestic and laboratory data. Studies confirming the diagnosis were carried out in the laboratory of St. Petersburg State Pediatric Medical University. In addition to routine examinations (clinical minimum), determination of M- and G- antibody titers to the Epstein–Barr virus, detection of the Epstein–Barr virus genome in the blood by PCR, and determination of cortisol levels by enzyme-linked immunosorbent assay in blood serum.</p> <p><bold>RESULTS: </bold>In all age groups, the cortisol concentration during the peak period was higher than during the convalescence period. At the same time, in girls in groups from 6 to 10 years old and from 11 to 16 years old, this difference was significant (1045.19 ± 111.10 versus 740.97 ± 120.68 nM/l and 977.31 ± 81.40 versus 735.48 ± 113.04 nM/l respectively). In boys of all age groups, the differences were significant and amounted to 1211.30 ± 128.36 versus 864.63 ± 126.90 nM/l in the group from 1 to 5 years old, and 1066.76 ± 39.01 versus 817,90 ± 101,52 nM/l in the age group from 6 to 10 years old, in the age group from 11 to 16 years 950.14 ± 101.54 versus 779.61 ± 125.68 nM/l.</p> <p><bold>CONCLUSIONS:</bold> Changes in cortisol levels in children during the peak of infectious mononucleosis depend on the severity of the disease, the presence and nature of complications. The level of cortisol determines various options for adaptation to infectious mononucleosis.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Актуальность. </bold>Инфекционный мононуклеоз — острое инфекционное заболевание, вызываемое вирусом Эпштейна – Барр. Этот вирус реплицируется в основном в В-лимфоцитах, но также может быть обнаружен в эпителиальных клетках глотки и околоушных протоков. Типичные признаки и симптомы включают лихорадку, лимфаденопатию и герпетическую ангину, в патологический процесс также может вовлекаться и эндокринная система.</p> <p><bold>Цель</bold> — изучить изменение уровня кортизола у детей различного пола и возраста в период разгара и реконвалесценции инфекционного мононуклеоза, вызванного вирусом Эпштейна – Барр.</p> <p><bold>Материалы и методы. </bold>Под наблюдением находились 94 ребенка в возрасте от 1 до 17 лет, среди них от 1 до 5 лет — 22 %, от 5 до 10 лет — 39 %, от 10 до 15 лет — 39 %, от 15 до 17 лет — 12 %, с диагнозом «вирус Эпштейна – Барр — мононуклеоз», среднетяжелая степень тяжести. Диагноз выставлен на основании клинико-анамнестических и лабораторных данных. Исследования, подтверждающие диагноз, проведены в лаборатории Санкт-Петербургского государственного педиатрического медицинского университета. Помимо рутинных обследований (клинический минимум), определения титров антител M и G к вирусу Эпштейна – Барр, выявление генома вируса Эпштейна – Барр в крови методом полимеразной цепной реакции определен уровень кортизола методом твердофазного иммуноферментного анализа в сыворотке крови.</p> <p><bold>Результаты.</bold> Во всех возрастных группах концентрация кортизола в период разгара заболевания была выше, чем в период реконвалесценции. При этом у девочек в группах от 6 до 10 лет и от 11 до 16 лет это различие было достоверно (1045,19 ± 111,10 против 740,97 ± 120,68 нмоль/л и 977,31 ± 81,40 против 735,48 ± 113,04 нмоль/л соответственно). У мальчиков всех возрастных групп различия были достоверны и составили в группе от 1 до 5 лет 1211,30 ± 128,36 против 864,63 ± 126,90 нмоль/л, в возрастной группе от 6 до 10 лет — 1066,76 ± 39,01 против 817,90 ± 101,52 нмоль/л, в возрастной группе от 11 до 16 лет — 950,14 ± 101,54 против 779,61 ± 125,68 нмоль/л.</p> <p><bold>Выводы. </bold>Изменение уровня кортизола у детей в период разгара инфекционного мононуклеоза зависит от тяжести заболевания, наличия и характера осложнений.
