The choice of the term and method of surgical treatment of congenital spinal deformities with vertebral malformations does not have diametrically opposed views in the domestic and foreign literature, but the scientific discussion continues regarding the use of orthotics. Currently, several designs of orthoses on the trunk are known worldwide, which can be used not only for idiopathic but also for congenital scoliosis. А unified view of the use of this type of complex treatment is necessary, but there are opposite opinions in the scientific literature.</p> <p><bold><italic>AIM:</italic></bold> This study aimed to analyze publications that present the use of torso orthoses in the treatment of congenital malformations of the spine.</p> <p><bold><italic>MATERIALS AND METHODS:</italic></bold> We searched for data in the open electronic databases of PubMed and e-LIBRARY using the following keywords and phrases: congenital deformity of the spine, congenital scoliosis, orthosis, and effectiveness of orthosis. The search covered 30 years of literature.</p> <p><bold><italic>RESULTS:</italic></bold> Several authors have reported a significant correction of the congenital curves using orthotics, but this issue has not been sufficiently studied and is still at the research stage. Some specialists consider the formation or progression after the operation of compensatory counter-curve above or below the established metal structure, which can be corrected not only by repeated surgery, but also by using orthoses.</p> <p><bold><italic>CONCLUSIONS:</italic></bold> There is a clear trend toward complex treatment of children with isolated congenital malformations of the spine, including a reasonable combination of surgical treatment and orthotics. In Europe and Russia, preference is given to the Rigo system Cheneau brace.</p></abstract><trans-abstract xml:lang="ru"><p><italic><bold>Обоснование</bold></italic>. Несмотря на невысокую долю врожденных аномалий развития позвоночника в структуре общей ортопедической патологии, их лечение не теряет своей актуальности. Если взгляды на сроки и способы хирургического лечения врожденных деформаций позвоночника на фоне пороков развития позвонков не различаются в отечественной и зарубежной литературе, то в отношении средств ортезирования туловища дискуссия продолжается. Известен целый ряд конструкций ортезов на туловище, которые применяют не только при идиопатическом, но и при врожденном сколиозе. Однако в связи с отсутствием масштабных исследований, основанных на принципах доказательной медицины, единого взгляда на использование этого изделия при врожденном сколиозе нет, и в научной литературе встречаются противоположные мнения.</p> <p><bold>Цель</bold> — проанализировать публикации, в которых представлено применение ортезов на туловище при лечении врожденных аномалий развития позвоночника.</p> <p><italic><bold>Материалы и методы</bold></italic>. Осуществлен поиск данных в открытых электронных базах научной литературы PubMed и еLIBRARY по ключевым словам и словосочетаниям: «врожденная деформация позвоночника», «врожденный сколиоз», «ортезирование», «ортез на туловище», «эффективность ортезирования» (congenital spine deformity, congenital scoliosis, brace treatment, spinal brace, effectivity of brace). Глубина поиска составила 30 лет.</p> <p><italic><bold>Результаты</bold></italic>. Незначительное количество публикаций посвящено консервативному лечению врожденных деформаций позвоночника с применением ортезов на туловище, которое в основном рассматривают как необходимую для хирургического лечения отсрочку. Ряд авторов сообщают о значительной коррекции врожденной дуги с помощью средств ортезирования, но отмечают, что этот вопрос недостаточно изучен и находится на этапе исследований. Мнения об использовании ортезов на туловище после хирургического лечения врожденного сколиоза диаметрально противоположны. Некоторые исследователи категорически против применения ортезов на туловище после операции. Часть специалистов считают, что компенсаторные противодуги, которые формируются или прогрессируют после операции выше или ниже установленной металлоконструкции, могут быть корригированы не только путем повторной операции, но и путем ортезирования.</p> <p><italic><bold>Заключение</bold></italic>. Прослеживается четкая тенденция к комплексному лечению детей с врожденными изолированными аномалиями развития позвонков, включающему обоснованное сочетание хирургического лечения и ортезирования.
