TRAUMA AND ORTHOPEDIC MORBIDITY OF CHILDREN IN RUSSIA IN 2013. ORGANIZATION OF SPECIALIZED CARE FOR CHILDREN

Abstract

The article is a summary of the materials presented at the X Jubilee Congress of Orthopedic and Trauma Surgeons of Russia on September 16, 2014, in Moscow by the director of the FSBI «Scientific and Research Institute for Children’s Orthopedics n. a. G. I. Turner», corr.-member of RAS Alexey Georgievich Baindurashvili. Statistical data on traumatism and morbidity of the musculoskeletal system of the child population, information on the organization of trauma and orthopedic care for children are based on data from the state statistical reports of the Russian Ministry of Health. These data may be useful to public health officials, and orthopedic and trauma surgeons in subjects of the Russian Federation.

Abstract

The scientifically based organizational system of diagnosis, treatment and rehabilitation of children with fractures of the vertebrae is accepted in the medical institutions of St. Petersburg, including inpatient, sanatorium and outpatient care. Stages of conservative treatment in patients with vertebral compression fractures in the emergency departments of children’s general hospitals in St. Petersburg are reported, the St. Petersburg State budgetary institution of health care Children’s Rehabilitation Center of Orthopedics and Traumatology “Ogonyok” and children’s outpatient clinics. The algorithm is presented in details to provide advice and high-tech The St. Petersburg SFHI Children’s Rehabilitation Center of Orthopedics and Traumatology “Ogonek”, Saint-Petersburg surgical emergency care to children with unstable and complicated fractures of the vertebrae, which is carried out by experts of the Federal Children’s Center of spine and spinal cord injuries at the base of the department of spinal pathology and neurosurgery of FSBI “Scientific and Research Institute for Children’s Orthopedics n. a. G. I. Turner” under the Ministry of Health of the Russian Federation.

MODERN CONCEPTS OF CONSERVATIVE TREATMENT METHODS OF CLUBFOOT

Abstract

Conservative treatment of congenital clubfoot is generally accepted standard in the world orthopedic practice. There are many techniques that basically include functional methods and techniques of passive correction of the deformity. We analyzed 10 years of experience in the treatment of primary clubfoot according to three techniques - Zatsepin’s method, author’s method and Ponseti method. The evaluation of treatment results in the short and long terms was carried out. Analysis of long-term results showed a statistically significant advantage of Pontseti method over the other used techniques.

THE USE OF ROBOTIC MOTOR REHABILITATION IN COMPLEX TREATMENT OF SUBLUXATION AND DISLOCATION OF THE HIP IN CHILDREN WITH SEQUELAE OF SPINA BIFIDA

Abstract

The article presents the results of rehabilitation in children with sequelae of spina bifida using the robotic complex “Pediatric Lokomat” produced by «Hocoma» company (Switzerland). In such patients, one of the most significant orthopedic problems in frequency and clinical relevance is the instability of the hip joint. The approach to rehabilitation treatment in children after reconstructive surgery of the hip involves the principle “early motion - late weight bearing”. Conventional methods of rehabilitation treatment are performed in supine position, and don’t allow to combine this principle with training the skills of vertical posture, which leads to the development of osteoporosis and the risk of pathological fractures. Robotic mechanotherapy enables to cope with this problem by working in isokinetic mode with no load on the supporting surface.

The results of a complex evaluation and treatment of children with chronic osteomyelitis in age from 7 months to 18 years. Defines the methods of using the several forms of CollapAns. Minimal invasiveness the method of the using CollapAn-gel allows to implement surgery in detection of pathological center near zone growth. Almost all children have comorbid diseases and latent bacterial and viral infections. Therefore, surgery is necessary to supplement a full treatment of comorbidity.

Abstract

Bone reconstruction is one of the most topical problems of orthopedics and traumatology. According to the literature, search for the best methods and materials for bone grafting is carried out for a long time and is currently ongoing. The development of microsurgery provided new opportunities of bone remodeling, especially in pediatric orthopedics. However, as with any method, the predominance of the positive aspects of this method is accompanied by the existence of flaws. So the question of bone grafting remains relevant and debatable.

Abstract

Hip dysplasia in children is the subject of careful study by specialists both in our country and abroad. There are three basic variants of hip dysplasia: congenital acetabular dysplasia, congenital subluxation of the hip and congenital dislocation of the hip. However, treatment of congenital dislocation of the hip, which is the most severe form of hip dysplasia in children, currently remains one of the topical problems of children’s orthopedics, despite the significant achievements of modern medicine. In spite of the large amount of research in this field, treatment principles of hip dysplasia are still under discussion.

CHRONIC TRAUMATIC DISLOCATION OF II-V METACARPAL BONES: CASE REPORT

Abstract

Dislocations in the carpometacarpal joints of three-phalanx fingers are rare form of injury. Their clinical manifestations are often veiled by swelling, and radiographs in standard views provide little information, which leads to difficulty in diagnosis and a high incidence of unidentified dislocations in the primary treatment. The article describes the basic provisions of the diagnosis and treatment of this type of injury, a clinical case of surgical treatment of undiagnosed dislocations of II-V metacarpal bones in the acute period, and long-term results of treatment.
