Лечение детей с переломами дистального метаэпифиза лучевой кости (обзор литературы)

Аннотация
Обоснование. Переломы дистального метаэпифиза лучевой кости относятся к наиболее частым травмам у пациентов детского возраста. Это обусловлено особенностями анатомического строения и структуры костной ткани лучевой кости в данной области.
Цель — проанализировать литературу по вопросам лечения детей с переломами дистального метаэпифиза лучевой кости.
Материалы и методы. Изучена литература, посвященная вопросам лечения детей с переломами дистального метаэпифиза лучевой кости. Поиск данных проводили в базах научной литературы PubMed, eLibrary, Google Scholar без языковых ограничений за период с 1990 по 2024 г.
Результаты. При переломах дистального метаэпифиза лучевой кости без смещения осуществляют иммобилизацию конечности. При переломах со смещением проводят закрытую репозицию и/или хирургическое лечение. В отношении стабильных переломов специалисты придерживаются консервативной тактики. Методом выбора при оперативном лечении нестабильных переломов должен быть минимально инвазивный остеосинтез. На сегодняшний день золотым стандартом хирургического лечения переломов дистального метаэпифиза лучевой кости у детей являются закрытая репозиция и интрамедуллярный металлоостеосинтез спицами Киршнера с последующим наложением гипсовой повязки.
Заключение. В настоящее время отсутствует единое мнение в вопросе выбора метода хирургического лечения переломов дистального метаэпифиза лучевой кости у детей, результаты лечения детей по представленным оперативным методикам противоречивы, отмечены высокие риски развития интра- и послеоперационных осложнений. Учитывая вышеизложенное, на сегодняшний день сохраняется актуальность проведения исследований в данной области и разработки новых, наиболее эффективных методов хирургического лечения, позволяющих снизить риск осложнений и оптимизировать процесс реабилитации данной категории пациентов.
