The main criterion for selection of patients was the presence of thumb adduction contracture, the absence of significant positive outcome in a patient after conservative treatment, the impossibility of active thumb abduction more than 30° and the instability of the first metacarpophalangeal joint. Total we examined and treated 9 patients with spastic types of cerebral palsy.</p><p><bold>Results and conclusions</bold>.</p><p>We evaluated the results of the following types of surgical treatment: release of adductor pollisis muscle, release of adductor pollisis muscle and tendon shortening of m.abductor pollicis longus, release of adductor pollisis muscle and tendon transfer of m.extensor indicis in the tendon m. abductor pollicis longus, the fixation of the first metacarpophalangeal joint with extramedullary plate. Based on these data, we were able to confirm the effectiveness of surgical treatment of thumb adduction contracture.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Целью</bold> работы являлась оценка эффективности различных методов хирургического лечения сгибательно-приводящей контрактуры первого пальца у детей с детским церебральным параличом.</p><p><bold>Материалы и методы</bold>.</p><p>Настоящее исследование основано на результатах обследования детей, страдающих детским церебральным параличом с поражением верхней конечности. Основными критериями отбора пациентов являлись наличие сгибательно-приводящей контрактуры первого пальца, отсутствие у пациента значимого положительного результата от консервативного лечения, невозможность активного отведения первого пальца кисти более 30° и нестабильность первого пястно-фалангового сустава. Всего обследовали и пролечили 9 пациентов со спастическими формами церебрального паралича.</p><p><bold>Результаты и выводы</bold>.</p><p>Мы оценили результаты следующих видов хирургического лечения: релиз приводящей первый палец мышцы, релиз приводящей первый палец мышцы и укорочение сухожилия m. abductor pollicis longus, релиз приводящей первый палец мышцы и пересадка сухожилия разгибателя второго пальца кисти (m. extensor indicis) на сухожилие m. abductor pollicis longus, фиксация первого пястно-фалангового сустава накостной пластиной.
